The management of intrauterine devices following uterine perforation.

The generally accepted indications for removal of an intra-abdominal intrauterine device (IUD) are: a) a closed variety of IUD, b) medicated IUDs with copper or hormone release, and c) a medico-legal or psychological problem. A perforated IUD can be left in the peritoneal cavity if it is made of non-irritating plastic. The copper IUD causes adhesions, but these lead to few cases of serious complications. The reason for removal of the IUD stems only from the fear of adhesion formation. Our experience, supported by the literature, leads us to believe that this indication should no longer be acce

Auger and pixe microanalysis of intrauterine devices (IUDs).

Auger Emission Spectroscopy (AES) and Particle Induced X-ray Emission (PIXE) analysis were employed in a study on the calcification and oxidation processes of the copper wires used in Intra-Uterine Device (IUDs) contraceptives. A strong copper release (approximately 50 micrograms/day) has been observed in the first year of IUDS application during the high device antifertility effect. The copper release is reduced with the IUD-use time, because of the calcification process on the surface by organic liquids. This effect produces a drastic reduction in the contraceptive efficiency, and a new IUDs

Morphological characteristics of incrustates of intrauterine contraceptive devices.

The morphology of incrustates formed on intrauterine contraceptive devices (IUD) is described. It is demonstrated that due to manufacturing technology some IUDs have long, parallel excavations serving as sites for incrustate formation. A regional difference of incrustation on the different parts of IUD has also been observed. Electron microscopy was carried out in tissue fragments attached to removed IUDs. Alterations correspond to acute or chronic inflammation. Incrustate on the surface of IUD appears to have a primary role in induction of the process.

Management of translocated and incarcerated intrauterine contraceptive devices.

Thirty-seven consecutive patients who reported with absent or snapped strings and failed attempts at removal of intrauterine contraceptive devices (IUCD) by hooks or curette were posted for hysteroscopy/laparotomy following a plain X-ray of the pelvis to exclude unrecognized spontaneous expulsion. Direct visualization of the endometrial cavity was invaluable in locating and removing IUCDs, particularly the embedded and fragmented ones. Translocated medicated devices were associated with dense adhesions, suggesting the need for their prompt removal. There appears to be an increased risk of tran

Colposcopy in the localization of missing intrauterine devices.

The use of colposcopy as an office procedure is shown to be an effective method in localizing the IUD's string deep in the cervical canal, and for following those patients who wish to continue with IUD, thus avoiding replacement of the device. In 24 patients with missing string, colposcopy was used, and was found to be an effective procedure which obviates the use of unnecessary x-ray exposure, invasive procedures and expensive ultrasonography.

[Analysis of surface deposits on intrauterine contraceptive devices].

The surface deposits on stainless steel contraceptive devices removed from the uterus after various periods of insertion were examined and analyzed by means of scanning electron microscopy, X-ray energy dispersive analysis, Augar electron spectroscopy, X-ray photoelectron spectroscopy, transmission electron microscopy and atomic absorption spectrophotometer for microquantitative analysis. The data obtained were compared with those of copper-bearing IUDs (TCu 200) obtained by the same methods. The wire coils of stainless steel IUDs which had been in situ for 5 years showed on evident changes bu

Pelvic actinomycosis and usage of intrauterine contraceptive devices.

Pelvic inflammatory disease (PID) is one of the most commonly encountered serious infectious disease entities in gynecology. The past decade … nterest is the reported observation of an increased frequency of salpingo-oophoritis among users of intrauterine devices (IUDs). These reports have noted the specific occurrence of serious pelvic infections due to Actinomyces species, and this will be the topic of the infectious disease conference. Our patient presented with a chronic illness characterized by lethargy …

Pseudo-sulfur granules associated with intrauterine contraceptive devices.

A structure resembling a sulfur granule found in histologic sections of endometrium from patients with intrauterine contraceptive devices is described. It is distinguished from a true sulfur granule on morphologic grounds and is further characterized as a foreign substance, most probably fragments of the synthetic contraceptive device. It is not associated with clinical disease. Microbiologic cultures of removed intrauterine contraceptive devices show no consistent or predominant growth pattern.

Pelvic actinomycosis in women using intrauterine contraceptive devices.

Several recent reports have indicated the possible association between pelvic infection caused by Actinomyces and the use of intrauterine contraceptive devices. Seven cases of infection or colonization of the female genital tract have been detected among women using intrauterine contraceptive devices (IUD's) at Grady Memorial Hospital, Atlanta, Georgia, from March, 1975, until May, 1977. No single IUD type has been incriminated. The shortest duration of consecutive IUD use before the diagnosis was two and a half years. Six of these cases were detected incidentally at the time of endometrial or

Hormonal intrauterine devices and heat exchange during exercise.

Synthetic progestins in oral contraceptives are thought to blunt heat dissipation by reducing skin blood flow and sweating. However, whether progestin-releasing intrauterine devices (IUDs) modulate heat loss during exercise-heat stress is unknown. We used direct calorimetry to measure whole-body total (dry + evaporative) heat loss in young, physically active women (mean (SD); aged 24 (4) years, V ̇ O 2 peak ${\dot V_{{{\mathrm{O}}_{\mathrm{2}}}{\mathrm{peak}}}}$ 39.3 (5.3) ml/kg/min) with (IUD; n = 19) and without (Control; n = 17) IUDs in the follicular and luteal phases of the menstrual …

Levonorgestrel vs. Copper Intrauterine Devices for Emergency Contraception.

BACKGROUND: In the United States, more intrauterine device (IUD) users select levonorgestrel IUDs than copper IUDs for long-term contraception. Currently, clinicians offer only copper IUDs for emergency contraception because data are lacking on the efficacy of the levonorgestrel IUD for this purpose. METHODS: This randomized noninferiority trial, in which participants were unaware of the group assignments, was conducted at six clinics in Utah and included women who sought emergency contraception after at least one episode of unprotected intercourse within 5 days before presentation and agreed

Dislocated intrauterine devices: clinical presentations, diagnosis and management.

OBJECTIVE: Intrauterine devices (IUDs) are globally one of the most popular methods of contraception. Uterine perforation is one of the most significant complications of IUD use and commonly occurs at the time of IUD insertion rather than presenting as delayed migration. This paper reports a series of 13 cases of displaced IUDs requiring retrieval by laparoscopy or laparotomy. All the IUDs were copper bearing and most perforations occurred immediately after IUD insertion. CASES: In two patients with sigmoid colon injury and IUD penetration of the appendix, laparoscopic management had failed a…

Mislocated extrauterine intrauterine devices: Diagnosis and surgical management.

OBJECTIVE: Presentation of the diagnostic and surgical treatment methods of our extrauterine intrauterine device (IUD) cases. MATERIAL AND METHODS: We retrospectively evaluated the data of 21 extrauterine IUD cases at our clinic between 2008 and 2010. The symptoms, diagnostic methods, and surgical treatments were evaluated. RESULTS: A total of 14 copper and seven levonorgestrel (LNG) IUDs were used. IUD had been inserted during lactation in 71.4% of the patients. The reasons for presentation of patients were unintended pregnancy in 19.05%, pelvic pain in 19.05%, and pelvic pain with vaginal
