We report the largest single-centre series in the literature of laryngeal cysts in infants. Most of these children presented with stridor within the first two weeks of life, and were found to have a supraglottic or vallecular cyst. Early endoscopic de-roofing of the cyst is the recommended form of treatment.

Kaposi's sarcoma of the larynx is a rarely observed manifestation of AIDS. Possible concomitant symptoms are hoarseness up to aphonia, urge to cough or stridor. Expectoration of small parts of tissue of Kaposi's sarcoma may be an additional sign. Similar to Kaposi's sarcoma in the oral cavity Kaposi's sarcoma of the larynx is characterised by a purplish, spongy nodule. Its surface can be verrucous due to deposits of dry secretion. Local extirpation of Kaposi's sarcoma removes the laryngeal signs and symptoms.

The otolaryngologist who is called upon to evaluate respiratory distress in the newborn should be familiar with the various congenital lesions of the larynx. A presumptive diagnosis can usually be made on the basis of the clinical history and physical findings. However, the mainstay of diagnosis is endoscopic examination of the larynx.

A newborn male presented with respiratory distress and laryngeal stridor at the time of birth. Laryngoscopy revealed a circumscribed mass in the right vocal cord which was diagnosed as an undifferentiated malignant neoplasm on frozen section. Further light microscopic studies, special stains and electron microscopy disclosed features consistent with a special subtype of sarcoma adopted by the Intergroup Rhabdomyosarcoma Committee. Several authors have also demonstrated similarities between this type of tumor and soft tissue Ewing sarcomas. Nonepithelial malignancies of the larynx are rare in c…

This study is based on a very large series of histological sections of non-tumour-bearing larynges, most of them in the horizontal plane. Six hundred sections in one larynx were performed to show the constitution of the anterior region of the larynx at all levels. Although the same tissues are present (fibres, glands and vessels) in almost the entire extent of the anterior segment of the larynx, there is a small zone immediately above the vocal cords with neither glands nor vessels. The authors designate this zone as 'plane O' which separates the anterior commissure from the thyro-epiglottic l…

Since 1973, evidence based on case-control and mortality studies has suggested that carcinoma of the larynx was an asbestos-related tumour. In this study, 305 male and 206 female patients were interviewed on admission to the Royal National Throat Nose and Ear Hospitals in London. Following endoscopy, biopsy material was reviewed and classified as either neoplastic, cysts and polyps or inflammatory; a fourth group consisted of patients for whom the surgeon recorded that the larynx appeared normal on inspection and from whom no biopsy was taken. Only 15 neoplastic conditions were detected among …

A rational choice of treatment for patients with malignant tumors of the larynx depends upon a thorough evaluation of the location, size, and extent of the lesion. Roentgenological examinations of the larynx are a valuable adjunct to clinical and laryngoscopic examinations. In interpretation of roentgenographic findings of malignant tumors of the larynx, a good knowledge of the anatomy of the head and neck, and of the nature, history, and routes of spread of the lesions of the larynx is essential. A correct interpretation needs careful scrutiny of all the roentgenograms; only a persistent abno…

There is little concensus concerning the boundaries of the subglottic space. To better define the subglottis, 50 head and neck surgeons were asked to define its anatomic limits. In addition, the larynx of each of 50 adult cadavers was removed and four measurements in the anterior subglottis were taken. The results revealed significantly smaller subglottic dimensions for women than for men. In most women, the cricothyroid membrane was situated less than 1 cm from the glottis in the anterior midline.

Eight cases of laryngeal oncocytic lesions treated at Toronto Western Hospital between 1957 and 1977 are reviewed. The typical clinical course was of a benign lesion with prolonged dysphonia. A smaller number with bulkier lesions developed symptoms of airway obstruction. The lesions were supraglottic and cystic. They were excised either endoscopically or via a laryngofissure. The pathologic features support the concept that the lesion represents a metaplasiahyperplasia, and therefore the term 'oncocytic cyst' is preferred to terms which imply a neoplastic process. Three patients with oncocytic

Laryngeal obstruction due to neurofibromatosis in a girl with von Recklinghausen's disease first occurred in the neonatal period, and recurred by age seven years. She has done well for four years following laryngofissure surgery. Pharyngeal and laryngeal neurofibromatosis are rare manifestations of von Recklinghausen's disease. The dominant hereditary pattern of this disease requires that the condition be considered when respiratory distress occurs in the child of an affected parent, whether or not skin tumors or other features are present.

This paper presents a case report of a 67-year-old man who was seen in the Otolaryngology Clinic, University of Wisconsin Medical Center with a seven-month history of dyspnea and laryngeal stridor. On examination there were several slightly tender firm submucosal nodules in the soft palate and left tonsilar area and a 1.5-cm polypoid subglottic mass arising from the body of the cricoid cartilage. Because of the marked airway obstruction, a tracheotomy was necessary. The laryngeal polypoid mass was removed and biopsies were obtained from the submucosal nodular masses of the palate. Microscopic

An attempt was made to develop a new method of microsurgery of the larynx. A newly designed laryngostroboscope suitable for our purpose was employed in our new method. The vibratory pattern was stroboscopically examined through the operating microscope throughout the operative procedure while the patients who were able to phonate due to neuroleptanalgesia were asked to do so. Thus, the determination of the completion time of the operation was obtained mainly by the observation of stroboscopic vibratory patterns of the vocal cords. Since the beginning of 1972, this stroboscopic microsurgery of …
