The impact of hospitalization on mortality in patients with connective tissue disease-associated interstitial lung disease: a medical records review study.

BACKGROUND: Interstitial lung disease (ILD) remains one of the most important causes of morbidity and mortality in patients with Connective Tissue Diseases (CTD). This study evaluated the impact of hospitalization on mortality in an ethnically and racially diverse cohort of CTD-ILD patients. METHODS: We conducted a medical records review study at Montefiore Medical Center, Bronx, NY. We included 96 patients and collected data on demographic characteristics, reasons for hospitalization, length of stay, immunosuppressant therapy use, and mortality. We stratified our patients into two cohorts: h

Munchausen syndrome presenting as major trauma.

The case is described of a man who feigned being struck by a vehicle, leading to an unnecessary major trauma response by the ambulance service and hospital. Suspicion that the patient suffered from Munchausen syndrome was confirmed by later investigation. Accident and emergency staff should file details of such patients on the department computer records system, where available, so that staff are alerted automatically to their presence, and share this information with neighbouring hospitals. All such patients should be treated according to ATLS guidelines until injury is ruled out, as for any

Interrupting providers with clinical decision support to improve care for heart failure.

BACKGROUND: Evidence-based therapy for heart failure remains underutilized at hospital discharge, particularly for patients with heart failure who are hospitalized for another cause. We developed clinical decision support (CDS) to recommend an angiotensin converting enzyme (ACE) inhibitor during hospitalization to promote its continuation at discharge. The CDS was designed to be implemented in both interruptive and non-interruptive versions. OBJECTIVES: To compare the effectiveness and implementation of interruptive and non-interruptive versions of a CDS to improve care for heart failure. ME

Drug Allergy Alert System in a Spanish University Hospital: Description and Dynamics of Use.

INTRODUCTION: The drug allergy alert system reduces the frequency of adverse drug events, although it is subjected to collateral effects, since 80-90% of alerts are not real, and a large percentage of alerts are overridden (46.2-96.2%). We reviewed how the alert system is used at University Hospital Fundación Alcorcon (HUFA). METHODS: Data were obtained from the drug allergy alert and the alert overriding notification forms (both in the period 2011-20). We also recorded drug allergy diagnoses at HUFA, drug consumption in primary care in 2016. We calculated the incidence of drug allergy alert

Flood fails to 'wash out' ED's ability to communicate.

When disaster strikes, communication often can be one of the first victims. However, during the recent floods in Iowa the communications in the ED at Mercy Medical Center in Cedar Rapids, IA, continued working flawlessly due to system redundancies and advanced planning. The strategies included: a wireless documentation system that was not dependent on being in a specific room or location; phones using a Voice over Internet Protocol, which enabled them to be plugged into any jack in the building; a statewide radio system that enabled the staff to speak to other hospitals about potential patient

A more accurate approach to define abortion cohorts using linked administrative data: an application to Ontario, Canada.

BACKGROUND: The shifting landscape of abortion care from a hospital-only to a distributed service including primary care has implications for how to identify abortion cohorts for research and surveillance. The objectives of this study were to 1) create an improved approach to define abortion cohorts using linked administrative data sets and 2) evaluate the performance of this approach for abortion surveillance compared with standard approaches. METHODS: We applied four principles to identify induced abortion cohorts when some services are delivered beyond hospital settings; 1) exclude early p

Comparison of handheld computer-assisted and conventional paper chart documentation of medical records. A randomized, controlled trial.

BACKGROUND: Daily documentation and maintenance of medical record quality is a crucial issue in orthopaedic surgery. The purpose of the present study was to determine whether the introduction of a handheld computer could improve both the quantitative and qualitative aspects of medical records. METHODS: A series of consecutive patients who were admitted for the first time to a thirty-six-bed orthopaedic ward of an academic teaching hospital for a planned operation or any other treatment of an acute injury or chronic condition were randomized to daily documentation of their clinical charts on a

Pharmacy-nursing report form used to monitor and adjust schedules of large volume parenteral solutions.

