Revascularization in Acute Myocardial Infarction and Cardiogenic Shock in the Era of Mechanical Circulatory Support.

This review summarizes contemporary evidence and evolving revascularization strategies for acute myocardial infarction-cardiogenic shock (AMI-CS). Despite prompt percutaneous coronary intervention (PCI) as the standard of care, AMI-CS continues to carry approximately 40% to 50% 30-day mortality. We discuss the ongoing debate of culprit-only versus multivessel PCI in shock (informed by the CULPRIT-SHOCK trial and registry data) and examine the role of mechanical circulatory support devices (eg, Impella and ECMO) in improving outcomes. New trial data (eg, DanGer Shock) and recent guideline chang

The Role of Amiodarone in the Prevention of Postoperative Atrial Fibrillation After Surgical Myocardial Revascularization.

BACKGROUND: Postoperative atrial fibrillation (POAF) is the most common complication after cardiac surgery, increasing morbidity, ICU stay, dialysis need, hospitalization length, and costs. Despite prophylactic strategies, POAF incidence remains high, especially in moderate and high risk patients. OBJECTIVE: To assess the effect of amiodarone on POAF incidence and onset timing in moderate and high risk patients. METHODS: This prospective observational study included 454 patients undergoing elective coronary artery bypass grafting (CABG). Risk stratification of patients was based on the McSPI

STEMI, Revascularization, and Peak Troponin by Adverse Pregnancy Outcomes in Women With Myocardial Infarction.

BACKGROUND: Women with a history of adverse pregnancy outcomes have a higher risk of coronary heart disease. Emerging evidence suggests that women with a history of preeclampsia have a different pattern of overall coronary atherosclerosis and that they at the time of myocardial infarction (MI) more frequently present with ST-segment elevation MI (STEMI) compared to women with no such history. OBJECTIVES: The purpose of this study was to determine whether among women with MI, those with a history of adverse pregnancy outcomes are more likely to present with STEMI or other clinical characterist

Diabetes, heart failure, and myocardial revascularization: Is there a new message from the ISCHEMIA trial?

There is no evidence that the indications for myocardial revascularization differ between patients with and without diabetes. Accepted indications include stable angina that cannot be adequately managed by medication, acute coronary syndromes, severely reduced left ventricular (LV) function due to coronary artery disease, left main stenosis, and advanced coronary artery disease causing substantial inducible ischemia. The recent ISCHEMIA trial challenged the criterion of ischemia. With respect to its primary endpoint, ISCHEMIA showed no benefit of an invasive strategy with systematic myocardial…

Effectiveness of home-rehabilitation in patients after an acute coronary syndrome and myocardial revascularization.

UNLABELLED: Physical activity is a mainstay (class IA) of rehabilitation programme after an acute coronary syndrome, but less than 40% of patients is physically active at one year. Home-rehabilitation, initially designed to manage the increasing number of patients in rehabilitation programmes, could result in a better strategy to increase adherence and persistence to physical activity. OBJECTIVES: To test such hypothesis, At Cardiac Rehabilitation Centre (Institute of Physical Medicine and Rehabilitation, Udine, Italy), physical activity adherence was compared between patients treated with a

[Surgical myocardial revascularization in a female patient with bilateral occlusion of the common carotid arteries].

Concurrent stenosis of the coronary and carotid circulation is one of the most serious disease. The authors describe the case of a 74-year old female patient with ischaemic heart disease with concurrent bilateral occlusion of the common carotid arteries. Surgical revascularization of the myocardium was made using extracorporeal circulation under general normothermia with protection of the heart muscle from ischaemia by the method of warm blood cardioplegia. In the discussion the authors analyze contemporary possibilities of surgical treatment of ischaemic disease (MIDCAB, OPCAB, operations wit

[An unusual indication of deep hypothermia with total circulatory arrest for myocardial revascularization surgery].

