Prioritizing the Health of Underserved Communities in Prenatal Genetics.

Prenatal cell-free genetic screening (cfDNA) for fetal chromosomal conditions, including aneuploidy and microdeletion syndromes, is increasingly offered in diverse patient populations and clinical settings, including in safety-net clinics and other public health settings. Companies testing for cfDNA have argued for first-tier cfDNA by state Medicaid programs, arguing that differences in coverage of these screens between public and private insurance constitute a health disparity. We contend that this framing is unhelpful considering the ongoing maternal and perinatal health crisis affecting low

Project ECHO Diabetes Trial Improves Outcomes for Medically Underserved People.

OBJECTIVE: The Project Extension for Community Healthcare Outcomes (ECHO) model is used in 180 countries to address chronic disease care through a provider empowerment, tele-education approach. Few studies have rigorously evaluated the impact of the program on patient outcomes using randomized designs. RESEARCH DESIGN AND METHODS: Implementation of an ECHO Diabetes program was evaluated using a stepped-wedge design with recruitment of 20 federally qualified health centers (FQHCs) across California and Florida with randomized, phased-in intervention entry. Participating FQHCs (referred to as "

INTRODUCTION: Demand for dental care in Basse-Normandie has been severely affected by the reduced availability of local services. One of the missions of hospital dental services is to respond to these difficulties in accessing care. PURPOSE OF THE RESEARCH: The objective of this study is to determine how hospital activity fits into the local dental care offer. To do this, we compared the activity of the dental service of the Caen hospital with that of private practices. RESULTS: The proportion of young patients, particularly those under fifteen, was greater at the university hospital than in

Preliminary clinical algorithm to optimise remote delivery of paediatric concussion care in Canada's North.

Concussion is a form of traumatic brain injury that affects thousands of children and adolescents across Canada annually. With timely access to comprehensive medical care, the majority of patients with acute concussion will recover within 1-4 weeks. Those who develop persistent post-concussion symptoms often benefit from early recognition and referral to multi-disciplinary concussion clinics that have the personnel and resources to meet their complex needs. Youth who live in remote and isolated communities within Canada's North, a significant proportion of whom are Indigenous, face unique barr

Contraceptive risk-taking in a medically-underserved, low-income population.

This study examines the phenomenon of contraceptive risk-taking, which is the non-use of contraception by women who are sexually active, fertile and not pregnant or trying to become pregnant. Based on a survey sample of low-income women in their childbearing years who had not received family planning care or advice from a doctor or clinic for at least three years, the study analyzed demographic predictors of contraceptive risk-taking, and also compared levels of contraceptive knowledge and pregnancy histories of non-contraceptors versus barrier method contraceptors. Contraceptive risk-taking w

Maternal mortality in a Nigerian teaching hospital - a continuing tragedy.

The aim of this review is to determine the maternal mortality ratio (MMR) in a Nigerian tertiary health institution (University of Ilorin Teaching Hospital, Ilorin, Nigeria). The review was done through a retrospective analysis of maternal mortality records. The MMR for the 6-year period (1997-2002) was 825 per 100,000 live births. The common causes of maternal mortality included severe pre-eclampsia/eclampsia, 30 (27.8%); haemorrhage, 22 (20.4%) and complications of unsafe abortion 16 (14.8%). Grandmultiparous and patients aged 40 years and above were at the highest risk. This hospital-based

Geospatial Access to Pediatric Cardiac and Cardiac-Surgical Services by Neighborhood-Level Determinants of Health.

BACKGROUND: Access to quality health care is a key social determinant of health. We determined whether geographic access to pediatric cardiac services differed by neighborhood-level opportunity, racial and ethnic population composition, and urbanicity. METHODS: Geospatial analysis delineated geographic access to hospitals with pediatric cardiac and cardiac-surgical services. Geographic access was defined in 4 drive-time catchments (0-30, 31-60, 61-120, and 121-240-minutes) and dichotomizing direct (≤60-minute) versus limited (>60-minute) access. Mean predicted Child Opportunity Index (COI), p

Access to Oral Health Care: A National Crisis and Call for Reform.

Purpose: According to the report Healthy People 2020, oral health is integral to overall health and access to dental services is essential to promoting and maintaining good oral health. Yet, those who need dental care the most are often the least likely to receive it. The dental hygiene profession is poised to play a pivotal role in the resolution of oral health disparities. The purpose of this manuscript is to examine the critical issue of access to oral health care in the United States from various perspectives and consider potential implications for dental professionals and the oral health

Food choices and practices during pregnancy of immigrant and Aboriginal women in Canada: a study protocol.

BACKGROUND: Facilitating the provision of appropriate health care for immigrant and Aboriginal populations in Canada is critical for maximizing health potential and well-being. Numerous reports describe heightened risks of poor maternal and birth outcomes for immigrant and Aboriginal women. Many of these outcomes may relate to food consumption/practices and thus may be obviated through provision of resources which suit the women's ethnocultural preferences. This project aims to understand ethnocultural food and health practices of Aboriginal and immigrant women, and how these intersect with re

The influence of medical and nonmedical factors to the progression of renal osteodystrophy.

We report a 13-year-old boy hospitalized for the first time at the age of 17 months with clinical and laboratory signs of chronic kidney disease (CKD) and renal osteodystrophy caused by severe obstructive uropathy of the single kidney. Prevention and treatment of renal osteodystrophy has been target for aggressive therapy and the great challenge for pediatric nephrologists. The outcome of the therapy of renal osteodystrophy is influenced by medical and non-medical factors. It was concluded that the place of living (in our example a small village distant from primary care physicians, far from t

Postpartum hemorrhage in developing countries: is the public health community using the right tools?

OBJECTIVES: To identify new and underutilized technologies that may assist in reducing maternal mortality due to obstetric hemorrhage. METHODS: Review of published and unpublished literature, including systematic reviews of randomized trials and individual clinical studies. RESULTS: Hemorrhage, primarily postpartum, accounts for approximately 25% of maternal deaths globally. Uterotonic drugs offer great promise for both prevention and management of postpartum hemorrhage (PPH). Other technologies--such as anti-shock garments, umbilical vein injection of oxytocin, and simple anemia detection m

The Potential for Community-Directed Interventions: Reaching Underserved Populations in Africa.

Community-directed interventions (CDIs) have the potential for fulfilling the promise of primary health care by reaching underserved populations in various settings. CDI has been successfully tested by expanding access to additional health services like malaria case management through local effort in communities where ivermectin distribution is ongoing. The question remains whether the CDI approach has potential in communities that do not have a foundation of community-directed treatment with ivermectin. The UNICEF/UNDP/World Bank/WHO Special Program of Research and Training in Tropical Diseas