The use of an i.v. report form to regularly monitor and adjust the schedules of large volume parenteral solutions and irrigations is discussed. The departments of pharmaceutical services and nursing instituted a program that allowed both departments to compare the schedules of large volume parenterals and irrigations for each shift. This program centers around the use of an i.v. report form, for which clinical nurses record information concerning the progress of i.v. fluids and irrigations. The completed report is forwarded to the pharmacy, where pharmacy personnel use the information to conti

The Medication Affordability, Accessibility, and Availability in Care Transitions (Med AAAction) Study: Design and methods for a pharmacy-led trial implemented in low-income populations.

BACKGROUND: Medication nonadherence disproportionately affects low-income populations, especially during care transitions, by increasing readmissions, emergency visits, and mortality. These populations face financial, transportation, and coordination barriers that traditional medication therapy management often fails to address, highlighting the need for innovative solutions. We are conducting a pragmatic randomized controlled trial to determine the effectiveness of the patient-centered Medication Affordability, Accessibility, and Availability in Care Transitions (Med AAAction) intervention to

Evaluation of the Medication Never Events and Related Factors: A Nationwide Database Study.

BACKGROUND: The present study was conducted to evaluate the prevalence of medication errors regarded as never events in Iran and the related factors and develop a strategic operation plan (SOP) for its prevention. MATERIALS AND METHODS: A nationwide cross-sectional study with a documentary approach was conducted. For this purpose, all documented reports of medication-never events (over 515 cases) were extracted from the hospital Information system (HIS) of Iran between 2018 and 2020. After reporting each error, using root cause analysis (RCA), factors related to each medication never event we

Unintended medication discrepancies across key stages of the in-hospital medication process: a retrospective real-world study in hospitalized patients.

BACKGROUND: The in-hospital medication process is vulnerable to medication errors, especially during care transitions, when medication discrepancies occur most frequently. However, previous studies focused only on medication errors at single points in this process. This study therefore investigated key stages of the medication process. Also, we examined potential risk factors for unintended medication discrepancies (UMDs) at admission. METHODS: A single-center retrospective study was conducted on seven reference wards from different medical specialties. Technical UMDs at admission, intra-hospi

An Evaluation of Clinician and Informatician Perspectives on Medication Harm Risk Prediction Tools for Use in a Tertiary Hospital Setting.

INTRODUCTION: Risk prediction tools (RPTs) use clinical data to estimate the likelihood of adverse events such as medication harm and institute preventive strategies. However, their adoption in hospitals can be limited by implementation challenges, workflow integration barriers, and technical constraints. OBJECTIVES: This study aimed to determine clinicians' and informaticians' perspectives on the requirements, factors influencing adoption of digital RPTs for medication harm in hospitals, and to identify effective implementation strategies. METHODS: Semi-structured interviews were conducted

The role of the nutritional support team in preventing and identifying complications of parenteral and enteral nutrition.

As the technology of parenteral and enteral nutrition advances, a multidisciplinary nutritional support team (NST) can be used to assure quality care. NST functions include developing and using standard protocols and solutions, monitoring patients, and preventing complications. Interventions by NSTs can save money for the hospital as well. Examples from Truman Medical Center (Kansas City, Missouri) illustrate how the NST can perform these functions.

A multi-layered burden or a right fit? Physicians' perspectives on multidose drug dispensing in Denmark.

INTRODUCTION: Multidose drug dispensing (MDD) provides machine-packed, patient-specific dose pouches to improve medication safety and adherence, streamline medication management, and reduce pressure on healthcare systems. Despite the benefits, uptake in Denmark remains lower than in neighbouring countries, even after MDD prescriptions were integrated into the national electronic Shared Medication Record (SMR). This study explores general practitioners' (GPs) and hospital physicians' experiences with MDD following SMR integration. METHODS: A qualitative study was conducted through semi-structu

Standardizing default electronic health record tools to improve safety for hospitalized patients with Parkinson's disease.

Electronic Health Record (EHR) systems are often configured to address challenges and improve patient safety for persons with Parkinson's disease (PWP). For example, EHR systems can help identify Parkinson's disease (PD) patients across the hospital by flagging a patient's diagnosis in their chart, preventing errors in medication and dosing through the use of clinical decision support, and supplementing staff education through care plans that provide step-by-step road maps for disease-based care of a specific patient population. However, most EHR-based solutions are locally developed and, thus
