BACKGROUND: Despite the increased use of nipple-sparing mastectomies, there are limited data examining long-term cancer recurrence rates in these patients. The objective of this study was to analyze breast cancer recurrence in patients who underwent therapeutic nipple-sparing mastectomy with a median of 10 years of follow-up.
METHODS: All patients undergoing nipple-sparing mastectomy at a single institution were retrospectively reviewed temporally to obtain a median of 10 years of follow-up. Patient demographic factors, mastectomy specimen pathologic findings, and oncologic outcomes were anal

Nipple-sparing mastectomy is an alternative to skin-sparing mastectomy in select patients. Increasing evidence supports its use in the setting of breast cancer, however concerns still exist regarding oncological safety. The aim of this systematic review was to evaluate long-term oncological outcomes of patients who underwent nipple-sparing mastectomy for breast cancer. A systematic review of the literature was performed to evaluate oncological outcomes in patients with breast cancer who underwent nipple-sparing mastectomy. Five major databases (PubMed, Embase, Scopus, Web of Science and Cochra

Breast reconstruction using a modified latissimus dorsi island flap is presented. The whole muscle is raised on its vascular pedicle, folded beneath the skin island and sutured in layers, thus substituting the volume of an endoprosthesis. The method may give satisfactory results in carefully selected cases (mostly in women with a small contralateral breast). It can also be applied in reconstruction after subcutaneous mastectomy.

Three cases are presented of complications after bilateral subcutaneous mastectomy with immediate prosthetic replacement, performed for benign breast disease in women who ranged in age from 24 to 32 years, by surgeons outside the field of plastic surgery. This article is submitted as a plea to curb a rapidly increasing tendency by general surgeons to perform a basically mutilating operation on young women with benign breast disease.

Bilateral ductal carcinoma in situ of breast is a very rare disease in men. Ductal carcinoma in situ (DCIS) is an abnormal proliferation that involves the ductal epithelium and it has the potential of evolving into an invasive tumour. Gynaecomastia (female like breast in men) is a benign condition though it is associated with a reported 3% incidence of unilateral invasive breast cancer.(2) Synchronous bilateral breast cancer in association with gynaecomastia is exceptionally rare. The recommended treatment for DCIS in male is mastectomy. So far only 2 cases of bilateral DCIS in male patients h

Skin-sparing mastectomy has been advocated as an oncologically safe approach for the management of patients with early-stage breast cancer that minimizes deformity and improves cosmesis through preservation of the skin envelope of the breast. Because chest wall skin is the most frequent site of local failure after mastectomy, concerns have been raised that inadequate skin excision could result in an increased risk of local recurrence. Precise borders of the skin resection have not been well established, and long-term local recurrence rates after skin-sparing mastectomy are not known. The purpo

Ventriculoperitoneal (VP) shunts are the standard treatment for hydrocephalus, and the distal catheter is often routed subcutaneously across the anterior chest wall. The development of ipsilateral breast cancer is rare, with fewer than 15 cases reported in the English literature, and there is no established consensus regarding the management of the shunt catheter during mastectomy. We report a 58-year-old shunt-dependent woman with a right-sided VP shunt placed for post-subarachnoid hemorrhage hydrocephalus, who was diagnosed with right breast cancer (cT2N1M0, stage IIB). Preoperative imaging

INTRODUCTION: Skin-reducing mastectomy has been applied to several surgical techniques in which subcutaneous mastectomy is associated with various types of skin reduction, with preservation of a lower dermal flap to reinforce the inferior lateral seat of an implant. The aim of the study is to present a case series of patients with pendulous/ptotic and/or large-sized breasts treated for breast cancer at the Breast Surgery Unit of Istituto Nazionale Tumori IRCCS Fondazione "G. Pascale", Naples, Italy, with the superomedial pedicle skin-reducing mastectomy technique, two-stage reconstruction, and

BACKGROUND: Two-stage tissue expander/implant breast reconstruction has become increasingly popular, but reports have varied widely in the incidence of complications associated with this approach. A large consecutive patient experience using low-height tissue expanders in immediate breast reconstruction is presented and discussed in conjunction with other recent reports using this approach following mastectomy.
METHODS: A total of 172 consecutive patients underwent immediate breast reconstruction following mastectomy, with placement of a total of 246 tissue expanders. All expanders used were

