Children with severe Trichuris trichiura infection pose significant social, clinical, and therapeutic problems in Malaysia. Thirty such children were investigated, and mebendazole was found to be safe and effective in the treatment of severe trichuriasis but it had to be given for a longer period than currently recommended. A poor correlation was found between egg load and worm burden in these children. The effect of micronised mebendazole on Echinococcus granulosus and Taenia hydratigena has been investigated in a trial involving 175 dogs. The hydatid organism was eliminated from the dogs at dose rates of either 160 mg/kg given on one occasion or 20 mg/kg given on two occasions. A single dose of 20 mg/kg eliminated T hydratigena. The drug was readily accepted when given in food and there were no untoward sequelae.

Mebendazole, a well-known anti-helminthic drug in wide clinical use, has anti-cancer properties that have been elucidated in a broad range of pre-clinical studies across a number of different cancer types. Mebendazole is currently used in the treatment of hydatid disease. Its poor absorption from the gastrointestinal tract and low bioavailability aroused further research on new formulations of mebendazole to increase the bioavailability and improve the therapeutic efficacy. This review summarizes the recent research progress.

We report a 4-year-old girl with disseminated cystic echinococcosis in the lung and the liver and a solitary cyst in the left kidney. Mebendazole therapy produced complete resolution of the lung and kidney cysts. In the liver, most of the smaller cysts disappeared, whereas the larger cysts showed only partial response and required surgical excision. Our experience reinforce the finding in previous reports that long term medical treatment of cystic echinococcosis with mebendazole can be lifesaving in cases that are unmanageable by surgical treatment.

The study included a universe of 376 persons, male and female, positive and asymptomatic, monoparasitic and multiparasitic. They were divided in treatment groups with not less than 50 people. The ultrastructural changes induced in hydatid cysts from lungs of sheep after medication with one or two courses of mebendazole (Ovitelmin), each lasting 3 wk, are described. The hydatid germinal membrane tissue collected from the treated sheep showed complete degeneration of the tegumental and subtegumental structures and minor degree of calcification. Lung hydatid cysts of the untreated sheep presented normal ultrastructural configurations.
