PURPOSE: To report a case of unilateral posterior dislocation of the cataractous lens and subluxation of the lens in the fellow eye of a patient with Parkinson-plus syndrome.
OBSERVATIONS: A 67-year-old-man who was a known case of Parkinson-plus syndrome on long-term dopamine agonists and anti-psychotic medications demonstrated apraxia of lid opening associated with moderate-to-severe blepharospasm. He had unilateral posterior dislocation of the cataractous lens and subluxation of the lens in the fellow eye with no prior history of trauma or other known ocular risk factors.

Hereditary ectopia lentis or lens subluxation can occur with and without systemic associations. Significant overlap can be found in the genetic mutations and pathogenesis of subluxated lenses in their isolated forms as well as with associated syndromes. Gene mutations have been identified for lens subluxation associated with Marfan syndrome, Weill Marchesani syndrome, Ectopia Lentis simplex, Ectopia Lentis et pupillae, Ehlers Danlos syndrome, homocystinuria, and sulfite oxidase deficiency. Herein we describe the ocular and systemic characteristics found in patients with ectopia lentis, as well…

BACKGROUND: Both, Stickler and Marshall syndrome are dominantly inherited, connective tissue disorders with highly variable individual manifestations. Probably, both syndromes represent the same entity with occasional partially different clinical manifestations.
PATIENT: We are reporting on a female patient with typical ocular and systemic manifestations of both syndromes: high myopia, glaucoma and goniodysgenesis, submucosal cleft palate, sensorineural hearing loss, and "flat" mid-face. Additionally, we observed subluxation of the ocular lens in one eye which was previously described in only

Marfan syndrome is an autosomal dominant genetic connective tissue disorder that results from mutations in the fibrillin-1 gene located on chromosome band 15q15-21. Fibrillin, a glycoprotein, is widely expressed throughout the body and contributes to the elasticity and force-bearing capacity of connective tissue. In the eye, fibrillin is a key constituent of the ciliary zonules, which suspend the crystalline lens in place. The zonular defect leads to ectopia lentis, which is a hallmark of Marfan ocular abnormalities and occurs in 60% to 80% of cases. Other less common ocular features of Marfan

The floating lens sign describes the radiologic appearance of a completely dislocated crystalline (native) lens or intraocular lens (IOL) on computed tomography imaging. It is visualized as a biconvex, hyperdense structure lying freely within the posterior vitreous cavity in the case of a native lens, and as a linear, hyperdense structure in the case of an IOL. This finding reflects complete zonular and/or capsular disruption, most commonly due to blunt ocular trauma, but it may also occur in patients with connective tissue disorders or advanced hypermature cataracts. Recognizing this sign is

OBJECTIVE: To report a case of canine posterior lens luxation in which the intact lens was successfully floated using perfluorocarbon liquid (PFCL) combined with pars plana vitrectomy (PPV) and subsequently removed.
ANIMAL STUDIED: A client-owned 7-year-old castrated female Italian Greyhound presented with rapidly progressive bilateral posterior lens luxation.
TREATMENT AND PROGRESSION: In the right eye, via a three-port 25-gauge pars plana approach, the vitreous was removed and PFCL was injected beneath the luxated lens to float the intact lens from the retinal surface to the pupillary plan

Two brothers had retinal degeneration, lens subluxation, and myopia since early life. There was no evidence of Marfan syndrome, homocystinuria, or other systemic disease. They had nystagmus, myopia, inferior dislocation of the lens, and posterior subcapsular opacities in both eyes. Fundus examination showed attenuated retinal vessels, macular atrophy with occasional pigment accumulation as clumps, and perivascular sleeves. Electroretinography revealed decreased photopic and scotopic responses. The visual fields were constricted. We believe this to be the first report of retinal degeneration wi…

PURPOSE: To analyze the intraoperative and postoperative complications of Cionni ring implantation in eyes with a subluxated lens or zonular … In 1 eye, corectopia was inadvertently produced by passing the suture needle through the iris root; lens subluxation temporarily increased during ring rotation and implantation. However, after the transscleral suture was tightened, capsular bag centration was excellent. In the eye with megalocornea, traction from the 2 hooks tore the edge of the anterior capsule r …

A 30-year-old man with bilateral congenital aniridia presented with a subluxated cataract in the right eye. He wore a soft contact lens for aphakic correction in the fellow eye after lensectomy for a subluxated cataract. We performed pars plana lensectomy and vitrectomy with implantation of a transscleral ciliary sulcus fixated posterior chamber intraocular lens in the right eye. Good postoperative visual acuity was obtained.

