Anatomic Landmark Technique Thoracic Paravertebral Nerve Block as a Sole Anesthesia for Modified Radical Mastectomy in a Resource-Poor Setting: A Clinical Case Report.

Mastectomy is mostly performed as definitive management for resectable breast cancer. Implementing paravertebral nerve block for patients with metastasis features of cancer to lungs and other organs, patients with co-morbidity, geriatrics, and malnourished individuals will eliminate the risks and complications of general anesthesia. Though thoracic paravertebral block is an established technique as postoperative pain management for breast surgery, there is no conclusive evidence on its use as a sole anesthetic for modified radical mastectomy. In this case report, we present a 33-year-old woman

Creation of a Successful Transposed Antecubital Radiocephalic Arteriovenous Fistula in Patient with Known Ipsilateral Modified Radical Mastectomy, Postoperative Radiation, and Proximal Central Venous Occlusion.

Native arteriovenous fistula (AVF) placement in patients with ipsilateral mastectomy and radiation has been avoided because of concerns regarding central venous outflow obstruction. To our knowledge, only 3 such cases have been reported. We present a patient with bilateral mastectomies and right-sided radiation therapy presenting for vascular access in the setting of multiple failed AVF in her left upper extremity and infected-groin catheter, central catheters, and axillary loop graft. We created and superficialized a radiocephalic AVF in her right upper extremity in the setting of central vei

Ultrasound-guided transversus thoracic muscle plane-pectoral nerve block for postoperative analgesia after modified radical mastectomy: a comparison with the thoracic paravertebral nerve block.

BACKGROUND: Modified radical mastectomy (MRM) is the most effective and common type of invasive surgery for breast cancer. However, it causes moderate to severe acute pain and even lasts for a long postoperative period. Transversus thoracic muscle plane-pectoral nerve block (TTP-PECS) is a novel and promising interfacial plane block which can provide analgesia for MRM while thoracic paravertebral nerve block (TPVB) is also widely used for this purpose. This study compared the postoperative analgesia between the ultrasound-guided TTP-PECS and TPVB in patients undergoing MRM. METHODS: In this r

Analgesic and opioid-sparing effects of single-shot preoperative paravertebral block for radical mastectomy with immediate reconstruction: A retrospective study with propensity-adjusted analysis.

BACKGROUND: Before radical mastectomy with immediate latissimus dorsi flap reconstruction, single-shot paravertebral block (PVB) can be added to general anesthesia to improve analgesia. As this technique was introduced in 2014 in our centre, our aim was to retrospectively assess its clinical effects. METHODS: Among 175 patients who underwent surgery over four years (40 receiving PVB), we studied the intra-operatively administered doses of opioids and vasopressors, postoperative pain as estimated by a composite score based on the intensity scores for maximum postoperative pain and the amounts

Erector spinae plane block (ESPB) versus modified pectoral plane block (PECS II) in managing post modified radical mastectomy pain: A systematic review and meta-analysis.

BACKGROUND: Breast cancer remains the most frequently diagnosed malignancy among females worldwide. Surgical management is the cornerstone of the therapeutic strategy, however associated with multiple postoperative complications, particularly acute pain and shoulder mobility limitations. Regional anesthetic techniques have been proposed to counteract this phenomenon, such as the modified pectoral plane block (PECS II) and erector spinae plane block (ESPB). This systematic review and meta-analysis aims to compare between ESPB and PECS II in managing post-operative pain in modified radical maste

Liposomal Bupivacaine for Serratus Anterior Plane Block in Modified Radical Mastectomy: Protocol for a Randomized, Double-Blind Trial Evaluating Pain-Related and Early Recovery Outcomes.

PURPOSE: Modified radical mastectomy is commonly associated with substantial postoperative pain, opioid use and impaired early recovery. Serratus anterior plane block (SAPB) is increasingly used as part of multimodal analgesia for breast surgery. Although SAPB and quality of recovery outcomes have both been studied in breast surgery, published evidence regarding the use of liposomal bupivacaine in SAPB for modified radical mastectomy remains limited. This trial was designed to address whether adding liposomal bupivacaine to bupivacaine hydrochloride (bupivacaine HCl) for SAPB improves pain-rel

Rethinking Radical Mastectomy Anesthesia: Combined Erector Spinae Plane and Pectoral Nerve Block Type II Blocks in Bullous Chronic Obstructive Pulmonary Disease Without Positive-Pressure Ventilation.

Breast surgery with axillary dissection is traditionally performed under general anesthesia with positive-pressure ventilation. However, in patients with bullous chronic obstructive pulmonary disease (COPD), mechanical ventilation may increase the risk of barotrauma and pneumothorax. We report the case of a 75-year-old male with moderate COPD and a large pulmonary bulla who underwent modified radical mastectomy with axillary dissection using combined erector spinae plane and pectoral nerve block type II, associated with sedation and preservation of spontaneous ventilation. The procedure was co

A randomised comparative study of erector spinae plane block versus low-dose ketamine-dexmedetomidine intravenous infusion as intraoperative opioid-free analgesia for modified radical mastectomy.

BACKGROUND AND AIMS: Opioid-sparing analgesia for acute postoperative pain after breast cancer surgery is crucial due to opioid-related side effects. The utilisation of erector spinae plane block and low-dose intravenous ketamine-dexmedetomidine are widely recognised as non-opioid analgesic methodologies. The objective of this study was to conduct a randomised trial to examine the analgesic efficacy of both approaches while minimising the use of opioids. METHODS: Seventy-two female patients scheduled for unilateral modified radical mastectomy were recruited. They were allocated randomly to Gr

Findings from NSABP Protocol No. B-04: comparison of radical mastectomy with alternative treatments. II. The clinical and biologic significance of medial-central breast cancers.

Findings from 1665 women with primary breast cancer, treated at 34 NSABP institutions in Canada and the United States, have failed to demonstrate that patients with medial-central tumors had a greater probability of developing distant metastases or dying than did those with lateral tumors despite the greater incidence of internal mammary (IM) node involvement when tumors are medial-central in location. A comparison of patients with similar clinical nodal status and tumor location who were treated either by radical mastectomy (RM) or by total mastectomy plus radiation therapy (TM + RT) failed t
