Peri-Implant Metastasis as First Manifestation of an Unknown Lung Cancer: Literature Review and Two New Cases.

Metastases around dental implants are extremely rare. They usually represent a secondary localization of a multiorgan metastatic neoplasm but also are, exceptionally, recognized as the first clinical manifestation of a still unknown cancer of whatever organ. Metastases usually manifest as rapidly growing lesions of hard and soft tissues and always represent a true diagnostic/therapeutic dilemma both for clinicians in choosing the more appropriate treatment and for pathologists in recognizing the primary tumor when still undiagnosed. We report 2 distinct cases of metastasis occurring around den

Solitary plasmacytoma of bone and soft tissues.

PURPOSE: To define the optimal treatment and outcomes for patients with solitary plasmacytoma of bone and soft tissue. METHODS: Review of the literature. RESULTS: Solitary plasmacytomas are uncommon and account for less than 5% of plasma cell neoplasms. Solitary plasmacytomas of bone (SPB) usually occur in the vertebra and skull and are more common than extramedullary plasmacytomas (EMP) that almost always arise in the head and neck and may spread to regional lymph nodes. The optimal treatment is moderate-dose radiotherapy (40-50 Gy) and occasionally surgery. Adjuvant chemotherapy does not i

Keratin-Positive Giant Cell-Rich Tumor in Early Infancy: Metastatic Presentation and Imatinib Response.

BACKGROUND: Keratin-positive giant cell-rich tumor (KPGCT) is a rare bone and soft tissue neoplasm, with pediatric and metastatic cases being exceedingly uncommon. OBSERVATION: We report a 1.5-month-old male infant presenting with multifocal metastatic disease involving the skull, adrenal glands, vertebrae, mandible, soft tissue, and long bones. Histopathologic evaluation confirmed KPGCT, molecular testing was negative for HMGA2 rearrangement. Treatment with imatinib resulted in marked regression of lesions and clinical improvement without toxicity. CONCLUSIONS: This case expands the clinica

Current approaches in tissue engineering-based nanotherapeutics for osteosarcoma treatment.

Osteosarcoma (OS) is a malignant bone neoplasm plagued by poor prognosis. Major treatment strategies include chemotherapy, radiotherapy, and surgery. Chemotherapy to treat OS has severe adverse effects due to systemic toxicity to healthy cells. A possible way to overcome the limitation is to utilize nanotechnology. Nanotherapeutics is an emerging approach in treating OS using nanoparticulate drug delivery systems. Surgical resection of OS leaves a critical bone defect requiring medical intervention. Recently, tissue engineered scaffolds have been reported to provide physical support to bone de

Malignant lymphoma with middle ear involvement in a Sprague-Dawley rat.

Sprague-Dawley rats are amongst the most widely used animals in biomedical research and malignant lymphoma has long been known to be a frequent neoplasm in these animals. A 9-month-old male control Sprague-Dawley rat from a toxicity study showed gelatinous material in the cranial cavity and dark, thickened cerebral meninges at necropsy. At microscopic evaluation of the temporal bone, neoplastic lymphocytes were seen invading several structures of the middle ear. The neoplastic cells appeared to extend from the marrow of the temporal bone, covered the dorsal part of the tympanic cavity wall, an

Computed tomography of osteosarcoma after intraarterial chemotherapy.

The response to intraarterial cis-diamminedichloroplatinum II (CDP) chemotherapy was evaluated by computed tomography (CT) in 33 patients with pathologically proved osteosarcoma of the long or flat bones. Twenty-one of the 33 patients had a CT scan before chemotherapy was started. In the other 12 patients, a CT scan was obtained after at least two courses of treatment, and additional studies were performed during the course of therapy. In those patients responding to treatment, the posttherapy scan revealed a remarkable decrease or complete disappearance of the associated soft-tissue mass and

Undifferentiated sarcoma arising from fibrous dysplasia in a young adult.

