Infliximab for chronic cystoid macular edema associated with uveitis.

PURPOSE: To assess the efficacy of the anti-TNF monoclonal antibody infliximab in uveitis patients without clinically evident ocular inflammation and impaired visual acuity because of chronic cystoid macular edema (CME). DESIGN: Prospective, noncomparative, interventional case series. METHODS: Patients with refractory CME (14 eyes, mean duration of 14 months), associated with intermediate uveitis (n = 6), Adamantiades-Behcet disease (n = 2), adult-type vascular pseudotumor (n = 1), and HLAB27+-related uveitis (n = 1) received an intravenous infliximab infusion (5 mg/kg); five patients were r…

Tomographic assessment of vitreous surgery for diabetic macular edema.

PURPOSE: To evaluate the retinal structure before and after vitrectomy for diabetic macular edema and to assess the correlation between thickness of neurosensory retina and best-corrected visual acuity. METHODS: Tomographic features of 13 eyes (nine patients) with diabetic macular edema were prospectively evaluated with optical coherence tomography before and after vitrectomy. The foveal thickness (the distance between the inner retinal surface and the retinal pigment epithelium) and the retinal thickness (thickness of neurosensory retina) were measured by optical coherence tomography preoper…

Patterns of diabetic macular edema with optical coherence tomography.

PURPOSE: We report cross-sectional images of diabetic macular edema and correlation between tomographic features and visual acuity with best correction by means of optical coherence tomography. METHOD: In a prospective study, optical coherence tomography was performed in 59 eyes of 42 patients with diabetic macular edema and in 10 eyes of 10 normal control subjects. RESULTS: Optical coherence tomography showed three patterns of structural changes in diabetic macular edema: sponge-like retinal swelling (52 [88%] of 59 eyes), cystoid macular edema (28 [47%] of 59 eyes), and serous retinal deta…

Pars plana vitrectomy for chronic pseudophakic cystoid macular edema.

PURPOSE: We determined the efficacy of pars plana vitrectomy in a series of patients with chronic pseudophakic cystoid macular edema. METHODS: Clinical records were reviewed on 24 consecutive patients who underwent pars plana vitrectomy in one eye for chronic pseudophakic cystoid macular edema. All 24 patients had failed to improve on medical therapy and had preoperative evidence of either vitreous adhesions to anterior segment structures (23 eyes) or iris capture of the intraocular lens (one eye). RESULTS: The mean, best-corrected Snellen visual acuity was 20/190 preoperatively (median, 20/…

Subretinal fibrosis after laser photocoagulation for diabetic macular edema.

Seven eyes had subretinal fibrosis after grid laser photocoagulation for diabetic macular edema. The fibrosis caused persistent loss in visual acuity, and in six of the seven eyes, was not associated with detectable laser-induced Bruch's membrane rupture or subretinal hemorrhage. Choroidal neovascularization was detected in only one patient, who was notably younger (27 years) than the median age of 70 years in this series. The median preoperative visual acuity was 20/80 (range, 20/40 to 20/400); the median postoperative visual acuity was 20/400 (range, 20/80 to counting fingers). The subretina…

Cystoid macular edema: the apparent role of mechanical factors.

CME is the final common pathway of many intraocular and systemic diseases. It involves the retinal vasculature and often has a choroidal and vitreal component. Obviously, at some level of this complex pathophysiological process, mediators must be involved. The question is whether the characteristic distribution of vascular leakage and retinal edema is best explained by the diffusion of mediators released by from a remote site, as proposed by Miyake (see Miyake et al., 1989), or whether it reflects the distribution of pre-existing anatomical structures causing the local release of mediators by

Pseudophakic and aphakic retinal detachment mimicking cystoid macular edema.

Seventeen eyes of patients who were being followed by their ophthalmologists for aphakic (8 eyes) or pseudophakic (9 eyes) cystoid macular edema (CME) were examined on referral. All eyes showed retinal detachment (RD), and eight eyes had CME with RD. Most RDs were inferior and shallow. Proliferative vitreoretinopathy (PVR) was noted in 15 eyes (88.2%). There was history of vitreous loss during the last intraocular surgery in 88.2% eyes. Visual acuity ranged from 20/40 to hand movements. Fundus view was hazy in most eyes, and indirect ophthalmoscopy was most useful in detecting RD. Retinal reat…

Aphakic cystoid macular edema. The pharmacology of ocular trauma.

