Limbal stem cell deficiency secondary to vitrectomy in the context of an autoimmune polyendocrine syndrome type.

PURPOSE: Autoimmune polyendocrine syndromes (APS) are rare diseases characterized by immunologic activity against multiple endocrine organs. Bilateral keratitis and limbal stem cell deficiency are manifestations described. METHODS: A 28-year-old man presented to our service with discomfort in his left eye. Past medical history included autoimmune polyendocrine syndrome, under hormonal treatment. Ocular history included an idiopathic bilateral peripheral occlusive vascular retinal disease, with areas of non-perfusion, neovascularization, and bleeding, treated with laser photocoagulation. 3D bioprinting as a prospective therapeutic strategy for corneal limbal epithelial stem cell deficiency.

Limbal epithelial stem cells (LESCs) are responsible for the maintenance and repair of the corneal surface. Injuries and diseases of the eye may result in a vision condition called limbal stem cell deficiency (LSCD). Without limbal stem cells, the cornea becomes opaque, vascularized, and inflamed. Cultured LESC therapy as a treatment method was first described in 1997, and LESCs cultured from either patients or donors have been used to treat LSCD successfully. However, the main source of cornea for LSCD treatment is from donors, which are too few to meet the demand (less than 1:70 of cases). T…

Characterization of corneal pannus removed from patients with total limbal stem cell deficiency.

PURPOSE: To determine the epithelial lineage of origin in corneal pannus tissue surgically removed from patients with total limbal stem cell (SC) deficiency. METHODS: The lineage of origin of the entire conjunctivalized pannus removed from eight corneas with a diagnosis of total limbal SC deficiency was characterized by anti-keratin (K)-3 and anti-K19 monoclonal antibodies. The protein and mRNA of epithelial outgrowth from segments of five such pannus specimens were analyzed by Western blot and reverse transcription-polymerase chain reaction, respectively. RESULTS: Cross sections of all eigh

Homologous penetrating central limbo-keratoplasty (HPCLK) in bilateral limbal stem cell insufficiency.

PURPOSE: Bilateral stem cell deficiency can be overcome only by keratoplasty plus additional homologous limbal transplantation. We have four … trally clear 12-41 months after HPCLK. CONCLUSIONS: In the majority of the grafts the transplanted limbal stem cells underwent immune destruction. The survival of seven grafts, however, shows that HPCLK is principally a promising new procedure. Further progress can be expected from the use of well HLA-matched grafts instead of unmatched grafts and from further imp …

Mechanistic insights into conjunctivalization in limbal stem cell deficiency using a lineage tracing approach.

PURPOSE: Disease that develops on the ocular surface in animals and humans that have their limbal stem cells erased or their niche severely damaged by physical or chemical trauma, is difficult to visually evaluate in a progressive and long-term manner. When limbal stem cell deficiency (LSCD) develops under such circumstances, intravital clinical microscopy or other techniques cannot be used to accurately detail what transpires on the cornea. Here, we investigated the evolution of LSCD and characterize cellular transformations that arise when the limbal niche is lost. METHODS: We employed the …

[Experimental evaluation of the efficacy of tissue-engineered constructs in the treatment of limbal stem cell deficiency].

UNLABELLED: Limbal stem cell deficiency (LSCD) is one of the leading factors negatively affecting the success of keratoplasty, and its treatment remains an urgent problem in ophthalmology. With the development of regenerative medicine, one of the promising approaches is the transplantation of tissue-engineered constructs from cultured limbal stem cells (LSCs) in biopolymer carriers. PURPOSE: This study was conducted to develop an experimental model of LSCD and evaluate the effectiveness of transplantation of a tissue-engineered construct consisting of cultured cells containing a population of…

Development and characterization of a preclinical mouse model of alkali-induced limbal stem cell deficiency.

PURPOSE: Limbal stem cell deficiency (LSCD) secondary to ocular surface alkali burn is a blinding condition that features corneal conjunctivalization. Mechanistic insights into its pathophysiology are lacking. Here, we developed a mouse model that recapitulates human disease to comprehensively delineate the clinicopathological features of a conjunctivalized cornea. METHODS: LSCD was induced in the right eyes of 6-8-week-old C57BL/6 male and female mice (n = 151) by topical administration of 0.25N sodium hydroxide on the cornea. Uninjured left eyes served as controls. Clinical, histological, p

Limbal stem cell transplantation with amniotic membrane for the treatment of uniocular chemical burn in children.

This study evaluated the outcome of corneal surface reconstruction with conjunctival limbal autograft and amniotic membrane transplantation in two children with unilateral ocular chemical burn who underwent surgery. Visual acuity improved from counting fingers at face to 20/60 in one patient and from counting fingers at 30 cm to 20/100 in the other. Both patients received simultaneous symblepharon lysis and regained deep fornices. When used in combination with amniotic membrane, conjunctival autograft allows correction of cicatricial abnormalities and early restoration of corneal surface integ

Rare corneal clones in mice suggest an age-related decrease of stem cell activity and support the limbal epithelial stem cell hypothesis.

The anterior ocular surface comprises the cornea, conjunctiva and a narrow intermediate region called the limbus. It is widely accepted that the corneal epithelium is maintained by stem cells but different hypotheses propose that the stem cells that maintain the mouse corneal epithelium during normal homeostasis are located either in the basal limbal epithelium or throughout the basal corneal epithelium. There are no specific markers to help test these alternatives and new methods are required to distinguish between them. We observed that KRT5(LacZ/-) transgenic mice produced rare β-galactosid

Temporal Differences in the Expression of p63α and Proliferation of Limbal Epithelial Stem Cells From Donor Corneas.

PURPOSE: Limbal stem cell deficiency is a blinding corneal disease that requires transplantation to replenish the limbal epithelial stem cells (LESCs) and restore vision. Keratolimbal allografts from cadaveric donors are commonly used as a source for LESCs. The goal of this study was to verify the efficacy of Food and Drug Administration-approved hypothermic corneal storage solutions in preserving LESCs within human donor corneas over time. METHODS: Limbal segments from deceased donor corneas were stored in Food and Drug Administration-approved corneal storage solutions for 0, 4, or 7 days. T
