[Naltrexone in the therapeutic relation in cases of opioid abuse and dependence].

The paper describes the use of naltrexone, an opium-receptor antagonist, in the multimodal treatment of drug addiction and in particular in preventing the recidivation of heroin abuse. Naltrexone appears to lack the ambiguous characteristics of methadone which, in spite of not provoking all the effects of heroin, may be seen by the drug addict as a reasonable substitute, in particular if it is difficult to find. Since naltrexone is clearly a member of the drug category, and does not overlap into the pleasure category, it is possible to break the perverse triangle which is often created between…

A controlled trial of propoxyphene and naltrexone in patients with Tourette's syndrome.

To investigate the effect of drugs acting on the endogenous opioid system, we studied 10 adults with Tourette's syndrome who received propoxyphene hydrochloride (260 mg/day), naltrexone hydrochloride (50 mg/day), and placebo in a double-blinded, randomized clinical trial. Using a self-report scale (Tourette's Syndrome Symptom List), subjects noted a significant (p less than 0.04) lessening of tics after treatment with naltrexone when compared with placebo. An improvement in performance on the Trail Making B test, a measure of attention and visuomotor sequencing and planning, occurred after rec…

Differential effects of CGS 8216 and naltrexone on ingestional behaviour.

Effects of the pyrazoloquinoline CGS 8216 (a partial benzodiazepine receptor inverse agonist) and the opiate antagonist, naltrexone, were compared in several tests of ingestion in non-deprived and deprived male rats. Both naltrexone (0.1-10.0 mg/kg, SC) and CGS 8216 (1.25-10.0 mg/kg, IP) significantly reduced the consumption of a highly palatable saccharin-glucose solution by non-deprived rats. Both compounds were also effective in reducing, dose-dependently, the intake of palatable sweet or oily mash by non-deprived animals. Hence, naltrexone and CGS 8216 attenuated palatability-induced inges…

The role of the National Institute on Drug Abuse in the development of naltrexone.

Naltrexone was administered to 114 opiate-addicted business executives and 15 opiate-addicted physicians within a highly structured abstinence-oriented aftercare program following an inpatient detoxification program that used clonidine. The majority of patients successfully completed 6 months of naltrexone treatment without missed visits or drug-positive urines, and were still opiate-free at 12- to 18-months follow-up. The most successful outcomes were achieved by those who had entered treatment with unequivocal job jeopardy. Our findings suggest that naltrexone can be an extremely useful adju…

The sequential use of clonidine and naltrexone in the treatment of opiate addicts.

The efficacy of clonidine in the management of opiate withdrawal states has improved and refined the medical approach to this condition. In addition, the use of clonidine for opiate detoxification paves the way for naltrexone maintenance. Naltrexone, by providing chronic opiate receptor blockade, prevents opiate intoxication and subsequent readdiction in recovered addicts. The sequential use of clonidine and naltrexone, in conjunction with drug rehabilitation, appears to represent a viable and effective treatment for opiate addiction in motivated patients. The development of clonidine and nalt…

Comparative efficacy of naltrexone-accelerated versus sublingual buprenorphine in opioid withdrawal management and initiating injectable naltrexone treatment: study protocol for a randomized controlled trial.

BACKGROUND: Extended-release injectable naltrexone (XR-NTX), a long-acting opioid antagonist, is an effective relapse prevention strategy in the treatment of opioid use disorder (OUD). However, standard XR-NTX initiation requires completion of opioid withdrawal management and a 1-2 week opioid-free period. This interval is associated with heightened vulnerability to lapse and relapse, and many patients fail to initiate XR-NTX treatment successfully. Therefore, alternative strategies that enable rapid XR-NTX initiation are needed. This study aimed to compare the effectiveness and safety of nalt…

Buprenorphine-Naloxone vs Extended-Release Naltrexone Following Opioid Withdrawal Treatment.

IMPORTANCE: Buprenorphine-naloxone and extended-release (XR) naltrexone are both efficacious medications for opioid use disorder that reduce unregulated opioid use and improve opioid cravings and treatment retention. While prior comparative effectiveness studies demonstrated superiority of buprenorphine-naloxone over XR naltrexone for relapse and treatment retention, the comparative effectiveness of both treatments for all-cause mortality and nonfatal opioid overdose outcomes has not been rigorously tested. OBJECTIVE: To compare the effectiveness of buprenorphine-naloxone vs XR naltrexone aft…

Retrospective Analysis of Naltrexone for Persistent Neuropathic Pain Case Series.

