Preservation of the inferior alveolar vasculonervous bundle in mandibular resective therapies: systematic review and report of two cases.

Creutzfeldt-Jakob disease (CJD) is one of several related disorders collectively called prion diseases. These disorders affect man and animals and are now known to be caused by the abnormal configuration of a naturally occurring protein, PrP(c). By mechanisms still not well understood, this natural protein is converted into a pathologic variant, PrP(sc). The disease is 'acquired' spontaneously perhaps by posttranslational conversion of a PrP(c) into a PrP(sc) population. This sporadic form of CJD has been reported worldwide with a frequency of 1/million. Other modes of acquisition include the

Central neurilemmoma of the jaw in concurrence with radicular cyst: a case report.

We report the features of a central neurilemmoma in the mandible of a patient having an inflammatory apical dental (radicular) cyst in the same region. A 29-year-old woman complained of numbness on the right side of the lower lip of 3 months duration and noted following endodontic therapy to the right mandibular first molar tooth. Panoramic radiography revealed a bilocular radiolucency in the right body of the mandible. Excisional biopsy was performed. The histopathology revealed neurilemmoma of the mandible in concurrence with inflammatory apical dental (radicular) cyst. No recurrence was det

Multiple radiolucent areas of the jaw.

A case of neurofibromatosis, or von Recklinghausen's disease, was presented. The skin manifestations include the development of numerous, variable sized, smooth-surfaced nodules (benign neural tumors) and asymmetric areas of hyperpigmentation called cafe-au-lait spots. Oral manifestations are not always apparent but, when they do occur, include multiple, discrete, nonulcerated mucosal nodules (typically benign neural tumors). Intraosseous tumors (usually neurofibromas) occasionally develop in the mandible as was reported in this patient. Clinicians should be aware of the characteristics of thi

Temporomandibular joint synovial chondromatosis with a giant cell component.

Synovial chondromatosis is a benign locally aggressive neoplasm that rarely affects the temporomandibular joint (TMJ). This report describes a case of synovial chondromatosis in a 34-year-old woman presenting TMJ pain and mandibular deviation with limited mouth opening. Imaging examinations revealed multiple calcified masses within the TMJ. During surgery, cartilage fragments were easily detached. On histological examination, multiple coalescent nodules of hyaline cartilage with areas of endochondral ossification and mild cell pleomorphism were observed, together with a significant giant cell

Elastofibromatous changes in giant cell fibroma and amalgam tattoo: Unusual findings in common oral lesions.

Several cases of elastofibromatous lesion affecting the oral mucosa have been reported. Clinically, these lesions may appear as small exophytic lesions or less often as white lesions. Therefore, fibrous hyperplasia and leukoplakia are not uncommonly considered in clinical differential diagnosis. Microscopically, elastic and fibrous connective tissue deposition is seen. Rarely, elastofibromatous changes can be detected when assessing intraoral lesions, including cysts, salivary gland neoplasms, and epithelial dysplasia. Here we report two oral lesions showing elastofibromatous changes, expandin

Endoscopic Transvestibular Transmandibular Approach for Trigeminal Schwannoma in Infratemporal Fossa and Parapharyngeal Space.

BACKGROUND: Schwannomas that involve the mandibular division of the trigeminal nerve and localize exclusively in the parapharyngeal space (PPS) and infratemporal fossa (ITF) are extremely rare, and a surgical approach to treat such tumors has not been well established. CASE DESCRIPTION: We present our experience in the treatment of a giant trigeminal schwannoma with wide extension in the ITF and PPS using an endoscopic transvestibular transmandibular approach. The clinical and radiologic findings, preoperative planning, advantages of the surgical approach, and clinical outcome are discussed.

Giant Cell Tumor of Infratemporal Fossa and Mandibular Condyle: A Case Report.

Giant cell tumors (GCT) of the bone are uncommon primary bone neoplasms that occur mainly in the epiphyses of long bones. However, GCT with tumoral growth in the cranium is rarely encountered, particularly in the temporal bone. GCT demonstrates benign histological features but is locally aggressive, and surgical excision is the treatment of choice. The authors describe a 31-year-old man who presented with trismus and an extensive GCT, which originated in the petrous and squamous portions of the temporal bone and extended into the left mandibular ramus and middle ear. He was treated by en bloc

Reconstruction of complex defects of the parotid region using a lateral thoracic wall donor site.

