Epithelial iris cyst after cataract surgery.

We report a case of an epithelial inclusion cyst of the iris following cataract surgery that was successfully treated with en bloc excision, after an unsuccessful attempt with Neodymium-doped Yttrium Aluminium Garnet (Nd YAG) Laser. A 60-year-old man had undergone cataract surgery two years back. One year later, he developed a pigmented epithelial inclusion cyst of the iris which progressively increased in size. His vision reduced to finger counting close to face as the cyst grew over the pupil. We performed Nd YAG laser cystotomy of the cyst wall initially, but the treated lesion recurred. So…

HIV & immune reconstitution inflammatory syndrome (IRIS).

Antiretroviral therapy (ART) initiation in HIV-infected patients leads to recovery of CD4+T cell numbers and restoration of protective immune responses against a wide variety of pathogens, resulting in reduction in the frequency of opportunistic infections and prolonged survival. However, in a subset of patients, dysregulated immune response after initiation of ART leads to the phenomenon of immune reconstitution inflammatory syndrome (IRIS). The hallmark of the syndrome is paradoxical worsening of an existing infection or disease process or appearance of a new infection/disease process soon a…

[Iris-fixated intraocular lenses: reinforced monitoring].

In 1986, the concept of the claw lens was applied to correct myopia in phakic patients. Since then, progress has made iris-fixated phakic intraocular lenses (IOL) relatively safe, predictable, and effective for the correction of myopia, hyperopia, and astigmatism. All these models have undergone a series of design improvements to prevent complications. Despite having excellent refractive results, the principal risk is a potential progressive endothelial cell loss. Many authors have presented encouraging results. Phakic iris-fixated IOL surgery is a potentially reversible procedure, but the sur…

[Iris heterochromia in acquired Horner's syndrome].

Horner's syndrome (HS) is related to an interruption of the oculosympathetic nerve pathway. The classic clinical findings associated with this condition are ptosis, miosis, and enophthalmos. Heterochromia is typically described in congenital HS, but it is an uncommon finding in acquired HS. We report a case of post-traumatic HS associated with heterochromia. A literature review indicates that this type of heterochromia may be related to a reduction in the number of iris melanocytes. This mechanism may be the same in the physiological iris color modifications in adulthood.

Proton beam therapy for iris melanomas.

AIMS: To describe the results in terms of local control, eye preservation and systemic evolution of iris melanomas treated by proton beam irradiation. METHODS: Retrospective review of the charts of patients with iris melanoma treated by proton beam therapy between April 1998 and September 2002. Ciliary body melanomas with iris involvement or tumours with extrascleral invasion were excluded. Treatment consisted of 60 Gy of proton beam irradiation delivered in four fractions to the tumour volume. RESULTS: A total of 21 patients were treated, median follow-up of 33 months (8-72 months). 15 pati…

Proton beam radiotherapy of iris melanoma.

PURPOSE: To report on outcomes after proton beam radiotherapy of iris melanoma. METHODS AND MATERIALS: Between 1993 and 2004, 88 patients with iris melanoma received proton beam radiotherapy, with 53.1 Gy in 4 fractions. RESULTS: The patients had a mean age of 52 years and a median follow-up of 2.7 years. The tumors had a median diameter of 4.3 mm, involving more than 2 clock hours of iris in 32% of patients and more than 2 hours of angle in 27%. The ciliary body was involved in 20%. Cataract was present in 13 patients before treatment and subsequently developed in another 18. Cataract had a…

Incomplete and complete plateau iris syndrome.

Plateau iris syndrome is considered to be a rare entity and has been classified into two subtypes, the complete and the incomplete forms. Herein a case is reported of plateau iris syndrome that had a postmydriatic rise in intraocular pressure (IOP) in one eye (complete syndrome) and no increase in IOP in the other (incomplete syndrome). The pre- and post-mydriasis ultrasound biomicroscopic findings were documented. These findings suggest that incomplete plateau iris syndrome may be caused by a difference in the extent of closure of the angle circumferentially over the 360 degrees of the angle.

Conventional diagnostic ultrasound of iris lesions.

Eighty patients, referred for further evaluation of prominent iris lesions, were examined with conventional B-mode ultrasound equipment combined with a simplified immersion technique. The axial resolution of this system is about 350 microns. In 20.3% of the cases, the lesion was too small for ultrasound detection. A cystic lesion was diagnosed in 26% and a solid lesion in 53.7% of the patients studied. In 33.4% of the cases, the lesions were not confined to the iris but also appeared in the ciliary body. This last result shows the diagnostic value of the conventional ultrasound examination in …

Laser treatment of iris vascular tufts.

