Pathogenesis of myositis in children.

PURPOSE OF REVIEW: There is increasing evidence for involvement of innate immune mechanisms in the pathogenesis of idiopathic inflammatory myopathies. This review focuses on recent advances in understanding these mechanisms in juvenile dermatomyositis, the most common form of childhood inflammatory myopathy. RECENT FINDINGS: Type I interferon activity in juvenile dermatomyositis has been demonstrated by both global gene expression profiling and immunohistochemical analysis of affected tissues. Most recently, expression of interferon-inducible genes in peripheral blood cells has shown promise …

The heterogeneity of juvenile myositis.

Juvenile myositis is a heterogeneous group of systemic autoimmune diseases, in which clinical and serologic subgroups result in subsets of patients with distinct clinical manifestations, disease courses, immunogenetic associations, responses to therapy, and prognoses. A newly identified autoantibody of unknown specificity, anti-p155, is myositis-associated and seen in up to 20-30% of juvenile and adult DM patients. HLA DRB1*0301 and its linked allele DQA1*0501 have been identified as the major immunogenetic risk factor for juvenile and adult DM in both European- and African-American patients, …

Microbiology and management of myositis.

This review summarizes the microbiology, management, and prevention of myositis. Muscular infections frequently occur in areas of the body that have been compromised or injured by a foreign body, trauma, ischemia, injection of illicit drug, malignancy, or surgery. These infections can develop very rapidly to life-threatening systemic illness. The predominant pathogens are Staphylococcus aureus, Group A streptococci (GAS), gram-negative aerobic and facultative bacilli, and the indigenous aerobic and anaerobic cutaneous and mucous membranes local microflora. Pyogenic myositis can be classified i…

The association of malignancy with myositis.

The exact nature of the relation between malignancy and myositis has created continued controversy. Most of the evidence points toward an association between dermatomyositis (DM) with malignancy. The relation with polymyositis is much weaker, if at all present. The first 5 years after DM diagnosis is when the risk is greatest for malignancy. Although types of malignancy seen in DM and polymyositis are similar to those in the general population, ovarian cancer seems to be more common in DM. Most experts recommend that otherwise asymptomatic patients with DM have an age-specific examination for …

Recognition and management of myositis.

Polymyositis and dermatomyositis are severe inflammatory muscle disorders of unknown cause, with possible life-threatening complications. Prognosis and response to therapy may be predicted not only from the clinical and pathological diagnostic group to which a patient belongs, but also from the patient's myositis-specific antibody status, extraskeletal muscle involvement, and the interval between onset of muscle weakness and the start of the treatment. Corticosteroids are the mainstay of treatment, providing recovery of normal muscular function in about 60% of patients. However, adequate dose,…

Scleromyxedema and severe myositis.

A case of scleromyxedema with associated rhabdomyolysis is reported. This appears to be a well-documented, although rare complication of scleromyxedema. This case is unique in that mucin was demonstrated in the muscle as well as the skin, and spontaneous resolution of the rhabdomyolysis occurred with the implementation of IV fluids and bed rest.

Occupational and Hobby Exposures Associated With Myositis Phenotypes in a National Myositis Patient Registry.

OBJECTIVE: The objective of this study was to investigate occupational and hobby exposures to silica, solvents, and heavy metals and the odds of having the idiopathic inflammatory myopathy (IIM) phenotypes dermatomyositis (DM) and polymyositis (PM) versus inclusion body myositis (IBM), lung disease plus fever or arthritis (LD+), and systemic autoimmune rheumatic disease-associated overlap myositis (OM). METHODS: The sample included 1,390 patients (598 with DM, 409 with PM, and 383 with IBM) aged ≥18 years from a national registry. Of these, 218 (16%) were identified with LD+, and 166 (12%) wi…

Myositis-Associated Autoantibodies in Patients With Juvenile Myositis Are Associated With Refractory Disease and Mortality.

OBJECTIVE: Myositis-associated autoantibodies (MAAs) have been associated with overlap myositis, certain disease manifestations such as interstitial lung disease (ILD), and worse prognosis in the idiopathic inflammatory myopathies. MAAs overall remain largely uncharacterized in patients with juvenile-onset myositis. Moreover, it is unknown whether the number of MAAs is associated with disease severity. METHODS: Patients with juvenile myositis in cross-sectional natural history studies who underwent testing for myositis autoantibodies were included. Demographics, myositis autoantibodies, clini…

[New insights of myositis-specific and -associated autoantibodies in juvenile and adult type myositis].

