Assessment of a conductive component of heat transfer in selected insulation systems: an experimental study.

Post-traumatic hypothermia increases morbidity and mortality. Preventing heat loss plays an important role in improving outcomes of trauma patients. The purpose of the study was to compare selected thermal insulation systems regarding heat loss by conduction. An experimental study was conducted. We simulated the scenario mimicking a patient lying on a cold surface. A copper manikin of a human torso was placed on a cold water mattress. The manikin temperature was recorded every 1 min for 2 h. Tested insulation systems (Ultrathin reflective foil, LESS, HPMK, Blizzard, Mediwrap, Ferno) were place

Glottis Recognition Software Development Using Artificial Intelligence.

The use of video laryngoscopes has enhanced the visualization of the vocal cords, thereby improving the accessibility of tracheal intubation. Employing artificial intelligence (AI) to recognize images obtained through video laryngoscopy, particularly when marking the epiglottis and vocal cords, may elucidate anatomical structures and enhance anatomical comprehension of anatomy. This study investigates the ability of an AI model to accurately identify the glottis in video laryngoscope images captured from a manikin. Tracheal intubation was conducted on a manikin using a bronchoscope with record

Prediction of applied pressure on model lower limb exerted by an air pneumatic device.

Air pneumatic compression is a concept used for management of venous disease, including oedema. A typical air pneumatic compression device (PCD) consists of an inflatable sleeve composed of either single or multiple pressure chambers that encircle a limb. The aim of this research was to develop a mathematical model to predict the pressure applied by an air pneumatic device to an irregular cross-sectional lower limb manikin. The radius of curvature at any cross-section of the lower limb (i.e. calf (gastrocnemius), tibial crest (anterior edge of the tibia bone)) is irregular, and differs amongst

Resuscitation in late pregnancy.

This paper considers cardiopulmonary resuscitation in obstetric patients at term and the influence of aortocaval compression on the outcome. The maximum chest compression force produced by eight physicians was measured as a function of angle of inclination using an inclined plane. The compression force at an angle of 27 degrees is 80% of that in the supine position and the Cardiff resuscitation wedge, designed to prevent aortocaval compression, is described with this inclination. Midwives' expertise in basic life support 6 months after instruction was assessed using a manikin simulator. The ma

The AirView Study: Comparison of Intubation Conditions and Ease between the Airtraq-AirView and the King Vision.

We conducted a study assessing the quality and speed of intubation between the Airtraq with its new iPhone AirView app and the King Vision in a manikin. The primary endpoint was reduction of time needed for intubation. Secondary endpoints included times necessary for intubation. 30 anaesthetists randomly performed 3 intubations with each device on a difficult airway manikin. Participants had a professional experience of 12 years: 60.0% possessed the Airtraq in their hospital, 46.7% the King Vision, and 20.0% both. Median time difference [IQR] to identify glottis (1.1 [-1.3; 3.9] P = 0.019), fo

Comparison of the Glidescope and Pentax AWS laryngoscopes to the Macintosh laryngoscope for use by advanced paramedics in easy and simulated difficult intubation.

BACKGROUND: Intubation of the trachea in the pre-hospital setting may be lifesaving in severely ill and injured patients. However, tracheal intubation is frequently difficult to perform in this challenging environment, is associated with a lower success rate, and failed tracheal intubation constitutes an important cause of morbidity. Novel indirect laryngoscopes, such as the Glidescope and the AWS laryngoscopes may reduce this risk. METHODS: We compared the efficacy of these devices to the Macintosh laryngoscope when used by 25 Advanced Paramedics proficient in direct laryngoscopy, in a rando

FireSync EMS: A Novel Mobile Application for Burn Surface Area Calculation.

