Efficacy of Ivabradine in Preventing Inappropriate Shock due to Sinus Tachycardia in a Patient with Cardiac Sarcoidosis.

We herein report a case of left ventricular dysfunction due to cardiac sarcoidosis following cardiac resynchronization therapy defibrillator (CRT-D) in an 87-year-old man. He presented to our hospital after receiving shock therapy while conscious. Device interrogation revealed that inappropriate shock occurred due to sinus tachycardia. Due to the setting of a low ventricular tachycardia (VT) detection rate because of a history of slow VT, increasing the VT detection rate was not feasible. After initiation of treatment with ivabradine, the sinus rate decreased and there was no recurrence of ina…

Ivabradine in the treatment of non-paroxysmal junctional tachycardia with interference atrioventricular dissociation: A case report.

Ivabradine reduces the heart rate by selectively inhibiting the If current of the sinoatrial node, mainly for the treatment of chronic heart failure with decreased left ventricular systolic function and inappropriate sinus tachycardia, but the inhibitory effect on the atrioventricular node is rarely reported. The patient was admitted to hospital mainly because of intermittent chest pain for 7 years, which worsened for 10 days. Admission electrocardiogram (ECG) considered sinus tachycardia, with QS wave and T wave inversion in II, III, aVF, V3 R-V5 R, V4 -V9 leads, and non-paroxysmal junctional…

The use of ivabradine in a patient with idiopathic ventricular arrhythmia originating from the left ventricular summit.

Idiopathic ventricular arrhythmias (VA) are common and treatment options include anti-arrhythmic drugs (AAD) or catheter ablation. Patients presenting with idiopathic VA which is originating from the left ventricular summit (LVS) poses a particular challenge as the success for catheter ablation is low and AAD's may not be used long-term due to side effects. Ivabradine is an inhibitor of funny current (If) in cardiac pacemaker cells by blocking hyperpolarization-activated cyclic nucleotide-gated (HCN). In the present case, we reported the use of ivabradine in treatment of idiopathic VA which wa…

Ivabradine in heart failure patients with reduced ejection fraction and history of paroxysmal atrial fibrillation.

AIMS: Ivabradine is indicated for heart failure (HF) patients with reduced ejection fraction (HFrEF), but limited data are available with regards to the use of ivabradine in those with a history of paroxysmal atrial fibrillation (AF). To assess the effect of ivabradine in HFrEF patients with paroxysmal AF, we analysed heart failure (HF) hospitalization and mortality from multiple-centre registry database. METHODS AND RESULTS: We conducted a multicentre observational matched cohort study, and this study enrolled patient with symptomatic HFrEF from 1 January 2015 to 31 December 2018 who had a h…

Molecularly-imprinted polymer-base bulk optode for the determination of ivabradine hydrochloride in Procoralan®.

IVH may be administered orally twice per day for treating heart failure, sinus rhythm, heart-related chest pain and angina pectoris, and its overdose may cause problems such as blurred vision, uncontrolled blood pressure, prolonged bradycardia, and others. A molecularly imprinted polymer-based bulk optode, miptode, was constructed for the determination of ivabradine hydrochloride (IVH) in its pharmaceutical preparation Procoralan®. The molecularly-imprinted polymers (MIPs) were prepared in different ratios, and MIP3 had the highest imprinting factor (1.6) as an ionophore in the miptode prepara…

Report of a patient with refractory atrial tachycardia whose heart rate was controlled using ivabradine.

Ivabradine is an effective treatment for focal atrial tachycardia. However, it may also be effective for re-entrant atrial arrhythmia. An 85-year-old woman with a history of underlying ischaemic cardiomyopathy complained of worsening symptoms of heart failure because of rapid atrial tachycardia that was resistant to several rate-controlling drugs, but responded well to ivabradine. An electrophysiology study demonstrated a roof-dependent macro-re-entrant tachycardia of the left atrium. Linear ablation of the left atrial roof resulted in termination of the tachycardia. Thus, ivabradine can be an…

Effectiveness of enteral ivabradine for heart rate control in septic shock: A randomised controlled trial.

Persistent tachycardia in patients with septic shock predicts poor outcome. This study sought to investigate the effect of the cardiac pacemaker current inhibitor ivabradine on heart rate and cardio-circulatory function in patients with septic shock. After informed consent, 60 patients with septic shock and persistent tachycardia (heart rate >95 /minute) were prospectively randomly assigned to receive either standard therapy for septic shock (group S) or standard therapy along with enteral ivabradine (group I) for the initial 96 hours after enrolment. Primary outcome was the difference in hear…

Baseline intrinsic heart rate and response to ivabradine treatment in patients with inappropriate sinus tachycardia.

BACKGROUND: Treatment with ivabradine became a new therapeutic alternative for patients with inappropriate sinus tachycardia (IST). The aim was to determine a relation between intrinsic heart rate (IHR) and response to ivabradine treatment. METHODS: Twenty-seven patients (mean age 37 ± 11; 23 women) with symptomatic IST despite medical treatment were recruited into the study. Resting ECG, 24-hr ECG monitoring (24hECG), exercise treadmill test, and symptoms evaluation were performed initially and after 60 days on ivabradine. IHR was acquired at baseline after pharmacological autonomic blockade…

Limited role and benefit of ivabradine in the treatment of angina and heart failure with reduced ejection fraction.