The authors relate a case of 69 years old patient with unstable angina and left main coronary artery disease, who underwent aggressive mediastinal radiotherapy for lymphoproliferative syndrome 20 years ago and who needed urgent myocardial revascularization. Due to great calcification and heavily heart adherences it was performed 20 degrees C deep hypothermic circulatory arrest to avoid aortic clamping and to make the anastomosis with better exposition of the aorta and coronary arteries.

Concomitant vascular procedures in conjunction with myocardial revascularization: all or none? A report of a case.

Patients with coronary artery atherosclerosis usually have concomitant peripheral vascular lesions. The authors describe the case of a 65-year-old woman who had multiple symptomatic lesions: severe stenosis of the left main coronary artery and the carotid arteries, a large abdominal aortic aneurysm and bilateral renal artery occlusion. To manage these and to avoid myocardial infarction and cerebrovascular accident at operation, concomitant procedures were performed as follows: coronary artery bypass grafting, aneurysm resection, carotid endarterectomy and revascularization of the larger kidney

Complete Revascularization in Older Patients With Myocardial Infarction With or Without Complex Nonculprit Lesions.

BACKGROUND: The FIRE trial (Functional Assessment in Elderly Myocardial Infarction Patients With Multivessel Disease) showed the superiority of complete revascularization in older patients with myocardial infarction (MI) and multivessel disease. Whether this result applies equally to patients at higher risk of ischemic events due to nonculprit lesion complexity is unclear. METHODS: Overall, 1445 patients were randomized to culprit-only or complete revascularization. In this prespecified analysis, patients were divided into those with or without at least 1 complex nonculprit lesion. A nonculpr

Comparison of Outcomes of Coronary Revascularization for Acute Myocardial Infarction in Men Versus Women.

This study sought to examine the differences in the characteristics and outcomes between men and women who underwent percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) for acute myocardial infarction (AMI) in contemporary US practice. The Nationwide Inpatient Sample was used to identify patients who underwent revascularization for AMI between January 1, 2003 and December 31, 2016. The primary outcome was in-hospital mortality. Propensity score matching was utilized to account for differences in baseline characteristics. In total, 3,603,142 patients were included

Degree of risk related to procedures performed in conjunction with surgical myocardial revascularization in octogenarians.

INTRODUCTION/OBJECTIVES: We determined the degree of risk produced by the association of other surgical procedures with surgical myocardial revascularization in octogenarian patients and identified the risk factors that best explain hospital mortality. METHODS: This study was an observational analytical historical cohort study involving octogenarians operated on at our institution between January 1, 2000 and January 1, 2005. We stratified the objective population as follows: Group 1 comprised octogenarians revascularized without associated procedures, and Group 2 comprised octogenarians revas…

The timing of infarction pain in patients with acute myocardial infarction after previous revascularization.

Circadian variation of onset of acute myocardial infarction (AMI) has been noted in many studies, but there are no data about subgroups of patients with previous coronary artery bypass grafting (CABG). Because of abnormalities in the circadian rhythm of autonomic tone after surgery, it was very interesting to analyze the circadian patterns in the onset of symptoms of AMI in various subgroups of 1784 patients with previous CABG. As in the other studies, a peak occurred in the morning hours with 26.3% of the patients, but there was a second nearly equal, but higher, peak (26.4%) in the evening h

[Atresia or congenital stenosis of the left coronary ostium. Myocardial revascularization in 5 children].

Five patients, 3 months to 13 year old with atresia (4) or stenosis (1) of the left coronary artery underwent myocardial revascularisation at Marie Lannelongue Hospital. The preoperative symptoms were dominated in one case by cardiac failure due to myocardial infarction and in the other cases by effort angina with syncope. The operative technique of revascularisation in one case was angioplasty with enlargement of the left main coronary artery with a patch and, in the others, bypass graft of the left coronary artery with the left internal mammary artery. All 5 patients survived surgery and are