Radical treatment of parotid neoplasms may lead to complex parotid defects that present functional and aesthetic reconstructive challenges. We report our experience using the lateral thoracic wall as a single donor site. Between 2003 and 2009, four patients with malignant tumours in the parotid gland underwent radical parotidectomy and simultaneous reconstruction using a perforator latissimus dorsi cutaneous free flap (de-epithelialized and entire skin paddle in two cases each). A thoracodorsal nerve graft was used in all cases to replace the intraglandular branches of the facial nerve. Costal

Mandibular Osteoblastoma: A Case Report of a Rare Entity.

Osteoblastoma, an uncommon bone neoplasm characterized by the formation of osteoid tissue, constitutes a rare subset of bone tumors, representing only a negligible percentage of cases. While extensive research on the condition has identified a predilection for long bones and vertebrae, occurrences in facial bones are infrequent. This case report discusses a unique presentation in a nine-year-old female diagnosed with a tumor located in the mandibular parasymphysis region. Radiological examination revealed distinctive irregular contours surrounding the lesion, prompting a comprehensive investig

Management of a massive compound odontoma in a 9-year child.

The purpose of this paper is to describe the case of surgical management of massive compound odontoma with 36 denticles in a 9-year-boy who presented with a complaint of pain and swelling in mandibular left posterior region and retained 74. The denticles were removed after the removal of retained 74 completely and wound healing was observed. Odontomas are considered as hamartomatous dental malformation rather than true neoplasm of odontogenic origin. They are the most commonly occurring abnormally formed dental tissues that interfere with eruption of associated teeth. The eruption disturbances

Rehabilitating mandibulectomy defects: a review of prosthetic solutions with case series.

Prosthetic rehabilitation of mandibular defects is challenging for prosthodontists. Prosthetic treatment strategies require a detailed clinical evaluation of the patient. The first consideration is the mandibular continuity as the residual mandible deviates toward the site of the defect in segmental mandibulectomy without rigid reconstruction. In this condition, the initial prosthetic intervention often focuses on mandibular guidance therapy using interim appliances to re-educate muscles and re-establish acceptable occlusal relationships. Definitive restorations include conventional removable

Pathological mandibular fractures: a review of the literature of the last two decades.

Pathological mandibular fractures are rare, accounting for fewer than 2% of all fractures of the mandible. They could be defined as fractures that occur in regions where bone has been weakened by an underlying pathological process. Pathological fractures usually may follow surgical interventions such as third molar removal or implant placement, result from regions of osteomyelitis, osteoradionecrosis, and bisphosphonate-related osteonecrosis of the jaw, occur because of idiopathic reasons or be facilitated by cystic lesions, benign, malignant, or metastatic tumors. Pathological mandibular frac

Template based segmental mandibulectomy with nerve preservation and patient-specific PEEK plate reconstruction in a dog.

A 7-year-old French Bulldog presented with an acanthomatous ameloblastoma affecting approximately 30% of the right mandibular body. We utilized a patient-specific 3D-printed surgical template to perform lateral fenestration of the mandible and elevation of the inferior alveolar nerve (IAN), facilitating nerve preservation during subsequent segmental mandibulectomy. The resulting critical-sized bone defect was anatomically stabilized using a patient-specific polyetheretherketone (PEEK) bridging plate. The recovery process was uneventful, with maintained occlusion and orofacial sensitivity.Simil

Mental nerve dysfunction: a symptom of diverse mandibular disease.

Paraesthesia and anaesthesia of the mental nerve may result from a variety of pathological conditions, and in persistent cases of orofacial sensory disturbance thorough clinical assessment, including CT scanning, is vital to exclude underlying systemic or neoplastic disease. This paper presents three patients with right mental nerve dysfunction, and reviews the aetiology of mental nerve paraesthesia and anaesthesia.
