[Recent advances in prevention of sepsis in the preterm neonate].

An episode of sepsis occurs in 20 to 40% of all preterm patients, and such figures have been reported constantly increasing in Neonatal Intensive Care Units. Neonatal sepsis include bloodstream, urine, cerebrospinal, peritoneal infections, infections starting from burns and wounds, or from any other usually sterile sites. Many specific risk factors account for the increased risk of sepsis, including employment of broad-spectrum antibiotics selecting resistant microflora, parenteral nutrition, acid inhibitors and steroids, as well as the systematic and long-lasting use of invasive management. I…

Experience with teicoplanin in the treatment of neonatal staphylococcal sepsis.

We aimed to evaluate retrospectively the clinical and bacteriological efficacy and potential side-effects of teicoplanin treatment in neonates with proven staphylococcal infection. There were 37 episodes of staphylococcal septicaemia in neonates with a mean gestational age of 34.2 +/- 2.3 weeks; 26 were caused by coagulase-negative staphylococcal (CoNS) sepsis and 11 by Staphylococcus aureus sepsis. All episodes were treated with teicoplanin (intravenous loading dose 16 mg/kg followed by a maintenance dose of 8 mg/kg daily). The methicillin resistance and antibiotic susceptibilities of both mi

Ignác Semmelweis and the etiology of fetal and neonatal sepsis.

It is well-known that Ignác Semmelweis discovered the etiology and prophylaxis of puerperal sepsis. However, few historians have focused on his understanding of the pathophysiology of fetal and neonatal sepsis. Based on several key observations, Semmelweis realized that puerperal fever (also known as "childbed fever") could be transmitted to the fetus, especially when the first stage of labor was prolonged and multiple examiners performed vaginal examinations while their fingers were contaminated. This insight was particularly valuable in that it helped him decipher the mystery of puerperal se…

[Prospective study of Candida-related sepsis in the neonate].

OBJECTIVE: Our objective was to carry-out a prospective study of newborns with systemic candidiasis admitted to our Neonatology Unit in a teritiary hospital during the period of March 1994-September 1997. PATIENTS AND METHODS: To be included in the study the patient had to have Candida sp recovered from a normally sterile body fluid and clinical signs of sepsis. We analyzed perinatal and neonatal antecedents, risk factors, clinical course, diagnosis, treatment and outcome. RESULTS: The incidence of systemic candidiasis was 0.62% (14 newborns). Two were term infants and 12 preterm infants, 9

Early-onset Hemophilus influenzae sepsis in the neonate.

Hemophilus influenzae sepsis has been increasingly reported in recent years. Thirteen neonates have been treated in our institution in the past 8 years, 12 of whom were admitted within the last 4 years. H influenzae is responsible for 8% of neonatal sepsis occurring during the first 3 days of life. It is usually transmitted before or at the time of birth and is more frequently encountered in premature and low birthweight infants. It is strongly associated with maternal obstetric complications particularly genitourinary tract infections and prolonged rupture of membranes before delivery. Most c…

Surgery, sepsis, and nonspecific immune function in neonates.

An audit of 143 neonatal surgical operations identified low birth weight infants as being at particular risk of developing post-operative sepsis following major surgical trauma. Activity of the nonspecific immune system was examined pre- and post-operatively in order to elucidate the susceptibility to post-operative sepsis of these infants. A chemiluminescence micro-method was developed specifically for this purpose. Neonatal plasma was shown to have significantly impaired opsonising activity, and activity was further reduced in low birth weight infants. Levels of immunoglobulin and complement

Tachycardia as a sign of early onset neonatal sepsis.

A prospective study in two parts was designed to assess tachycardia in neonates (less than 72 hours of age) and its role as an early indicator of neonatal sepsis. In Part I of the study we determined the incidence of tachycardia among all inborn neonates in 1 year, and in Part II we assessed the presence of tachycardia in inborn and outborn infants evaluated for sepsis. Twenty-one infants were tachycardic among 4530 live births (incidence, 4.64/1000). Ten tachycardic infants were septic. Of 82 infants who were evaluated for sepsis, 13 had proved sepsis, 12 of whom had tachycardia as one of the…

Selection of antimicrobial agents for treatment of neonatal sepsis.

The incidence of sepsis among neonates born in the United States varies from less than one to more than eight per 1,000 live births. Bacterial meningitis occurs in about one-third of infants with sepsis and is more frequent during the first month of life than during any subsequent period. The clinical diagnosis of sepsis in newborn infants is difficult since signs are subtle and nonspecific. Because of the difficulty of diagnosis, many infants receive treatment although few significant bacterial infections are subsequently documents; approximately 5%-10% of neonates born in the United States r

Group G streptococcal colonization and sepsis in neonates.

