[Magnetic resonance imaging and the discoid meniscus in children].

Discoid meniscus is an uncommon dysplasia generally affecting the lateral meniscus. Its congenital or acquired origin remains a question of debate. We report 5 cases or lateral discoid meniscus in 4 girls and 1 boy, aged from 6 to 11 years. The main clinical signs were snapping knee and pain. The discoid meniscus appeared in MRI as an abnormally large meniscus extending into the joint space. It appeared as a central band giving increased signal in 3 out of 5 cases. The significance of this probably trauma-induced image was uncertain. Surgery confirmed MRI findings in 4 cases in which total res…

[Ulnodorsal impingement syndrome: meniscus lesions of the wrist].

In a consecutive series of 166 wrist arthroscopies, a lesion of the meniscus was found in 16 patients. All of them complained of ulnar wrist pain evoked by axial load in extension. 14 patients had previous wrist trauma. The preoperative physical examination showed marked ulnodorsal tenderness on palpation and the examiner could feel crepitation by passive motion in the radial-ulnar direction. The meniscus represents a part of the triangular fibrocartilage complex (TFCC). At this time it is not possible by magnetic resonance imaging (MRI) to differentiate between articular disc and meniscus les…

Failed meniscus transplantation. A report of three cases.

Treatment of patients with an established disabling compartment arthrosis after total meniscectomy is an important clinical problem. Meniscal transplantation may be an alternative method of treatment. Between 1989 and 1992, 25 cryopreserved nontissue antigen matched meniscal transplantations were performed. In three patients with a medial meniscus, the transplantation failed because of partial loosening of the meniscus. The donor meniscus was removed in one case after 12 months and in the other two cases after 20 and 24 months. In two cases, there was a varus malalignment of the knee. In the t…

[Value of MRI in the visualization of the meniscus in temporomandibular joints].

Modern imaging techniques, among which nuclear magnetic resonance currently ranks first, have made it much easier to understand the internal disorders of the temporomandibular joint. While computed tomography still is the ideal technique for all bony affections in the articular region, especially for injuries. MRI now is an essential first-intention complementary examination. It allows assessing the location of the meniscus and very well defines the pathology being explored: reducible or irreducible dislocation of the meniscus, either purely anterior or anterolateral, the latter being better v…

Meniscus repair in the anterior cruciate deficient knee.

From 1979 to 1986, isolated repair of a peripheral vascular zone meniscal tear was performed in 22 patients (23 menisci) who had ACL insufficiency. For various reasons none of these patients underwent repair or reconstruction of their ACL. The meniscus repair was done by open arthrotomy in 12 cases and by arthroscopic techniques in 11 cases. The purpose of this study was to evaluate the success rate of a meniscal repair in an anterior cruciate deficient knee. The average age of the patients at the time of surgery was 25 years and the average followup was 56 months. Six patients (26%) had mild …

The importance of the meniscus in unstable knees. A comparative study.

This study analyzes differences in knee function in ACL deficient knees with different meniscus status. Eighty patients, 12 women and 68 men, were matched in four groups according to sex, age, knee injuries, and follow-up time. Group I had two intact menisci; Group II, one meniscus successfully repaired; Group III, one meniscus removed; and Group IV, both menisci removed. The ACL was repaired acutely or reconstructed later in most cases and other significant ligament injuries were treated. All patients were reexamined clinically 6 to 8 years after injury. Knee function was recorded using the L…

Meniscus Tissue Engineering Scaffolds: Biomaterials, Biofabrication, and Translation.

The meniscus is a fibrocartilaginous tissue essential for load transmission, shock absorption, joint stability, and cartilage protection in the knee. However, its intrinsic healing capacity is severely limited, particularly in the avascular region and in complex defects, often resulting in persistent symptoms, functional impairment, and progressive joint degeneration. Although current clinical interventions, including meniscal repair, partial meniscectomy, allograft transplantation, and scaffold-assisted meniscal substitution, can provide symptomatic and functional improvement in selected pati…

Ten Pearls for Bucket Handle Medial Meniscus Repair.

Bucket-handle medial meniscus tears are complex injuries that represent approximately 10% of all meniscal lesions. Because of their vascular location, mainly being in the red-red zone, these injuries often have a favorable prognosis when properly treated. Surgical repair is the gold standard to preserve meniscal function and prevent osteoarthritis. This Technical Note outlines a 10-step technique for the effective evaluation and repair of medial bucket-handle medial meniscus tears using both inside-out and all-inside suturing techniques. Key surgical pearls include proper patient positioning, …

Arthroscopic all-inside repair of challenging meniscus tears.

Meniscus tears are prevalent in isolation and in combination with anterior cruciate ligament (ACL) injury. Meniscus lesions can be difficult to access and often display complex tear patterns, which result in technical challenges for the operating surgeon during surgical treatment. The aim of this video article is to demonstrate technical tips and tricks for performing all-inside repair of challenging meniscus tears. The presented techniques are indicated in young, physically active patients with symptomatic tears of the lateral and medial menisci, with or without concomitant ACL injury. The pr…

Aplanatic meniscus lens corrector for Ritchey-Chrétien telescopes.

