Fifty-five children with 55 fractures of the femur were prospectively studied for the presence of ligamentous instability of the knee within 3 weeks from the time of injury. Two patients (4%) demonstrated instability on examination under anesthesia. A 9-year, 4-month-old boy sustained a second-degree injury to the lateral collateral ligament that healed after hip spica cast treatment. The other patient, a 12-year, 8-month-old boy, sustained an avulsion fracture of the posterior cruciate ligament from the tibial insertion with > 1 cm posterior displacement of the tibia on the posterior drawer t

INTRODUCTION: Total knee replacement in valgus knees is more challenging than in varus knees, which needs meticulous handling and decision-making from the surgical approach to the rehabilitation protocol. In this case, we report an iatrogenic fibula head fracture during total knee replacement for a valgus knee with no concomitant common peroneal nerve palsy. This is the first report of this kind in the literature.
CASE REPORT: We report a case of a 67-year-old male with fixed valgus and flexion deformity of the right knee, who underwent total knee arthroplasty by lateral para-patellar approac

Combat-related blast injuries often cause devastating extremity trauma. We report a case of a 21-year-old male service member who sustained massive bilateral lower extremity trauma secondary to a blast injury. His orthopaedic injuries included a near traumatic disarticulation of the right knee and a left open type IIIB periarticular knee fracture with traumatic patellectomy, loss of the extensor mechanism, and segmental loss of the distal 11 cm of his femur. Definitive treatment of his injuries included a contralateral structural cortical femoral autograft which was implanted into the left kne

Proximal tibial fractures below an osteoarthritic knee have a greater risk of nonunion. Treatment by cast immobilization alone is problematic. Internal fixation alone means the patient will remain handicapped because of the knee. Total knee replacement with a long-stemmed prosthesis treats both the fracture and the arthritic knee.

Unicompartmental knee arthroplasty (UKA) is widely performed for medial compartment knee osteoarthritis due to its benefits of faster recovery, preservation of knee kinematics, and long-term survival. However, complications such as peri-implant fractures can occur. Most fractures are reported within three months postoperatively, but this study highlights two cases of delayed fractures occurring four and seven years after UKA, emphasizing the importance of long-term follow-up. In Case 1, a 79-year-old woman experienced a tibial peri-implant fracture seven years after UKA. Bone density loss and

BACKGROUND: Post-hip fracture knee pain (PHFKP) is an important issue that contributes to reduced gait speed and prolonged hospitalization. Femoral morphology has been reported to contribute to the development of PHFKP, but an independent association has not been confirmed and clinically applicable cutoffs for predicting the development of PHFKP remain unclear.
OBJECTIVE: To determine whether femoral morphology and knee extension range of motion limitation are independent factors in PHFKP and to determine cutoffs for predicting the development of PHFKP.
DESIGN: Retrospective chart review stu

INTRODUCTION: With the increasing incidence of total knee arthroplasty (TKA), there is an expected rise in rate of periprosthetic fractures in the coming years. It is unclear how the outcomes of patients with distal femur fractures (DFF) and a total knee arthroplasty compare to patients of the same age group with native knees (NK).
MATERIALS AND METHODS: A retrospective review was completed for distal femur fractures treated with surgical fixation from January 2019-March 2021. We excluded patients <50 years old, non-ambulatory patients, revision surgeries, and patients with less than 90 days

BACKGROUND: Knee osteoarthritis (KOA) represents a major clinical and economic burden globally, and total knee arthroplasty (TKA) remains the definitive treatment for end-stage disease. Upper extremity fractures in older adults often indicate skeletal fragility and reduced physiological reserve, but their influence on subsequent TKA incidence remains unclear. We investigated whether upper extremity fractures influence TKA incidence among patients with KOA. METHODS: We conducted a retrospective propensity score-matched cohort study using the TriNetX US Collaborative Network (2005–2024) and perf

Insufficiency fractures are rarely reported after total knee arthroplasty (TKA). A 79-year-old woman with osteoporosis underwent revision of a unicompartmental arthroplasty to TKA. Six weeks later, she developed ipsilateral ankle pain. Examination revealed lateral ankle tenderness with preserved motion; initial radiographs were normal. Worsening pain prompted an MRI, which uncovered an undisplaced talar neck insufficiency fracture. Conservative treatment with immobilization and protected weight bearing led to complete healing at 1 year. This is the first documented case of a talar neck insuffi