Myositis, which means inflammation of the muscles, is a general term used for inflammatory myopathies. Myositis is a rare idiopathic autoimmune disease. It is believed that environmental factors such as virus, bacteria, parasites, direct injuries, drugs side effect can trigger the immune system of genetically susceptible individuals to act against muscle tissues. There are several types of myositis with the same systemic symptoms such as muscle weakness, fatigue, muscle pain and inflammation. These include dermatomyositis, juvenile dermatomyositis, inclusion-body myositis, polymyositis, orbita…

[Sporadic Inclusion Body Myositis].

Sporadic inclusion body myositis (sIBM) is an intractable and progressive skeletal muscle disease of unknown etiology. Muscle biopsy typically reveals endomysial inflammation, invasion of mononuclear cells into non-necrotic fibers, and rimmed vacuoles, suggesting that inflammation and degeneration co-exist in the pathomechanism. According to a nationwide survey conducted by a research team of the Ministry of Health, Labor, and Welfare, the number of patients is increasing in Japan as well. The clinical progression shows a slow and chronic deterioration. sIBM is usually diagnosed five years aft…

Yersinia pseudotuberculosis-Associated Myositis.

Yersinia pseudotuberculosis (Y. pstb.) is a Gram-negative pathogen associated with gastrointestinal infections, such as enterocolitis. While complications like reactive arthritis can occur, progression to myositis is extremely rare. This report describes a five-year-old boy who developed myositis following a Y. pstb. infection. The patient presented with a five-day history of abdominal pain, fever, and leg pain that progressed to muscle weakness, which affected his ability to walk. Laboratory results showed elevated inflammatory markers, and stool cultures confirmed Y. pstb. infection. Magneti…

[Antisynthetase Syndrome-Associated Myositis].

Antisynthetase syndrome-associated myositis is a major form of autoimmune myositis defined by the presence of anti-aminoacyl tRNA synthetase autoantibodies. It involves the skeletal muscle as well as the lungs, joints, and skin. Severity of each symptom varies by autoantibody subtype; anti-OJ is associated with severe muscle involvement. Pathological changes from the perimysium to the adjacent perifascicular area, including perifascicular necrosis, is a distinctive feature. The skeletal muscle provides an immunological micro-milieu for specific plasma cells. Therapies against plasma cells or f…

[Sporadic Inclusion Body Myositis].

Inclusion body myositis (IBM) is an idiopathic inflammatory muscle disease that predominantly affects elderly men over the age of 50 years, and the number of patients is rapidly increasing in Japan. Generally, muscle weakness and atrophy occur asymmetrically in the flexor muscles of the fingers and wrists and the quadriceps muscles. Invasive muscle biopsy is essential for the diagnosis of IBM. Although its pathogenesis is not yet understood, both inflammatory as well as degenerative mechanisms are postulated to be involved. In particular, degeneration of the IBM muscle may be associated with t…

[Inflammatory muscle diseases (myositis)].

CLINICAL ISSUE: Inflammatory muscle diseases are quite heterogeneous and frequently a specific diagnosis is not easy. STANDARD RADIOLOGICAL METHODS: Magnetic resonance imaging (MRI) is best suited to demonstrate pattern and size of inflamed muscles. PERFORMANCE: Sensitivity of MRI in showing inflamed muscles is nearly 100%. Specificity, however, is much lower. PRACTICAL RECOMMENDATIONS: In cases of suspected bacterial myositis, sonography may be used to exclude abscess formation prior to MRI for more precise information about inflamed muscles.

Recurrent Bilateral Focal Myositis.

This report describes a rare case of recurrent bilateral focal myositis and its successful treatment via methotrexate. A 38-year-old man presented myalgia of the right gastrocnemius in May 2005. Magnetic resonance imaging showed very high signal intensity in the right gastrocnemius on short-tau inversion recovery images. A muscle biopsy revealed inflammatory CD4+ cell-dominant myogenic change. Focal myositis was diagnosed. The first steroid treatment was effective. Tapering of prednisolone, however, repeatedly induced myositis relapse, which progressed to multiple muscle lesions of both lower …