The percent total body surface area (TBSA) burned is a critical determinant of the required level of care, initial management, and prognosis in patients with burns. The current gold standard for estimating this measurement, the Lund-Browder (LB) chart, requires familiarity with its construction and may not be practical for use by first responders in the field. In this study, we present a novel burn surface area calculator mobile application developed for first responders and validate its accuracy. Infant, pediatric, and adult manikins were fabricated with 8 simulated burns of varying sizes and…

Effectiveness of a light-weight ice-vest for body cooling while wearing fire fighter's protective clothing in the heat.

The aim of the study was to examine the effects of wearing an ice-vest (ca 1 kg) on physiological and subjective responses in fire fighters. The experiments were carried out on a treadmill in a hot-dry environment. The physical cooling effect of the ice-vest was measured with a thermal manikin. The ice-vest effectively reduced skin temperatures under the vest. On average, heart rate was 10 beats/min lower, the amount of sweating was reduced by 13%, and subjective sensations of effort and warmth were lower during work with the ice-vest compared to work without it. Thermal manikin tests indicate

Detecting the Failures Nobody Sees: Applying Pareto Analysis and Failure Mode and Effects Analysis to Equipment Reliability in a Healthcare Simulation Institute.

Background Hospitals have used failure mode and effects analysis (FMEA) for two decades to examine clinical processes before they fail, and healthcare simulation has served as a useful instrument for carrying out that analysis. The equipment inside the simulation center has attracted far less scrutiny. Audiovisual (AV) and manikin failures interrupt teaching, and when video capture is affected, they destroy the recording on which objective structured clinical examination (OSCE) grading and debriefing depend. Methods We analyzed a 24-month log of reported equipment incidents (N = 22) from the s

Identification of the Optimal Position of a Nasal Oxygen Cannula for Apneic Oxygenation: A Technical Simulation.

Background: In a cannot-ventilate-cannot-intubate situation, careful preoxygenation with high FiO2 allowing subsequent apneic oxygenation can be life-saving. The best position for an oxygen supply line within the human airway at which oxygen insufflation is more effective than standard preoxygenation with a face mask is unknown. Methods: In this experimental study, we compared the effectiveness of preoxygenation by placing an oxygen cannula at the nose entrance, through the nose at the soft palatine, or at the base of the tongue; as a control we used ambient air. We connected a fully preoxygen

Comparison of direct laryngoscopy and digital intubation with and without bougie assistance in novice learners: A simulation-based study.

BACKGROUND: Endotracheal (ET) intubation is a common emergency procedure generally done with direct laryngoscopy (DL). Digital intubation (DI), which uses blind finger guidance to manipulate the ET tube, is rarely used but is useful if secretions block the view or a laryngoscope is not available. DI can also be done bougie-assisted. This study aimed to compare three different approaches to intubation in simulation. METHODS: Medical students without prior airway training were invited to participate in this simulation study on an airway manikin. Participants viewed brief instructional videos on

Effectiveness of Lay Bystander Hands-Only Cardiopulmonary Resuscitation on a Mattress versus the Floor: A Randomized Cross-Over Trial.

STUDY OBJECTIVE: Bystander cardiopulmonary resuscitation increases the likelihood of out-of-hospital cardiac arrest survival by more than two-fold. A common barrier to the prompt initiation of compressions is moving victims to the floor, but compression quality on a "floor" versus a "mattress" has not been tested among lay bystanders. METHODS: We conducted a prospective, randomized, cross-over trial comparing lay bystander compression quality using a manikin on a bed versus the floor. Participants included adults without professional health care training. We randomized participants to the ord

[Glottiscopes and videolaryngoscopes: a rational choice?].

Different airway devices can be used by paediatrician anaesthesiologists for difficult airway management in infant younger than 2 years. The four devices analyzed (Airtraq® [Prodol Meditec]; Glidescope® Cobalt [Verathon]; Vidéolaryngoscope [Storz]; laryngoscope Truview® [Truphatek]) increase the Cormack and Lehane grade against direct laryngoscopy and optimized external larynx movements. They need training, on manikin then in vivo, for an optimal use. The needed number of in vivo successful intubations is not determined. It is probably between 10 and 20. The choice between these devices, in ac