BACKGROUND: Chronic knee fracture-dislocation is an exceptionally rare and disabling condition, most often resulting from neglected trauma. In elderly patients with degenerative changes and extensive ligamentous insufficiency, total knee arthroplasty (TKA) may represent a reliable salvage procedure to restore alignment, stability, and function.
CASE PRESENTATION: A 76-year-old woman with hypertension and type 2 diabetes presented four months after a fall with a chronic posterolateral dislocation of the left knee. Clinical examination revealed a fixed varus deformity and posterior tibial trans

UNLABELLED: Tibial spine fractures are uncommon, particularly in the pediatric population. Multiple case reports have described the treatment of tibial spine fractures in pediatric patients as it presents unique anatomical challenges. Displaced tibial spine fractures can lead to instability and become entrapped in the knee, resulting in decreased range of motion and knee contractures. This case highlights the unique treatment complications of severe knee contracture coinciding with femur deformity after an iatrogenic physeal fracture sustained during manipulation under anesthesia following a t

Treatment strategies for periprosthetic distal femoral fracture depend on the type of fracture, level with respect to the prosthesis, and prosthesis fixation. The usual treatment is internal fixation by nail or locking plate. This can be more difficult when the fracture is close to the joint, is destabilizing the prosthesis or is associated with reduced bone stock, and implant exchange should be considered, depending on the patient's health status. The aim is to obtain the fastest possible recovery while limiting complications and minimizing surgery time for often frail and elderly patients. T

Hip fracture is one of the main causes of medical attendance in trauma centers. By contrast, knee dislocation is a challenging and limb-threatening injury. We report a case of a 68-year-old male who sustained a simultaneous ipsilateral hip fracture and knee dislocation after a low-energy fall. Popliteal artery and common peroneal nerve injuries were associated. Knee dislocation treatment included closed reduction, external fixation and popliteal artery repair. Hip hemiarthroplasty was performed using a direct anterior approach. Revision multiligament knee reconstruction surgery was not conside

OBJECTIVE: To propose an magnetic resonance imaging (MRI) grading system for subchondral insufficiency fracture of the knee (SIFK) to predict outcome and assess risk factors.
MATERIALS AND METHODS: A total of 50 SIFK patients were retrospectively reviewed utilizing two MRI examinations approximately a year apart and compared them with 51 control subjects. A grading system was introduced that classifies lesions as low- vs high-grade. Lesion location 3D dimensions, extent of bone marrow edema (BME), location of meniscal tears and associated extrusion, degree of chondrosis and among other parame

We report a rare case of proximal fibular fatigue fracture developing 14 years after total knee arthroplasty in a known case of rheumatoid arthritis. A valgus deformity of the knee can put abnormal stress on the upper fibula leading to its failure. We believe that, as the fibula acts as an important lateral strut, its disruption due to a fracture led to rapid progress of the valgus deformity of the knee in this patient.

Stress fracture of the femoral component is a rare complication following total knee arthroplasty. We report one such complication which occurred 6.5 years after primary implantation of an uncemented meniscal Low Contact Stress (LCS) prosthesis. The fracture affected the medial condyle of the femoral component, and a large osteolytic lesion was present in the underlying femoral condyle. The patient underwent revision arthroplasty with a cemented constrained condylar knee, which is still in place with a satisfactory result 12 years later. This complication appears to be very rare, although a nu

Acute locking is a sign of internal derangement of the knee and is an orthopaedic surgical emergency. Differential diagnoses of this condition are torn meniscus, intra-articular loose bodies and torn anterior cruciate ligament. We present a 15-year-old schoolboy who developed acute locking of the knee during dancing. Arthroscopy revealed that he had sustained an osteochondral fracture of the patella and the loose fragment from it caused locking of the knee. That this was not pseudo-locking was confirmed by examination of the knee under anaesthesia. Locking of the knee by a loose osteochondral

This study analyzes the results of treatment of 22 patients with 24 supracondylar femur fractures above a total knee arthroplasty. Ten knees were treated by closed methods utilizing traction and then a cast, 10 knees with immediate open reduction and internal fixation, 2 knees with a custom total knee integrated with a distal femoral allograft, 1 knee with external fixation, and 1 knee with primary arthrodesis. Nine fractures treated by closed means and 5 fractures treated by open reduction and internal fixation healed primarily. Two of the 5 surgical failures healed after replating and bone g