We describe a case of iris vascular tufts which came to our attention because of a hyphaema. Laser treatment was carried out for the vascular anomalies. After a period of 3 months, fresh vascular anomalies had appeared in different locations. Further treatment led to permanent obliteration. We emphasize the importance of fluoro-iridography in the monitoring of vascular anomalies as it allows accurate targeting treatment.

Ultrasound biomicroscopy in plateau iris syndrome.

Ultrasound biomicroscopy is a new technique that uses high-frequency ultrasound (50 to 100 MHz) to produce images of the entire anterior segment at high resolution (20 to 50 microns). The iridocorneal angle, iris, ciliary body, and posterior chamber can be imaged in detail and the dimensions and anatomic relationships of these structures determined. Plateau iris syndrome is a condition in which the angle remains appositionally closed or occludable after iridectomy for angle-closure glaucoma. How the iris remains in a position that allows it to occlude the angle has been uncertain. We performed…

Iris autonomic function in acute glaucoma.

Iris autonomic function was studied by binocular infrared pupillometry in 12 patients with acute primary angle closure glaucoma (APACG) and 28 control subjects. Light reflex amplitude and resting darkness pupil diameter were reduced and miotic response to pilocarpine 0.125% drops increased in APACG when compared to controls with anterior chamber depths (ACD) greater than 2.5 mm but not when compared to controls with ACD of 2.5 mm or less. These findings suggest relative autonomic dysfunction, predominantly parasympathetic, in the anterior segment of the eye in APACG. Correlation between result…

Iris transillumination defect and microhyphema syndrome.

We present a previously undescribed delayed complication attributable to sulcus-fixated posterior chamber lenses with elliptical polypropylene haptics containing a 10 degrees anterior angulation. Clinical signs of this complication are crescent-shaped iris transillumination defects overlying the lens haptics in the peripheral iris; in some cases these are associated with single or recurrent visually significant microhyphemas. This series describes 41 eyes that contain these transillumination defects; eight of the eyes have had lens-induced intraocular hemorrhage. We estimate the overall incide…

Late iris abscess after cataract extraction.

A 73-year-old woman underwent an uneventful intracapsular lens extraction. Two years later, an iris abscess, hypopyon, strong flare, corneal edema, and elevated intraocular pressure developed. Visual acuity diminished from 6/6 (20/20) with aphakic correction to hand movements. Gram-positive cocci sensitive to erythromycin and chloramphenicol were cultured from the anterior chamber. The hypopyon and the iris abscess reabsorbed because we administrated erythromycin at the beginning of the illness. Final visual acuity was 6/6 (20/20) and only a small hole remained in the iris at the site of the a…

From Crisis to Clarity: Retropupillary Iris-Claw Lens and Iris Cerclage Techniques for Post-traumatic Cataract Rehabilitation.

This study details two cases of traumatic cataracts with a history of blunt trauma. Both presented with progressive vision loss, mydriasis, and zonular dialysis. The surgical intervention involved complete cataractous lens removal, anterior vitrectomy, iris cerclage with 10-0 prolene sutures, and retropupillary iris-claw lens fixation. Postoperative outcomes showed significant visual acuity improvements: vision reached 6/12 with correction in both cases. These findings suggest that combining iris cerclage with retropupillary iris-claw lens implantation is a safe, faster, and effective strategy…

Congenital bilateral coloboma of iris and choroid accompanied by unilateral multiple primary pigmented iris cysts: A case report.

INTRODUCTION AND IMPORTANCE: Congenital iris and choroidal coloboma is a congenital ocular developmental anomaly, most occur in both eyes, which may exist in isolation or be accompanied by systemic developmental abnormalities. Herein, we report a case of congenital bilateral coloboma of iris and choroid accompanied by unilateral multiple primary pigmented iris cysts. The selection of treatment methods for iris cysts has always been a challenge for ophthalmologists. Especially for primary iris pigment epithelial cysts without clinical symptoms, no particular intervention measures are required. …

Correlation between secondary metabolites of Iris confusa Sealy and Iris pseudacorus L. and their newly explored antiprotozoal potentials.

BACKGROUND: In the last few decades, the use of plant extracts and their phytochemicals as candidates for the management of parasitic diseases has increased tremendously. Irises are aromatic and medicinal plants that have long been employed in the treatment of different infectious diseases by traditional healers in many cultures. This study aims to explore the potential of three common Iris species (I. confusa Sealy, I. pseudacorus L. and I. germanica L.) against infectious diseases. Their in vitro antiprotozoal potency against Plasmodium falciparum, Trypanosoma brucei brucei, T. b. rhodesiens…