BACKGROUND: Mastoscopic surgery is proven to have lower incidence of postoperative complications and better postoperative recovery than traditional breast cancer surgery. This study aimed to examine the feasibility of mastoscopic modified radical mastectomy (MRM) with skin nipple-areola preservation under air cavity-free suspension hook and stage I silicone prosthesis implantation (SMALND) compared with routine MRM.
METHODS: This was a retrospective study of patients who underwent MRM for breast cancer at the Shengjing Hospital Affiliated to China Medical University between January 1, 2019, a

Invasive ductal carcinoma of the breast is known to typically metastasise to the lungs, liver, bone and brain, with subcutaneous soft tissue involvement being rare, with only eight cases reported internationally. We report the case of a woman with a history of left-sided invasive ductal carcinoma treated with a mastectomy and adjuvant therapy, who presented 7 years later with a 2.8 cm subcutaneous lesion over the contralateral shoulder. Histopathological examination and immunohistochemistry confirmed metastatic invasive ductal carcinoma, displaying a morphological and receptor profile congruen

Background and Objectives Surgery to masculinize the chest is a frequently sought-after procedure by transmen who wish to conform their physical appearance to their gender identity. In our study, the outcomes of surgical masculinization comprising subcutaneous mastectomy through a modified elliptical incision, liposuction, reduced inert nipple-areola grafting, and quilting stitches were analyzed. Materials and Methods Thirty-two transmen with large breasts and hypertrophied nipples and areolae underwent the above-mentioned masculinizing procedure in this prospective cohort study. Our statist

BACKGROUND: To restore sensation after breast reconstruction, a modified surgical approach was employed by identifying the cut fourth intercostal lateral cutaneous branch, elongating it with intercostal nerve grafts, and coapting it to the innervating nerve of the flap or by using direct neurotization of the spared nipple/skin.
METHODS: This was a retrospective case-control study including 56 patients who underwent breast neurotization surgery. Breast operations included immediate reconstruction after nipple-sparing mastectomy (36 patients), skin-sparing mastectomy (8 patients), and delayed r

BACKGROUND: Nipple-sparing mastectomy (NSM) has evolved as a standard surgical option. The NSM complication rate remains high in large breasts. To reduce the risk of necrosis, several authors have proposed delayed procedures to enhance blood supply to the nipple-areola complex (NAC). The purpose of this study in a porcine model was to show adequate redirection of NAC perfusion by neoangiogenesis through circumareolar scars.
METHODS: Delayed two-staged NSM was simulated in 52 nipples (six pigs) with a 60-day interval. The nipples underwent a full-thickness, circumareolar incision onto the musc

Patients with macromastia and/or moderate-severe ptosis who desire mastectomy are typically offered skin sparing mastectomies. Recent technical advances have broadened the inclusion criteria to allow for nipple sparing mastectomy (NSM) in this population; however, they have been limited to patients with unifocal disease. We report on the multidisciplinary approach used to offer a NSM to a patient with multicentric triple-negative breast cancer and symptomatic macromastia with severe ptosis in the setting of positive genetic testing. We discuss the combination of a staged oncoplastic approach w

Capsular contracture around gel and gel-saline implants is the most common complication following breast augmentation and reconstruction. Thirty-one patients with postaugmentation gel-implant contractures were treated with open capsulotomy and replacement with polyurethane-covered implants. Eighty-seven percent remain soft after a minimum follow-up of 14 months. Results of replacement after subcutaneous mastectomy and reconstruction are more varied and less successful. Results in 14 patients are presented.

One case of an ulcerating paraffinoma of the male mammary gland and another of the penis are reported. The former is the third and the latter the eighth of its kind described in the literature. Psychopathological features were associated with both cases, and additionally impotence with the latter. The treatment of paraffinomas is surgical. Subcutaneous mastectomy was performed on the mammary gland, leaving a high proportion of the affected skin in situ. Recovery was slow and the cosmetic end result not satisfactory. Re-injection of paraffin later required total excision of the entire anterior

BACKGROUND: Societal awareness of transgender individuals has led to increased acceptance and demand for sex-confirming surgery. In female to male transsexuals, the most common procedure is removal of breast tissue to masculinize the chest.
METHODS: Eighty-eight transgender patients underwent either a subcutaneous nipple-sparing mastectomy (NSM) with or without a periareolar mastopexy or nipple reduction, or bilateral mastectomies with free nipple grafts (MFNG) with or without nipple reduction. Surgical techniques are discussed. Demographic data, use of testosterone, specimen weights, rates o