Fibrous dysplasia (FD) is a skeletal disorder characterized by the replacement of normal bone by fibrous tissue. Malignant transformation of FD is extremely rare and has been reported in both monostotic and polyostotic forms of FD. The most frequently reported malignant transformation is osteosarcoma. Among malignant bone tumors, spindle cell sarcomas are uncommon and difficult to diagnose. This report presents the case of a 30-year-old woman with an unusual presentation of a malignant undifferentiated spindle cell neoplasm secondary to fibrous dysplasia. The clinical features, radiological fi

Extraosseous Ewing sarcoma arising in a chronically lymphedematous limb.

Ewing sarcoma is a highly aggressive small round cell malignant neoplasm of bone and soft tissue that typically is manifested in children and young adults. It is most commonly a primary bone tumor; however, extraosseous cases have been increasingly reported. We report a case of metastatic extraosseous Ewing sarcoma with the primary lesion occurring within a limb affected by primary lymphedema. Lymphedema, in this case, played a role not only in the genesis of the tumor because of the relative local immunosuppression but also in masking the development of the lower limb mass.

Primary Extraskeletal Osteosarcoma in the Anterior Mediastinum: A Case Report and Review.

Extraskeletal osteosarcoma (ESOS) is a malignant soft tissue neoplasm producing osteoid, without any continuity with the bone or periosteum. Primary ESOS presenting in the mediastinum is an extremely rare, yet aggressive malignant tumor associated with a poor prognosis. We report a case of primary ESOS arising from the thymus in a 63-year-old male patient.

Cavernous hemangioma--uncommon presentation in zygomatic bone.

Hemangiomas are benign vascular neoplasms characterized by an abnormal proliferation of blood vessels. They may occur in any vascularized tissue including skin, subcutaneous tissue, muscle, and bone. Intraosseous hemangiomas are uncommon, constituting less than 1% of all osseous tumors. The most frequent sites are the calvaria and the vertebral column. Involvement of the facial bones is rare and occurs most commonly in the maxilla, mandible, and nasal bones. In literature, only 20 cases of zygomatic involvement have been reported. We report a case of an intraosseous hemangioma of the zygoma wi

Plasmacytoma of larynx--a case report.

Plasma cell myeloma, the most common plasma cell neoplasm, is characterized by the presence of multiple lesions in the bone marrow. A single isolated lesion may occur either in bone (solitary plasmacytoma of bone) or in soft tissue (extramedullary plasmacytoma). Most cases of extramedullary plasmacytoma occur in the head and neck region. The diagnosis is established by histopathology and immunohistochemistry. A detailed evaluation for lesions at other sites is recommended as extramedullary plasmacytoma treated by radiation therapy has better survival rates than plasma cell myeloma, which is tr

Malignant Peripheral Nerve Sheath Tumor With Bilateral Adrenal Metastases: Role of 18F-FDG PET/CT in Response Assessment to Tyrosine Kinase Inhibitor and Liposomal Doxorubicin.

MPNSTs (malignant peripheral nerve sheath tumors) are a highly malignant group of soft tissue sarcomas and carry a very poor prognosis. Metastasis to bilateral adrenal glands is very rare in such group of neoplasms. We discuss a case of 85-year-old man who was diagnosed with MPNST from prevertebral mass with metastases to bilateral adrenal glands and bone marrow from the beginning and role of serial F-FDG PET/CT scans in response assessment first to sunitinib (tyrosine kinase inhibitor) and then to liposomal doxorubicin.

Multiple Distinct Neoplasms in a North African Hedgehog (Atelerix algirus).

This paper describes the clinical and pathological features of 4 different tumors, located in the integumentary, digestive, and endocrine systems, presenting in a North African hedgehog (Atelerix algirus). A 3.5-year-old female hedgehog was presented with a cutaneous mass on the right flank. The lesion consisted of a well-differentiated dermal mast cell tumor with no recurrence and metastasis after complete surgical excision. Six months later, the hedgehog developed a mass in the left lower jaw, lethargy, anorexia, and progressive weight loss. Clinical and radiographic evaluations revealed swe

The reproductive system and breast metastases - a narrative review and case series of metastases from soft tissue and bone sarcomas in girls.