Ivabradine is an original drug that has been approved in two indications (systolic heart failure and angina). The aim of this short review is to draw the attention of clinician prescribers to the evidence base of ivabradine. Three large randomized trials testing ivabradine versus placebo have been performed. The BEAUTIFUL and SIGNIFY trials were in fact negative in the treatment of angina while the SHIFT trial found a marginal benefit of ivabradine over placebo in the treatment of heart failure. These important results are put into perspective in order to improve the assessment of risk-cost/be…

Microaxial Flow Pump and Ivabradine for Tachycardia-Induced Cardiogenic Shock Caused by Automatic Atrial Tachycardia.

BACKGROUND: Management of tachycardia-induced cardiomyopathy (TIC) complicated by cardiogenic shock is challenging because rate control and … e left ventricular dysfunction was considered TIC. Under percutaneous microaxial flow pump support, ivabradine and landiolol achieved rate control without hemodynamic deterioration. Left ventricular function improved, allowing microaxial flow pump weaning and transition to oral bisoprolol. DISCUSSION: Microaxial flow pump-supported pharmacological rate cont …

Inappropriate sinus tachycardia-induced cardiomyopathy with severe functional mitral regurgitation and successful treatment with ivabradine.

Ivabradine increases stroke volume, but does not have a negative impact on blood pressure (BP). Thus, a patient with low BP can benefit from treatment with ivabradine. A 72-year-old Japanese woman with asthma and chronic bronchitis presented with dyspnea. Her heart rate (HR) was 126 beats per minute and an electrocardiogram showed sinus tachycardia. The chest X-ray showed cardiomegaly and pulmonary congestion. A transthoracic echocardiogram (TTE) showed reduced left ventricular ejection function (LVEF) and severe functional mitral regurgitation (MR). We diagnosed her with inappropriate sinus t…

Carvedilol Combined With Ivabradine Improves Left Ventricular Diastolic Dysfunction, Clinical Progression, and Survival in Cirrhosis.

BACKGROUND: Left ventricular diastolic dysfunction (LVDD) refers to impaired cardiac diastolic relaxation and may be improved by targeted heart rate reduction (THR). The authors evaluated whether a combination of carvedilol and ivabradine, an If channel blocker that reduces heart rate without affecting blood pressure, could improve LVDD and outcomes in cirrhosis. PATIENTS AND METHODS: THR was defined as heart rate reduction to 55 to 65 beats per minute. Of 260 patients with cirrhosis, 189 (72%) with LVDD were randomized to THR [group (Gr.)A; n=94; carvedilol±ivabradine)] or standard care (Gr.…

Efficacy and Safety of Ivabradine for Patients With Acute Heart Failure: Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: The efficacy and safety of Ivabradine for patients with acute heart failure (AHF) is controversial, and there are few clinical trials addressing this topic. METHODS: We performed this meta-analysis to evaluate efficacy and safety of Ivabradine treatment for patients with acute heart failure. We obtained data for controlled trials using the PubMed, Cochrane Library, EMBASE, and Clinical Trials.gov databases. The efficacy endpoints included change in heart rate, brain natriuretic peptide (BNP) levels, N-terminal pro-brain natriuretic peptide (NT-proBNP) levels, ejection fraction (…

Ivabradine in patients with acute ST-elevation myocardial infarction: a meta-analysis of randomized controlled trials.

BACKGROUND: Elevated resting heart rate (HR) predicts poor outcomes in patients with coronary artery disease. Ivabradine has been recommended as a second-line anti-anginal agent in chronic coronary syndrome, while there are no clear indications for acute ST-elevation myocardial infarction (STEMI). RESULTS: We systematically searched PubMed, Medline, EMBASE, Clinical Trials.gov, and the Cochrane Central Register of Controlled Trials with search terms Ivabradine and Acute myocardial infarction. There are two study outcomes from this study: therapeutic and safety effects. Therapeutic effects inc…

Label-free green estimation of atenolol and ivabradine hydrochloride in pharmaceutical and biological matrices by synchronous spectrofluorimetry.

In this work, a label-free, rapid, and sensitive synchronous spectrofluorometric method was implemented to assay atenolol (ATL) and ivabradine hydrochloride (IVB) in pharmaceutical and biological matrices. Simultaneous determination of ATL and IVB by conventional spectrofluorometry cannot be implemented because of the clear overlap of the emmision spectra of ATL and IVB. To overcome this problem, synchronous fluorescence measurements at a constant wavelength difference (Δλ) combined with mathematical derivatization of the zero order spectra were perforemed. The results indicated a good resolut…

Development and validation of a LC-MS/MS method for simultaneous quantification of Ivabradine and Metoprolol in rat plasma.

The combination of Ivabradine (IVA) and Metoprolol (MET) was approved by US-FDA for symptomatic treatment of chronic stable angina pectoris. Hence, a potential analytical method that can simultaneously quantify these two drugs is required. In view of this, a novel and fully validated LC-ESI-MS/MS method has been established for the quantification of IVA and MET in rat plasma. Analytes and their deuterated analogues were quantitatively extracted from rat plasma by protein precipitation technique. The analytes were separated using acetonitrile-water consisting 0.1% orthophosphoric acid buffer (3…
