Pericaliceal haemangioma-a cause of papillary necrosis? Case report and review of 7 similar vascular lesions.

A pericaliceal haemangioma associated with papillary necrosis caused haematuria which led to nephrectomy. Seven similar vascular lesions were found on review of the surgical histopathology records from the last 25 years at the St Peter's Hospitals, Institute of Urology, London. In 4 of these there was histological evidence of damage to the renal papilla adjacent to the vascular abnormality; a possible mechanism for these changes is discussed. In all 8 cases the patients presented with haematuria and, in 5, abnormalities were seen on renal radiological investigation. Nephrectomy was performed i

[Radiological evolution of renal pelvis tumours (author's transl)].

The tumours of renal pelvis show a polymorphic radiological pattern, often rather difficult to interpret, sometimes even not evident owing to the presence of some other pathology. The most characteristic cases, and to be considered specimen, found in researches executed at different times, are represented by carcinomas, among which the ones showing the most elevated histological malignancy present a more rapid and infiltrative growth, as testified by repeated radiological tests.

Brush biopsy of pyelocalyceal lesions via a percutaneous translumbar approach.

Percutaneous translumbar selective brush biopsy of pyelocalyceal and ureteric lesions is simple, permits ready access to all calyces as well as the ureter and renal pelvis, and facilitates multiple biopsies from different sites. The ability to retrieve tissue fragments from between the bristles allows histological diagnosis in addition to a Papanicolaou smear. Selective washings can be obtained for bacteriologic diagnosis; in many instances, cultures taken directly from the lesion proved to be different from samples taken from the bladder urine or renal pelvis. This method can be expanded to s

Infiltrative renal lesions: CT-sonographic-pathologic correlation.

The CT and sonographic findings in 23 cases of renal lesions in which the abnormality was infiltrative, as opposed to expansile, were analyzed (invasive transitional cell carcinoma [seven], renal lymphoma [six], metastasis to the kidney [three], acute bacterial nephritis [three], mesoblastic nephroma [two], squamous cell carcinoma of the renal pelvis [one], and renal cell carcinoma [one]). CT scans were evaluated for shape and density of the mass, pelvocaliceal displacement, effect on the renal sinus fat, and the presence of perirenal involvement. Sonograms were evaluated for the shape and ech

Ureteropelvic junction obstruction in children with renal ectopy.

Of 49 children with renal ectopy, 25 (51%) had hydronephrosis on excretory urography. Dilatation of the renal pelvis was the result of primary ureteropelvic junction obstruction (17 children), significant vesicoureteral reflux (6 children), or extrarenal pelves and calices with renal malrotation producing apparent ureteropelvic junction obstruction (2 children). Vesicoureteral reflux was a common finding in these patients, occurring in 52% of those with crossed renal ectopy and in 70% of those with pelvic kidney. The need for surgical intervention results from either (1) ureteropelvic junction

New drainage technique in the ureteropelvic junction obstructions. Side-to-side ureterocalicostomy (or ureterocaliceal by-pass) preserving renal pelvis and ureteropelvic junction.

We present a new ureterocalicostomy technique in a patient with intrarenal pelvis together with renal artery compression on the pelvis and ureteropelvic junction (UPJ). The patient had grade 4 hydronephrosis in the left kidney and angulation at the ureteropelvic junction. We performed side-to-side ureterocalicostomy between the ureter and lower calyx of the kidney.

[Spontaneous extravasation of urine at the pyelo-caliceal level. Spontaneous urinomas].

We present two cases of spontaneous rupture of the excretory tract within the context of reno-ureteral colic due to lithiasis. We comment on the etiopathogenic aspects and underscore the need for emergency evaluation in patients with very severe nephric colic and/or symptoms of peritoneal irritation (ileus, abdominal guarding), and temporary urinary diversion must be performed via nephrostomy as a palliative procedure for the symptoms and to avoid complications.

Robot-Assisted Laparoscopic Calyceo-Pyelostomy for Vascular Compression of the Upper Calyx (Fraley Syndrome).

Fraley's Syndrome is a rare anatomic vascular malformation described in 1966 where an aberrant crossing vessel compresses the upper infundibulum and leads to upper calyx massive dilation. It is mostly asymptomatic and the diagnosis often missed; however, surgery is usually required for symptomatic cases. Open surgery is still frequently used while minimally invasive management is anecdotal in the literature. We report the first robot-assisted calyceo-pyelostomy. An 18-year-old female patient presented with intermittent, recurring, severe left flank pain leading to insomnia, evolving in the las

Renal calyceal microlithiasis: clinical presentation may precede sonographic evidence.

