Epidural corticosteroid injections for sciatica: a randomised, double blind, controlled clinical trial.

OBJECTIVE: To determine the efficacy of epidural corticosteroid injections for sciatica. METHODS: Three epidural injections (two day intervals) of 2 ml prednisolone acetate (50 mg) or 2 ml isotonic saline were administered to patients with sciatica presumably due to a disk herniation lasting 15-180 days. Self evaluation was the main judgment criterion at day 20. Patients who recovered or showed marked improvement were considered as success. Pain measured by VAS, the SLR test, Schober's test, Dallas pain questionnaire, and the Roland-Morris index were evaluated at days 0, 5, 20, and 35. Only a

Epidurography and therapeutic epidural injections: technical considerations and experience with 5334 cases.

BACKGROUND AND PURPOSE: Even in experienced hands, blind epidural steroid injections result in inaccurate needle placement in up to 30% of cases. The use of fluoroscopy and radiologic contrast material provides confirmation of accurate needle placement within the epidural space. We describe our technique and experience with contrast epidurography and therapeutic epidural steroid injections, and review the frequency of systemic and neurologic complications. METHODS: Epidural steroid injections were performed in 5489 consecutive outpatients over a period of 5 1/2 years by three procedural neuro

Epidural Steroid Injections for Lumbar Disc Herniation and Lumbar Degenerative Disc Disease.

INTRODUCTION: Patellofemoral osteoarthritis (PFOA) is a common source of anterior knee pain and functional limitation, affecting quality of life and mobility in older adults. While conventional exercise remains central to conservative management, adjunctive modalities such as laser therapy and longwave diathermy are being investigated for their added benefits. OBJECTIVES: The objective of the study is to compare the effectiveness of conventional exercises combined with laser therapy versus conventional exercises combined with longwave diathermy in reducing pain and enhancing functional outcom

Transverse plane ultrasound-guided caudal epidural injections: sonographic anatomy and stepwise technique.

PURPOSE: The present study aims to provide a step-by-step procedural and anatomical familiarization guide for transverse plane ultrasound (US)-guided caudal epidural (CE) injection. METHODS: The study cohort consisted of 23 chronic low back pain patients (23-67 years old) previously unresponsive to conservative management. A transverse plane US-guided CE injection was performed, with each procedure step documenting and emphasizing sonographic anatomy. Several Thiel's method fixed cadaveric specimen dissections were also performed to demonstrate relevant CE injection-related anatomy. RESULTS:

Ultrasound-Guided Lumbar Transforaminal Epidural Injections; A Single Center Fluoroscopic Validation Study.

OBJECTIVE: To investigate the success rate (technical precision) of ultrasound-guided lumbar transforaminal epidural steroid injection, which was validated by conventional fluoroscopic technique. METHODS: A total of 20 patients with unilateral single-level lumbar foraminal disc protrusion causing radiculopathy were enrolled. Using transforming route, the needle location was determined by an axial (transvers) view of the ultrasound with fluoroscopic confirmation. We determined the needle placement accuracy of ultrasound- guided lumbar transforaminal injections approach. RESULTS: The accuracy

The retroneural approach: an alternative technique for lumbar transforaminal epidural steroid injections.

BACKGROUND: Compared with other available injection techniques for lumbar transforaminal epidural steroid injections (LTFESIs), the traditionally performed subpedicular approach is associated with a higher risk of spinal cord infarction, a rare but catastrophic complication. PURPOSE: To assess the short-term efficacy of the retroneural approach for computed tomography (CT)-guided LTFESIs with respect to different needle-tip positions. MATERIAL AND METHODS: This retrospective analysis included 238 patients receiving 286 CT-guided LTFESIs from January 2013 to January 2016. Short-term outcomes

Immediate and acute adverse effects following transforaminal epidural steroid injections with dexamethasone.

BACKGROUND: Transforaminal epidural steroid injections (TFESI) are widely used for the conservative treatment of radicular pain. The use of dexamethasone in TFESIs is relatively new; therefore, immediate and acute adverse effects that it may cause are not fully updated. OBJECTIVE: To evaluate immediate and acute adverse effects following TFESI with dexamethasone. STUDY DESIGN: Prospective, observational study. SETTING: A spine center affiliated with a rehabilitation hospital. METHODS: One hundred fifty consecutive patients receiving TFESI for the management of radicular and axial spinal pa

[Efficacy of transforaminal lumbar epidural steroid injections in patients with lumbar radiculopathy].

OBJECTIVES: This study looks into the efficacy and safety of the transforaminal lumbar epidural steroid injection (TLESI) applied to patients with radiculopathy due to lumbar disk herniation. METHODS: The patients' files which were applied TLESI, were retrospectively scanned. Patients who did not respond to one-month conservative treatment and who were detected to have bulging or protruding lumbar disk herniation as a result of imaging methods were included in the study. All applications were performed with C-arm fluoroscopy under local anesthesia by outpatient method. In all cases, a mix of

The efficacy of lumbar epidural steroid injections: transforaminal, interlaminar, and caudal approaches.

Conservative management of low back pain includes the use of lumbar epidural steroid injections, which have become increasingly more popular in the last 20 years. The body of literature regarding the efficacy of these injections is vast, conflicting, and difficult to summarize. This article reviews the updated evidence for efficacy and the indications for these injections.

Diagnosing the unrecognized systemic absorption of intra-articular and epidural steroid injections.

OBJECTIVE: To present a case series of patients who were misdiagnosed with endocrine disorders because of failure to recognize the systemic absorption of intra-articular and epidural steroids and to discuss the utility of performing a urine screen to detect synthetic glucocorticoids. METHODS: In this case series, we describe the clinical, laboratory, and imaging findings of 3 patients referred to our clinic, each with a presumed endocrine disorder. RESULTS: Patient 1 was a 54-year-old woman with weakness, loss of appetite, and a hormonal profile suggestive of hypopituitarism. Patient 2 was a