BACKGROUND: A 23-year-old female Bornean sun bear presented with symptoms of right eye discomfort, including frequent blinking and scratching. Ophthalmic examination revealed a dilated right pupil, anterior lens displacement, and early-stage bilateral cataracts. Corneal edema developed in the right eye.
CASE DESCRIPTION: An intracapsular lens extraction was performed to remove the right dislocated lens and ruptured capsule 3 weeks later. Postoperative care included topical eye drops, oral anti-inflammatory drugs, and antibiotics. Mild eyelid swelling, ocular rubbing, and decreased appetite we

A 29-year-old man presented with longstanding, stable, unilateral vision loss in the setting of a remote paintball injury. His examination was notable for a sensory exotropia as well as multiple foci of posterior synechiae, segments of white lenticular material and islands of lenticular cells within a grossly intact capsule, and severe zonular compromise in the affected eye. The majority of the nuclear lens material was absent. The patient was diagnosed with post-traumatic lens absorption and underwent synechialysis, capsulotomy, excision of remnant lenticular material, and placement of a sulc

We illustrate, from the pathologist's viewpoint, the configuration of posterior chamber lens loops within the eye. The final configuration of these loops is often a "C" shape, particularly following capsular fixation. When one or both loops are placed in the ciliary sulcus, where tissue resistance to loop compressive forces is less than in the capsular sac, an invagination or erosion of the loop into the ciliary body stroma or ciliary muscle frequently occurs. Although a surgeon should first and foremost choose a lens design with which he or she is comfortable in terms of ease of implantation

A highly myopic patient complained of 2 months of visual acuity loss and 4 months of ocular pain in the right eye after 16 years after bilateral cataract extraction. The slit lamp microscopy examination showed a residual lens capsule dislocation in the right anterior chamber of the eye, which was diagnosed as a residual lens capsule dislocation in the right eye. After diagnosis and timely treatment, the patient's postoperative vision and visual field recovered well.

The authors describe a simplified and safer technique for surgical removal of hereditary or idiopathic soft subluxated lenses. Five eyes of three patients underwent intercapsular aspiration of the lens material. A side-port cannula was introduced through the pars plicata while the eyes were pressurized by an anterior chamber maintainer. This was followed by anterior vitrectomy and capsulectomy. With this technique, the risk of vitreous loss, which can occur with an anterior approach, and the risk of the spread of lens particles into the vitreous, which can occur with a lensectomy with a vitrec

PURPOSE OF REVIEW: To evaluate and review the current literature on the management of retained lens material with pars plana vitrectomy (PPV) and lensectomy after complicated cataract surgery.
RECENT FINDINGS: Recent studies on retained lens fragments in cataract surgery support early referral to a vitreoretinal specialist, and indicate that visual outcomes are favorable with current vitrectomy and lensectomy techniques. Nearly 83% of patients without preexisting eye disease who undergo PPV achieve visual acuity of 20/40, whereas 5.5% of patients have postoperative acuity of 20/200 or worse.

We present the case of a 14-year-old boy with a known diagnosis of Knobloch syndrome (KS) referred for bilateral cataract evaluation and possible cataract surgery. At time of initial presentation, no lens subluxation was appreciated, and no phacodonesis was detected on slit lamp biomicroscopy. However, 7-weeks later, on the day of surgery, his right eye was found to have a complete lens dislocation into the vitreous cavity, with no zonule attachment. The left eye did not have subluxated lens; however, intraoperatively, after irrigation into the eye, near complete zonular dialysis was observed.…