An analytically designed aplanatic meniscus lens is proposed for increasing the field of view (FoV) of Ritchey-Chrétien (RC) telescopes. Compared to other field correctors, the proposed solution does not affect the initial RC telescope's aplanatic properties, and it can be added or removed from an existing RC telescope without changing the mirrors' shape. The final system features less astigmatism, which is balanced to flatten the image surface.

Apelin Alleviates Meniscus Endothelial Cell Apoptosis in Osteoarthritis.

Источник пометил публикацию как отозванную

Osteoarthritis (OA) is a degenerative disease characterized by articular cartilage and/or chondrocyte destruction, and although it has long been considered as a primary disease, the importance of meniscus endothelial cell modulation in the subchondral microenvironment has recently drawn attention. Previous studies have shown that apelin could potentially inhibit cellular apoptosis; however, it remains unclear whether apelin could play a protective role in protecting the endothelium in the OA meniscus. In this study, with the advantages of single-cell RNA sequencing (scRNA-seq) data, in combina…

Isolated meniscus extrusion associated with meniscotibial ligament abnormality.

PURPOSE: The purpose of this study was to describe meniscus extrusion, present imaging characteristics, and provide clinical correlations for patients with isolated meniscus extrusion. METHODS: Of the 3244 MRI reports identified as having meniscus extrusion, 20 patients were identified to have isolated meniscus extrusion (0.62%). Patients with moderate to severe chondromalacia, meniscus tears, intra-articular fractures, tumours, and ligament tears were excluded. Radiographs were reviewed and graded using Kellgren-Lawrence (K-L) scores. MRI's were reviewed for the extent of extrusion and wheth…

Injury patterns of medial meniscus posterior root tears.

INTRODUCTION: Medial meniscus posterior root tear (MMPRT) can occur in middle-aged patients who have a posteromedial painful popping during light activities. MMPRTs are more common in patients with increased age, female gender, sedentary lifestyle, obesity, and varus knee alignment. However, injury mechanisms of minor traumatic MMPRTs are still unclear. We hypothesized that high flexion activities are the major cause of MMPRTs. The aim of this study was to clarify injury patterns of MMPRTs. MATERIALS AND METHODS: One hundred patients were diagnosed having MMPRTs after posteromedial painful po…

Medial meniscus grafting restores normal tibiofemoral contact pressures.

BACKGROUND: Tissue excision in the setting of a meniscal tear has been shown to dramatically increase peak contact stresses in the affected tibiofemoral joint compartment, leading to the development of degenerative changes and osteoarthritis. PURPOSE/HYPOTHESIS: The current in vitro study utilized a porcine model to evaluate the effectiveness of segmental medial meniscal grafting following partial meniscectomy. The study hypothesis was that the procedure would normalize medial tibofemoral joint compartment pressure magnitudes, areas, and locations relative to an intact meniscus. STUDY DESIGN…

Cell distribution and regenerative activity following meniscus replacement.

PURPOSE: Meniscus replacement is of clinical benefit, but universal efficacy remains elusive. A greater understanding of the biological activity within implanted allografts or synthetic scaffolds may assist the development of improved surgical strategies. MATERIALS: Biopsies of fresh-frozen allograft (n=20), viable allograft (n=18) and polyurethane scaffolds (n=20) were obtained at second-look arthroscopy. Histological evaluation of tissue morphology and cell density/distribution was performed using haematoxylin-eosin (H&E) staining. Immunohistochemistry was used to detect the presence of CD3…

PLDLA/PCL-T Scaffold for Meniscus Tissue Engineering.

The inability of the avascular region of the meniscus to regenerate has led to the use of tissue engineering to treat meniscal injuries. The aim of this study was to evaluate the ability of fibrochondrocytes preseeded on PLDLA/PCL-T [poly(L-co-D,L-lactic acid)/poly(caprolactone-triol)] scaffolds to stimulate regeneration of the whole meniscus. Porous PLDLA/PCL-T (90/10) scaffolds were obtained by solvent casting and particulate leaching. Compressive modulus of 9.5±1.0 MPa and maximum stress of 4.7±0.9 MPa were evaluated. Fibrochondrocytes from rabbit menisci were isolated, seeded directly on t…

Discoid lateral meniscus: prevalence of peripheral rim instability.

The purpose of this study was to determine the prevalence of peripheral rim instability in discoid lateral meniscus. A consecutive series of 112 patients (128 knees) (mean age 10.0 years [range 1 month to 22 years]) who underwent arthroscopic evaluation and treatment of a discoid lateral meniscus between 1993 and 2001 was reviewed. Of those discoid menisci classified intraoperatively (n = 87), 62.1% (n = 54) were complete discoid lateral menisci and 37.9% (n = 33) were incomplete discoid lateral menisci. An associated meniscal tear was present in 69.5% (n = 89) of all knees studied. Overall, 2…

Postoperative follow-up and rehabilitation after meniscus replacement.

Even though basic scientific knowledge about the meniscal loading pattern may advocate restrictive rehabilitation after meniscus repair, experience and one controlled study favour accelerated rehabilitation. The current protocols for rehabilitation after meniscal substitution follow personal experience with parameters such as pain, effusion, locking, and gait pattern used as clinical guidance. Controlled clinical studies on rehabilitation should be encouraged. An example of a rehabilitation protocol is given. As meniscus replacement is a new treatment option, it is essential to document detail…
