PURPOSE: To describe the frequency of anterior uveitis and ocular hypotony in cidofovir-treated patients with acquired immune deficiency syn … re at significant risk of ocular adverse effects. Prompt treatment with topical corticosteroids and mydriatics may control uveitis and in some cases cidofovir treatment may be cautiously continued. In the event of ocular hypotony cidofovir should be discontinued in favour of an alternative anti-cytomegaloviral agent.

OBJECTIVE: To evaluate a series of case that developed iris changes after performing Descemet's stripping automated endothelial keratoplasty (DSAEK).
METHODS: Retrospective study of eyes that developed iris abnormalities, such as pupil ovalisation, iris atrophy, iridocorneal synechiae, mydriatic pupil, and pigmentary changes after performing DSAEK in a tertiary hospital.

Approximately 3% of individuals in road traffic accidents suffer ocular injuries. We present a case of a man in his late 80s who presented with bilateral corneal decompensation following airbag deployment during a road traffic accident. Brainstem strokes affecting the periaqueductal gray matter of the midbrain can cause vertical ophthalmoplegia. Accompanying clinical features are frequently associated and reflect the involvement of other brainstem structures. We report on an adolescent presenting with vertical gaze palsy and left mydriatic pupil as the only clinical expression of a small infarct located in the left periaqueductal gray matter. Even when the lesion was strictly unilateral, vertical ophthalmoplegia affected both eyes.

METHODS: This was a prospective randomized analysis of 90 eyes submitted to senile cataract surgery. Group A (30 eyes) included topical mydriasis, group B (30 eyes) included mydriasis with the ophthalmic insert (Mydriasert), and group C (30 eyes) included intracameral mydriasis. The asplenia has little influence on immunocompetence, and the patients have survived 300 patient years without critical infections. The gene defect has not yet been unravelled.

Two young adults, aged 24 and 31, had a long history of small, poorly reactive pupils. There was no history of large pupils, and a review of old photographs confirmed 10 and 5 years, respectively, of miosis. Both were found to have bilateral tonic pupils that were supersensitive to diluted pilocarpine. Although it is possible that they had an unusually early onset of bilateral Adie's syndrome with dilated pupils that was not noticed, it is suggested that some patients might have primary miotic Adie's pupils without ever passing through a mydriatic phase.

Although rare, ocular reticulum cell sarcoma presents a recognisable clinical pattern, as confirmed by 3 new cases. Typical patients, in their sixth and seventh decades, initially complain of gradual visual loss. Examination reveals 'uveitis' with prominent vitreous debris and/or chorioretinal infiltrative lesions. Topical steroid and mydriatic therapy is ineffective. Reticulum cell sarcoma in the central nervous or other systems may precede or accompany the ophthalmic presentation. In an increasing number of cases tissue diagnosis and effective therapy have followed vitreous aspiration.