Group G streptococci were isolated from the blood of seven neonates over a five-year period at the New York Hospital. All but two were born near term. All presented clinically with signs and symptoms of neonatal sepsis. Only one case was associated with a major complication--epidermolysis bullosa; this patient died. Two presented as late-onset sepsis at 11 and 12 days of age. Six of the seven responded promptly to antibiotic therapy. A one-year survey was carried out of colonization with groups B and G streptococci at the time of discharge (day 3). Colonization with group G streptococci varied…

This study suggests that alternative approaches to the treatment of neonatal sepsis should be explored; administration of an antibiotic which crosses the blood-cerebrospinal fluid barrier more readily should be considered.

Neonatal sepsis in the neonatal intensive care unit: characteristics of early versus late onset.

Neonatal sepsis is a major cause of death in newborns despite sophisticated neonatal intensive care. This retrospective study reviewed the clinical characteristics of cases of culture-proven sepsis in a neonatal intensive care unit from January 1992 to December 2001. Patients were divided into those with onset of sepsis in the first 7 days of life (early-onset group) and those with onset after the seventh day of life (late-onset group). A total of 270 cases with 325 episodes of sepsis and 353 isolated pathogens were identified and included in the study. The male-to-female ratio was 1.4. The ma…

Development of a Novel Assessment Tool and Code Sepsis Checklist for Neonatal Late-Onset Sepsis.

BACKGROUND: Accurate diagnosis and timely management of neonatal late-onset sepsis (nLOS) have been less well-studied than those of early-onset sepsis. We noticed a delay in nLOS detection and management in our neonatal intensive care unit. PURPOSE: To develop an assessment tool to aid in the recognition and reporting of nLOS and to standardize the management process once sepsis is recognized. METHODS: The Plan-Do-Study-Act (PDSA) improvement model provided the framework for interventions for our antibiotic stewardship program, including the aims of this project. A literature review was perf

Macrococcus caseolyticus in early-onset neonatal sepsis, Kassala, Sudan.

INTRODUCTION: Neonatal sepsis, a clinical syndrome characterized by systemic signs of infection in newborn infants (< 28 days old), are a significant cause of neonatal mortality and long-term morbidity globally, particularly in low- and middle- income countries. CASE PRESENTATION: We report the first case of neonatal sepsis caused by Macrococcus caseolyticus in a 48-hours old newborn who attended to the intensive care unit (ICU) of the Kassala Specialized Hospital for Pediatrics in Sudan, with signs of severe bacterial infection. M. caseolyticus is an opportunistic pathogen normally associate…

Transcriptomic mortality signature defines high-risk neonatal sepsis endotype.

INTRODUCTION: Neonatal sepsis remains a leading cause of global childhood mortality, yet treatment options are limited. Clinical and biological heterogeneity hinders the development of targeted therapies. Gene-expression profiling offers a potential strategy to identify neonatal sepsis subtypes and guide targeted intervention. METHODS: We performed secondary analyses of publicly available gene-expression datasets. Differential gene expression analysis and T-distributed Stochastic Neighbor Embedding (t-SNE) identified biologically relevant patient clusters. Mortality and organ dysfunction were…

Acinetobacter spp. in neonatal sepsis: an urgent global threat.

Neonatal sepsis causes substantial morbidity and mortality, the burden of which is carried by low-income countries (LICs). The emergence of multidrug-resistant pathogens in vulnerable neonatal populations poses an urgent threat to infant survival. Acinetobacter spp. are increasingly responsible for severe disease in neonates globally. The cause of this escalation remains unclear, but host, pathogen and environmental factors are all likely to contribute. Acinetobacter spp. strains are frequently resistant to the first line empirical treatment for neonatal sepsis as recommended by the World Heal…

Utility of Anaerobic Blood Cultures in Neonatal Sepsis Evaluation.

BACKGROUND: Clinicians variably obtain anaerobic blood cultures as part of sepsis evaluations in the neonatal intensive care unit (NICU). Our objective was to determine if anaerobic blood culture bottles yielded clinically relevant information by either recovering pathogens exclusively or more rapidly than the concurrently obtained aerobic culture bottle in the NICU. METHODS: A retrospective cohort study of blood cultures obtained from infants admitted to the NICU from August 01, 2015 to August 31, 2023. Standard practice was to inoculate 2 mL of blood divided equally between an aerobic and a
