BACKGROUND AND OBJECTIVES: Day-case pediatric sedation is challenging. Dexmedetomidine is a sedative analgesic that does not induce respiratory depression. We compared dexmedetomidine to propofol when it was added to ketamine for sedation during pediatric endoscopy, regarding recovery time and hemodynamic changes.
METHODS: We enrolled 120 patients (2−7 years in age) and randomly assigned them into two groups. Each patient received intravenous (IV) ketamine at a dose of 1 mg.kg-1 in addition to either propofol (1 mg.kg-1) or dexmedetomidine (0.5 μg.kg-1). The recovery time was compared. Hemody…

BACKGROUND: Treatment-resistant depression (TRD) poses significant clinical challenges, often necessitating alternative treatments such as intravenous (IV) ketamine or intranasal esketamine. This study compared the effectiveness of these modalities and examined predictors of treatment response, including comorbidity profile, prior antidepressant failures, age, and gender.
METHODS: A retrospective chart review was conducted on 200 adults with TRD treated with IV ketamine (median 6 infusions) or intranasal esketamine (median 8 sessions) at a specialized clinic. Depression severity was assessed …

Ketamine has emerged as a key drug in emergency medicine in recent years due to its rapid onset of action, hemodynamic stability properties and wide applicability. However, Hungarian regulation still limits its use mainly to anesthetic indications. The aim of this study was to review the literature on the emergency use of ketamine, with particular reference to its role in prehospital and intrahospital analgesia, sedation and rapid sequence intubation (RSI) protocols. A structured literature search was performed in PubMed, Scopus and Cochrane Library databases, including relevant human studies …

For over 50 years, ketamine has been approved for anesthesia and indicated for analgesia. In recent years, new clinical uses of ketamine have become increasingly common and continue to emerge. Intramuscular administration is a main route of administration and is particularly important in situations where resources are limited, such as in emergencies and low-income areas. The goal of this review is to summarize the literature on the pharmacokinetics of intramuscular ketamine in humans. Ten studies assessing intramuscular ketamine pharmacokinetics were identified. These studies looked at doses b…

Ketamine, an N-methyl-d-aspartate receptor (NMDAR) antagonist first developed as an anesthetic, has shown significant promise as a medication with rapid antidepressant properties in treatment-resistant depression. However, concerns such as adverse side effects and potential misuse liability have limited its widespread use. Racemic ketamine has two enantiomers-(S)- and (R)-ketamine-that appear to have disparate underlying mechanisms. This brief review summarizes some of the most recent preclinical and clinical research regarding the convergent and divergent prophylactic, immediate, and sustaine…

Ketamine (Ket) was developed in 1962 as a less hallucinogenic and shorter acting agent than phencyclidine. It was given to humans for the first time in 1964. However, Ket produces several adverse reactions such as raised intracranial and blood pressures along with seizures, and patients still show low acceptance due to hallucinations. As new volatile and intravenous anesthetic agents with good emergence and favorable side effect profiles were developed, Ket use markedly decreased. In the 1990s, as the ultrashort-acting opioid remifentanil was developed, high dose opioid could be used to reduce…

BACKGROUND: There is no recommendation in Europe for the use of ketamine in patients with chronic pain. The heterogeneity of practice highlights the need to seek the advice of experts in order to establish a national consensus. This Delphi survey aimed to reach a national consensus on the use of ketamine in chronic pain in Pain clinics.
METHODS: A collaborative four-round internet-based questionnaire was used. It was created after literature search on ketamine administration in chronic pain and included about 96 items. It discussed utility and advantages, adverse events and deleterious aspect…

There has been substantial interest in the use of ketamine for perioperative analgesia. Recently published articles on 'low dose' ketamine mark the resurgence in interest in the use of the drug for acute pain. Continued interest in ketamine as an anti-depressant also has opened the door to applications beyond the operating room. In this article, we will review: the history of ketamine's clinical use; basic ketamine pharmacology; evidence for the use of perioperative ketamine for analgesia; comments on patient selection for ketamine research; a discussion of the safety and side effect profile o…

Ketamine is considered a dissociative anesthetic medication, and it is commonly administered by a parenteral route. It works mainly by blocking the N-methyl-D-aspartate receptor. It inhibits the voltage-gated Na and K channels and serotonin and dopamine reuptake; also, it affects specific receptors, such as α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid, kainate, and aminobutyric acid A receptors. Ketamine appears to have particular mechanisms that are potentially involved during analgesic induction, including enhancing of descending inhibition and antiinflammatory effects. More recently…

BACKGROUND AND OBJECTIVES: The burden of chronic headache disorders in the United States is substantial. Some patients are treatment refractory. Ketamine, an N-methyl-D-aspartate antagonist, provides potent analgesia in subanesthetic doses in chronic pain, and limited data suggest it may alleviate headache in some patients.
METHODS: We performed a retrospective study of 61 patients admitted over 3 years for 5 days of intravenous therapy that included continuous ketamine to determine responder rate and patient and ketamine infusion characteristics. Pain ratings at 2 follow-up visits were recor…
