Loss of Subsidized Drug Coverage and Mortality among Medicare Beneficiaries.

BACKGROUND: A total of 14 million Medicare beneficiaries receive the Low-Income Subsidy (LIS), which reduces cost sharing in Medicare Part D. Losing the LIS may impede medication access and affect mortality. METHODS: Using 2015-2023 Medicare data, we identified dual-eligible Medicare-Medicaid beneficiaries, who automatically receive the LIS, and calculated annual rates of Medicaid and LIS loss. To examine the relationship between LIS loss and mortality, we leveraged a natural experiment arising from the relationship between the timing of Medicaid disenrollment and subsequent LIS loss. We comp

The cost of glaucoma care provided to Medicare beneficiaries from 2002 to 2009.

PURPOSE: To estimate payments for glaucoma care among Medicare beneficiaries from 2002 to 2009. DESIGN: Database study. PARTICIPANTS: Data from a 5% random sample of Medicare billing information from 2002 to 2009. METHODS: Medicare beneficiaries, aged 65 years or older, with both Parts A and B fee-for-service (FFS) enrollment comprised the annual denominator. For each year, we included those with a defined glaucoma diagnostic code linked to a glaucoma visit, diagnostic test, or laser/surgical procedure. Open-angle, angle-closure, and other glaucoma were categorized separately. Claims were c

Is patient insurance type related to physician recommendation, administration and referral for adult vaccination? A survey of US physicians.

This study evaluated physician practices and perceived barriers for influenza, tetanus, diphtheria, pertussis (Tdap), and zoster vaccination of adults in the United States (US), with emphasis on patients with Medicare versus commercial insurance. A cross-sectional internet-based survey of board-certified general/family practitioners and internists (N = 1,000) recruited from a national US physician panel was conducted in May 2017. For influenza, rates of physician recommendation (84% of Medicare patients, 82% of commercially-insured patients), administration (80% Medicare, 78% commercial), and

Herpes zoster vaccination among Medicare beneficiaries with and without prescription drug coverage.

INTRODUCTION: The Inflation Reduction Act (IRA) eliminated cost sharing for Medicare Part D-covered vaccines but did not address the cost burden faced by Medicare beneficiaries who did not have prescription drug coverage. This study aimed to determine the characteristics of beneficiaries without prescription drug coverage and to assess the association between the receipt of a herpes zoster vaccine and prescription drug coverage status. METHODS: We used the 2019-2023 National Health Interview Survey and included Medicare beneficiaries aged 65 years and older who enrolled in both Parts A and B

Long-acting bronchodilators with or without inhaled corticosteroids and 30-day readmission in patients hospitalized for COPD.

BACKGROUND: The ability of a long-acting muscarinic antagonist (LAMA) and long-acting beta 2 agonists (LABAs; long-acting bronchodilators, LABDs) with or without inhaled corticosteroids (ICSs) to reduce early readmission in hospitalized patients with COPD is unknown. METHODS: We studied a 5% sample of Medicare beneficiaries enrolled in Medicare parts A, B and D and hospitalized for COPD in 2011. We examined prescriptions filled for LABDs with or without ICSs (LABDs±ICSs) within 90 days prior to and 30 days after hospitalization. Primary outcome was the 30-day readmission rate between "users"

Assessment of Medicare Advantage Stars Measures: Impact on Kidney Care Quality and Future Opportunities.

The Centers for Medicare & Medicaid Services uses the Star quality program to evaluate Medicare Advantage (Medicare Part C) care quality. Since the 21st Century Cures Act, ESRD enrollment within Medicare has shifted from <30% in 2021 with Medicare Advantage to more than 50% in 2024. Given this influx and growing CKD prevalence, evaluating how Stars measures shape kidney care has significant population health implications. The American Society of Nephrology Quality Committee convened a working group to assess the 2026 Medicare Part C and D Stars quality measures. Measures were reviewed using a

Price and Adoption of Exhalation Delivery System With Fluticasone, 2018-2023.

FDA approved an exhalation delivery system with fluticasone (EDS-FLU) for treatment of chronic rhinosinusitis with (2017) and without (2024) nasal polyposis. Concerns have emerged that high prices may limit access, but little is known about the extent of adoption. We analyzed publicly available Securities and Exchange Commission filings and Medicare Part D spending data to characterize EDS-FLU utilization, spending, and prices between 2018 and 2023. During this period, annual EDS-FLU utilization and spending increased 10.3-fold (32,869 to 339,400 prescriptions) and 12-fold ($21.1 million to $2

Differences in Home Health Services and Outcomes Between Traditional Medicare and Medicare Advantage.

IMPORTANCE: Private Medicare Advantage (MA) plans recently surpassed traditional Medicare (TM) in enrollment. However, MA plans are facing scrutiny for burdensome prior authorization and potential rationing of care, including home health. MA beneficiaries are less likely to receive home health, but recent evidence on differences in service intensity and outcomes among home health patients is lacking. OBJECTIVE: To examine differences in home health service intensity and patient outcomes between MA and TM. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study was conducted from Januar

What is Medicare Advantage and Why is it the Most Important Contemporary Policy Affecting Kidney Disease?

Medicare Advantage (MA) enrollment among patients receiving dialysis has surged following the passage of the 21st Century Cures Act, which lifted prior restrictions on enrollment. As MA becomes the plurality payer for dialysis, understanding its implications for patients, providers, and policymakers is critical. MA offers out-of-pocket spending caps and additional services not covered under fee-for-service (FFS) Medicare. Some plans also prioritize care coordination, which may improve patient outcomes. However, concerns remain regarding limited provider networks, prior authorization barriers,

Medicaid Eligibility Loss Among Dual-Eligible Beneficiaries Before and During COVID-19 Public Health Emergency.

IMPORTANCE: Medicaid coverage loss can substantially compromise access to and affordability of health care for dual-eligible beneficiaries. The extent to which this population lost Medicaid coverage before and during the COVID-19 public health emergency (PHE) and the characteristics of beneficiaries more at risk for coverage loss are currently not well known. OBJECTIVE: To assess the loss of Medicaid coverage among dual-eligible beneficiaries before and during the first year of the PHE, and to examine beneficiary-level and plan-level factors associated with heightened likelihood of losing Med