Calyceal microlithiasis (CM) is characterized by the sonographic finding of hyperechogenic spots less than 3 mm in diameter in renal calyces, and it may be the first step in calculus formation. From January 1992 to January 1998, we have observed 216 children with CM. The present report deals with 34 of them in whom renal sonography was negative at first observation and the diagnosis of CM was made only on repeated sonographic examinations. The presenting symptoms were recurrent abdominal pain, dysuria, and hematuria occurring alone or in combination. Half of the patients had hypercalciuria. A

[Renal hematoma as a result of internal ureteral catheterization].

INTRODUCTION AND OBJECTIVE: Urinary catheterization by means of an internal ureteral catheter in an usual procedure that may present complications. In this article we presented a case of perforation and later renal subcapsular hematoma after the placement of a "double-J" catheter. CONCLUSIONS: This complication, although rare, should be kept in mind. The manner of avoiding it is to maintain the tightness of the guidewire while inserting the ureteral catheter. Usually, the consequences are not life-threatening and are solved with conservative treatment. The administration of LFRE should be del

[Staghorn renal lithiasis treated with shock waves. Bacteriologic aspects].

Struvite renal stones are caused by infection of the urine with bacteria that synthesize the enzyme urease. Ammonium is released by the breakdown of urea by urease, the urine becomes highly alkaline, and magnesium ammonium phosphate (struvite) and carbonate apatite crystallize. Incorporation of the infecting bacteria within the developing stone, results in a focus of infection that is resistant to conventional antimicrobial therapy, and which is manifested clinically by repeated urinary tract infection caused by persistent bacteriuria. Extracorporeal shock wave lithotripsy (ESWL) currently is

[Milk of calcium renal stone in a patient with ureteral tumor: a case report].

We report a case of milk of calcium renal stone within a calyceal diverticulum in the patient with left ureteral tumor. The patient was a 77-year-old-man with the chief complaint of asymptomatic microscopic hematuria. A plain film revealed round opaque calcific densities in the left kidney in the supine position and a dense crescent-shaped shadow with a horizontal border in the upright position. An intravenous pyelography showed left hydronephrosis and calculi within a calyceal diverticulum. Computed tomography in the supine position revealed layering of the calcifications in cystic lesion of

[Retrograde endopyelotomy in the treatment of iterative stenoses of the pyeloureteral junction].

This work is a report on our preliminary experience with the treatment of iterative stenosis of the pyeloureteral junction (JPU) through a retrograde endoscopic approach. From June, 1991, to June, 1992, we used this method to operate 6 cases of failed surgical pyeloplasty due to abnormalities of the JPU. The success of this operation depends, on one hand, on the systematic dilatation of the intramural portion of the ureter, and on the other hand, on the possible installation of a double pigtail stent a few weeks before endopyelotomy. We have noted no complication. The stay in hospital lasted 3

The use of a cystoscopically placed cobra catheter for directed irrigation of lower pole caliceal stones during extracorporeal shock wave lithotripsy.

As the general experience with extracorporeal shock wave lithotripsy (ESWL) for renal calculi broadens, it is increasingly evident that the clearance of stone fragments in lower pole calices needs to be improved. The stone-free rate after treatment for lower caliceal stones is consistently less than that for other upper tract locations. Use of a cystoscopically placed cobra catheter for directed irrigation during ESWL resulted in an increase of our lower caliceal stone-free rate at 1 and 3 months of followup to 71% (15 of 21) compared to 33% (8 of 24) of randomly selected controls at 1 month a

[Determination of the borders and area of the graphic image of the human pelviureteral segment].

A method for determining borders of the human pelvic-ureteral segment (PUS) by means of its graphic image is presented. Using the material of 65 preparations of the renal calyceal-pelvic system obtained from human corpses of various age, the area of its graphic image has been determined. The dependence of the PUS parameters both from the form of the renal pelvis and from its position in the renal sinus has been revealed. A statistically significant increase of the PUS parameters from birth up to the mature age has been stated to be as great as great as 5.2-6.2, and from the mature to the old a

[Spontaneous renal rupture caused by renal pelvic tumor: a case report].

A case of spontaneous rupture of the right kidney caused by a primary renal pelvic tumor is reported. A 57-year-old man complaining of right flank pain and gross hematuria was referred to our hospital in November 1992. In 1982, transurethral resection of the bladder tumor (TUR-Bt) and vesical instillation with mitomycin-C (MMC) had been performed at another hospital for recurrent bladder tumor. In 1988, the excretory urogram showed right hydronephrosis in the absence of a bladder tumor. In 1992, the excretory urogram revealed nonvisualization of the right kidney and obscurity of the right psoa