Ocular tissues, like those of other organs, exhibit limited morphologic reactions to trauma, i.e., hyperemia, abrupt vasodilation, increased blood flow; increased permeability of blood vessels, edema and increased tissue pressure (disrupted blood-ocular barrier); and later, a cellular inflammatory response. The cystoid macular edema (CME) that occurs after surgery for cataract has a considerably higher incidence in more severely traumatized eyes. It is characterized by increased perifoveal capillary permeability that may be related either to prior vasoconstriction or to vasodilation, and it ma…

An anterior surgical approach to aphakic cystoid macular edema.

Fifteen consecutive eyes in 14 patients (six men and eight women, ranging in age from 43 to 75 years) underwent anterior vitrectomy for chronic cystoid macular edema documented by fluorescein angiography. All eyes had either vitreous adhesions to the cataract incision with pupillary distortion, or adherence of vitreous strands to the iris. Anterior vitrectomy via the corneoscleral limbus was performed in each case to remove vitreous from anterior segment structures. Thirteen of the 15 eyes showed visual improvement, with 12 of the 15 showing improvement of two Snellen lines or more. There was …

Incidence of cystoid macular edema following scleral buckling procedure.

Sixty eyes of 58 patients who underwent scleral buckling procedures were prospectively evaluated with postoperative fluorescein angiograms. Forty-three percent of these eyes showed angiographic cystoid macular edema (CME). Aphakic patients showed a 64% incidence of cystect the final visual acuity of aphakic patients but significantly reduced the visual recovery in the phakic group. The incidence of CME was not related to preoperative foveal detachment, duration of aphakia, duration of detachment, surgical approach, or refraction.

Iris angiography in cystoid macular edema after cataract extraction.

Iris angiography was performed on eight patients (nine eyes), who had cystoid macular edema postcataract extraction. The fundus fluorescein angiographies showed typical star-shaped, multiloculated staining of the macular area. The iris angiographies demonstrated moderate to massive leakage of dye from the iris, and possibly leakage of ciliary body vessels into the anterior chamber, thus evidencing the until now suspected but undocumented fact that fluorescein leakage is not restricted to the fundus vessels. These data tend to support the idea that this type of macular edema is due to a vascula…

Evaluation of macular function using focal macular electroretinography in eyes with macular edema associated with branch retinal vein occlusion.

PURPOSE: To study the usefulness of focal macular electroretinography (fmERG) for evaluation of macular function in eyes with macular edema (ME) associated with branch retinal vein occlusion (BRVO). METHODS: The authors prospectively performed fmERG on 34 patients with untreated unilateral BRVO at the initial visit and 12 months later. Amplitudes and latencies of the a-wave, b-wave, and photopic negative response (PhNR) were compared with visual acuity, with retinal sensitivity measured by microperimetry, and with measurements obtained by optical coherence tomography. RESULTS: In eyes with M…

Safety and Efficacy of Senolytic UBX1325 in Diabetic Macular Edema.

BACKGROUND: We tested the ability of a single intravitreal injection of foselutoclax (hereafter UBX1325), a novel senolytic small molecule inhibitor of antiapoptotic protein B-cell lymphoma-extra large, to mitigate the impact of diabetic macular edema. METHODS: Patients with diabetic macular edema with prior suboptimal response to anti-vascular endothelial growth factor treatment were randomly assigned (1:1) to either a single intravitreal injection of 10 μg of UBX1325 or sham and were followed for up to 48 weeks. The primary trial objective was to evaluate the safety and side-effect profile …

Intravitreal Dexamethasone Implant for the Treatment of Postoperative Macular Edema.

INTRODUCTION: Macular edema after cataract surgery (Irvine-Gass syndrome) or pars plana vitrectomy is a postoperative complication which can lead to permanent visual loss. Increased inflammatory substances, such as prostaglandins and cytokines, are discussed to be causative. Currently, there are no evidence-based guidelines for the treatment of postoperative macular edema. Intravitreal dexamethasone (DEX) could be effective by its anti-inflammatory effect. We examined the functional and morphological results of treatment with 0.7 mg intravitreal DEX implant (Ozurdex®). METHODS: In an observat…

Clinical evaluation of microcystic macular edema in patients with glaucoma.

PurposeTo investigate the prevalence of microcystic macular edema (MME) in patients with glaucoma and the relationship between glaucomatous visual field defects and MME.Patients and methodsWe analyzed 636 eyes of 341 glaucoma patients who underwent spectral domain optical coherence tomography (SD-OCT). MME was defined as vacuoles observed in the inner nuclear layer (INL) on SD-OCT. Quantitative assessment of MME area was performed using en-face imaging obtained swept-source OCT (SS-OCT) and Adobe Photoshop CS6 Extended software. These values were compared with the visual field results with the…