BACKGROUND: Persistent neuropathic pain (NP) is a prevalent and debilitating problem, often resistant to first-line treatment. Low-dose naltrexone (LDN) has shown promise in treating fibromyalgia and other forms of chronic pain. CASE REPORT: Retrospective analysis was performed on 14 of 18 patients with diverse forms of NP who met inclusion criteria by using LDN (1.5-4.5 mg) as a second-line medical pain therapy. Pain Numeric Rating Scale (NRS-11) in all patients decreased from 6.29 ± 2.20 to 3.71 ± 2.89 (P = 0.001) from initiation to final visit. Median treatment length was 252 days with lon…

Naltrexone alters cardiovascular function following acute forced swimming in mice.

PURPOSE: Naltrexone (NTX) is an opioid antagonist that can reverse the physiological effects of opioid receptors when bound. Opioid receptors have been found to play a role in cardiovascular (CV) function, and thus, binding of NTX may alter CV activity at rest and in response to acute and chronic exercise (EX). We hypothesized that opioid receptor blockade will alter the typical CV responses following acute EX. METHODS: We assessed the effects of opioid receptor blockade on CV function via echocardiography in mice following an acute bout of forced swimming (FSw), a model of rodent EX. We admi…

Immunometabolic Modulatory Role of Naltrexone in BV-2 Microglia Cells.

UNLABELLED: Background: Naltrexone is an opioid receptor antagonist commonly used to treat opioid and alcohol dependence. The use of low dose naltrexone (LDN) was found to have anti-inflammatory properties for treatment of diseases such as fibromyalgia, Crohn's disease, multiple sclerosis and regional pain syndromes. Related to its anti-neuroinflammatory properties, the mechanism of action is possibly mediated via Toll-like receptor 4 antagonism, which is widely expressed on microglial cells. The aim of the present study was to assess the immunometabolic effects of naltrexone on microglia cell…

Low-Dose Naltrexone for Chronic Pain: Update and Systemic Review.

PURPOSE OF REVIEW: The purpose of this review is to evaluate and explain our current understanding of the clinical use of low-dose naltrexone in the treatment of chronic pain. RECENT FINDINGS: Recent pre-clinical uses and clinical studies further elucidate the use of low-dose naltrexone in the treatment of chronic pain. Low-dose naltrexone (LDN) has shown promise to reduce symptoms related to chronic pain conditions such as fibromyalgia, inflammatory bowel conditions, and multiple sclerosis. The mechanism of LDN appears to be modulation of neuro-inflammation, specifically, the modulation of t…

Low-dose naltrexone: a novel treatment for Hailey-Hailey disease.

Hailey-Hailey disease (chronic benign familial pemphigus) is a rare inherited dermatosis typically characterized by erosions at intertriginous sites preceded by minor trauma or stress. We report a case of treatment-resistant Hailey-Hailey disease having failed topical and oral steroids, prophylactic aciclovir and doxycycline, and systemic therapies including dapsone, acitretin and ciclosporin. Low-dose naltrexone 4·5 mg once daily was commenced following an incidental benefit in this patient's similarly affected sister. The clinical and psychological response to date has been considerable.

Baseline characteristics of patients predicting suitability for rapid naltrexone induction.

BACKGROUND AND OBJECTIVES: Extended-release (XR) injection naltrexone has proved promising in the treatment of opioid dependence. Induction onto naltrexone is often accomplished with a procedure known as rapid naltrexone induction. The purpose of this study was to evaluate pre-treatment patient characteristics as predictors of successful completion of a rapid naltrexone induction procedure prior to XR naltrexone treatment. METHODS: A chart review of 150 consecutive research participants (N = 84 completers and N = 66 non-completers) undergoing a rapid naltrexone induction with the buprenorphon…

Naltrexone + bupropion (Mysimba). Too risky for only modest weight loss.

Weight loss and its long-term maintenance are mainly based on dietary measures and regular physical activity. There are currently no weight-loss medications with a favourable harm-benefit balance. Bupropion is chemically related to certain amphetamines, while naltrexone is an opioid receptor antagonist. A fixed-dose combination of these two drugs has received marketing authorisation in the European Union for obese patients and for over-weight patients with other cardiovascular risk factors. In five placebo-controlled, randomised, double-blind trials, the patients, weighing on average between 1…

Precipitated withdrawal during maintenance opioid blockade with extended release naltrexone.

Background There has been increasing interest in the use of extended release injectable naltrexone for the treatment of opioid dependence. Case description We report a case of precipitated withdrawal in a 17-year-old adolescent female receiving extended release naltrexone (Vivitrol) for opioid dependence, following her third serial monthly dose of the medication, several days after using oxycodone with mild intoxication. Conclusions This case suggests that, in some circumstances, the opioid blockade may be overcome when naltrexone levels drop towards the end of the dosing interval, producing v…