Four cases of girls with metastases of soft tissue or bone sarcomas to the reproductive system or breasts are reported. Two patients had metastases to the breast from rhabdomyosarcoma (RMS) of the limbs, one had metastases to the ovary from RMS of the foot, and one had metastases to the uterine venous plexus from chondrosarcoma of the sacrum. In each case, the appearance of metastases was shown in various imaging methods: ultrasound, magnetic resonance imaging, and computed tomography. A thorough literature review confirmed that only a few cases of soft tissue and bone sarcoma metastasis to th

Utility of hydroxyurea in mast cell activation syndrome.

Mast cell activation syndrome (MCAS) is a relatively recently recognized cause of chronic multisystem polymorbidity of a generally inflammatory theme. Patients with MCAS often report migratory soft tissue and/or bone pain which frequently responds poorly to typical (narcotic and non-narcotic) analgesics as well as atypical analgesics such as antidepressants and anticonvulsants. Hydroxyurea (HU) is an oral ribonucleotide reductase inhibitor commonly used in the treatment of chronic myeloproliferative neoplasms and sickle cell anemia. HU has been used to treat systemic mastocytosis, sometimes ef

Peripheral T-cell lymphoma presenting as a soft-tissue mass of the extremity.

An 87-year-old man was found to have a lymphoma in the deep soft tissue of the right shoulder with concomitant central nervous system involvement. There was no evidence of cutaneous, peripheral lymph node, mediastinal, abdominal, or bone marrow involvement. Light microscopic, ultrastructural, and immunohistochemical evaluation characterized the neoplasm as a peripheral T-cell lymphoma. Lymphomas presenting in soft tissue are rare, and the few well-documented cases in the literature are of B-cell origin. We report a T-cell lymphoma presenting in the soft tissue of the extremity, and delineate i

Mantle cell lymphoma presenting as a breast mass.

Breast lymphoma accounts for less than 1% of all non-Hodgkin's lymphomas (NHLs) and approximately 0.1% of all breast neoplasms. Most breast lymphomas are classified as diffuse large B cell or mucosa associated lymphoid tissue (MALT) lymphomas. The case of a 53 year old woman presenting with a breast mass and found to have mantle cell lymphoma is described. Core biopsy of the breast lesion showed a B cell NHL, probably of large cell type and of high grade. Morphological and immunophenotypic analysis of peripheral blood and bone marrow samples suggested a mantle cell lymphoma (MCL). This was con

A mixed mesenchymal sarcoma in the soft palate of a dog: light and electron microscopic findings.

Soft tissue tumors containing a mixture of neoplastic fibrous tissue, cartilage, and bone have previously been classified as extraskeletal osteosarcomas in the dog. These tumors are often poorly differentiated, contain multiple neoplastic cell types, and might be more appropriately called mixed mesenchymal sarcomas. The present neoplasm caused clinical signs of stridorous respiration and dysphagia in a four and one-half year old dog. Four neoplastic cell types were demonstrated by light and electron microscopy.

Soft tissue sarcoma invasion of the humerus via the nutrient foramen in a dog.

A 7-year-old castrated male dachshund dog was presented because of non-weight-bearing right thoracic limb lameness with a soft tissue mass of the proximal humerus. Orthogonal radiographs of the affected limb showed a region of geographic lysis and variable cortical thinning centered in the proximal humeral metaphysis. Differential diagnoses for these lesions included benign and malignant processes, including primary osseous neoplasia. Right forelimb amputation was conducted, and histopathologic assessment facilitated diagnosis of a grade II/intermediate-grade soft tissue sarcoma invading the p

Results and perspectives of the treatment of skeletal neoplasms.

Improvements in the therapy of skeletal neoplasms at the end of the 20th century are the reason for a retrospective view on the development of the therapy of skeletal neoplasms, especially in the Slovak Republic. Up to the sixties years of the 20th century was amputation the imperative in the surgical treatment of malignant tumors, above all tumors of the extremities. Then chemotherapy was introduced into the treatment of these tumors, in Slovak Republic it was later. Bone transplantations were widely used in the 70 and 80 years of 20th century. Autotransplantations of bone grafts were used at
