Malignant Transformation of a Pediatric Intracranial Nongerminomatous Germ Cell Tumor to Embryonal Rhabdomyosarcoma: Case Report and Literature Review.

Malignant transformation of intracranial nongerminomatous germ cell tumors (NGGCTs) is a rare but clinically relevant phenomenon. We present the case of a 13-year-old boy with a localized, pineal NGGCT. After an initial favorable response, tumor growth was noted while still on chemotherapy. Histopathologic characterization revealed that 40% of the tumor was embryonal rhabdomyosarcoma (RMS), consistent with potential malignant transformation. Due to the rare nature of NGGCT malignant transformation, the best clinical approach to these cases remains unclear. We explore existing cases, treatments

Differences in future life expectancy of testicular germ-cell tumor patients vs. age-matched male population-based controls.

BACKGROUND: It is unknown whether five-year overall survival (OS) differs and to what extent between testicular germ-cell tumor (TGCT) patients and age-matched male population-based controls. MATERIALS: We identified newly diagnosed (2004-2014) TGCT patients within Surveillance Epidemiology and End Results database 2004-2019. We compared OS between non-seminoma (NS-TGCT) and seminoma (S-TGCT) patients relative to age-matched male population-based controls based on Social Security Administration Life-Tables. Smoothed cumulative incidence plots displayed cancer-specific mortality (CSM) vs. othe

[Coexistence of two germinal cell tumors, seminomatous and nonseminomatous, with an uncommon clinical presentation].

OBJECTIVES: The existence of non seminomatous mixed germ cell tumors of the testis is a frequent event in urologic oncology. Nevertheless, the presence of both components, seminomatous and non seminomatous, in a germ cell tumor is unusual. METHODS: We present a case of pure classic seminoma of the testis with a lymph node metastasis of pure embryonal carcinoma, with confirmatory immuohistochemical study and clinical outcome of the patient. RESULTS: A 34-year-old man presented with 3 cm supraclavicular tumor. CT scan also revealed multiple metastases in lymph nodes, liver, kidney and left adr

Mixed nonseminomatous germ cell tumor presenting as a subcutaneous tissue mass.

Extragonadal germ cell tumors most commonly arise in the midline of the retroperitoneum or the mediastinum. Primary tumors involving the skin are very rare. Only one case of malignant primary germ cell tumor located in the skin has been reported. We present the case of a 44-year-old white man with a primary subcutaneous mixed nonseminomatous germ cell tumor. This man had a long-lasting subcutaneous lump of the breast, which became painful. Surgery revealed 3 juxtaposed nodules. Microscopic examination showed a mixed germ cell tumor with a 90% immature teratoma component and a 10% embryonal car

Secondary leukaemia after testicular germ cell tumour treatment: a systematic review and meta-analysis.

BACKGROUND: Testicular germ cell tumours (TGCTs) are the most common malignancy in men aged 15-35 years. Management options for men with TGCTs include surgery, radiation and/or chemotherapy, depending on stage. Given TGCTs' excellent survival, most patients live long enough to experience delayed treatment toxicities, warranting careful consideration of therapeutic decisions. An important outcome of interest is the development of secondary leukaemia. METHODS: A systematic literature search was conducted through a combination of database searches (MEDLINE, EMBASE, and Cochrane library) and manu

Clinicopathologic Spectrum of Secondary Solid Tumors of the Prostate of Nonurothelial Origin: Multi-institutional Evaluation of 85 Cases.

Secondary involvement of the prostate by urothelial or hematolymphoid neoplasms is relatively common and well-described. In contrast, less is known about the clinicopathologic spectrum of secondary solid tumors of the prostate of nonurothelial origin. This study evaluated a series of secondary nonurothelial solid tumors of the prostate diagnosed at 21 institutions. Eighty-five patients with a median age at diagnosis of 64 years were included. Sixty-two patients had clinically manifest disease (62/85, 73%), 10 were diagnosed incidentally (10/85, 12%), and 13 (13/85, 15%) had no detailed clinica

Beyond germ cell tumors: focus on Leydig cell neoplasms from a single-center experience.

INTRODUCTION: Leydig cell tumors (LCTs) are rare testicular neoplasms that account for a small proportion of testicular tumors and are often diagnosed incidentally or on investigation of infertility or hormonal symptoms. Despite their generally benign behaviour, a small percentage may have malignant potential, which poses a diagnostic and therapeutic challenge due to the lack of standardizedguidelines. MATERIALS AND METHODS: We retrospectively analysed four casesof histologically confirmed LCTs diagnosed and treated at a single institution between 2000 and 2024. Clinical, biochemical, radiolo

Complete androgen insensitivity syndrome: factors influencing gonadal histology including germ cell pathology.

Patients with complete androgen insensitivity syndrome are at an increased risk for the development of gonadal germ cell cancer. Residual androgen receptor (AR) activity and abnormal gonadal location may influence the survival of atypical germ cells and the development of other histopathological features. To assess this, we evaluated 37 gonads from 19 patients with complete androgen insensitivity (ranging in age from 3 months to 18 years). Histological abnormalities were examined using hematoxylin and eosin-stained sections and sections stained for POU5F1 and KITLG, markers of early changes in

Gonadal tumor in Frasier syndrome: a review and classification.

Frasier syndrome is a rare inherited disease characterized by steroid-resistant nephrotic syndrome, gonadal tumor, and male pseudohermaphroditism (female external genitalia with sex chromosomes XY), which is based on a splice site mutation of Wilms tumor-suppressor gene 1 (WT1). Several unusual Frasier syndrome cases have been reported in which male pseudohermaphroditism was absent. We reviewed 88 Frasier syndrome cases in the literature and classified them into three types (type 1-3) according to external genitalia and sex chromosomes, and described their clinical phenotypes. Type 1 Frasier s

Contemporary diagnostic work-up of testicular germ cell tumours.

Diagnostic work-up of testicular masses should begin with a complete patient history and palpation of the testes with both hands. First-line imaging of the scrotum should be performed using multiparametric ultrasonography, that is the sequential use of grey-scale ultrasonography, colour Doppler ultrasonography (CDUS), and, if available, contrast-enhanced ultrasonography (CEUS) and real-time elastography (RTE). Increased vascularization--a characteristic of malignancy and inflammation--is visualized on CDUS and CEUS. RTE provides additional information for distinguishing between benign and mali

SOX2 and PRAME in the "reprogramming" of seminoma cells.

BACKGROUND: In recent years, several studies investigated the complex process called "reprogramming" of seminoma (S) cells. The accepted pathogenetic model is a complex network including SOX2, SOX17, OCT3/4 and PRAME, which modulates the epigenetic transcription of numerous downstream genes and drives a divergent gene expression profile resulting in the transition from pure S (P-S) to S component (S-C) of mixed germ cell tumors of the testis (M-GCTT), and finally to embryonal carcinoma (EC). Herein, we tested a large cohort of GCTT with SOX2 and PRAME to evaluate their expression in the evolut

[Germ-cell tumors of the brain--their course and management].

Germ-cell tumors (GCT) of the CNS constitute a small group with an incidence varying in the range 0.1 to 3.4 per cent of all primary brain neoplasms. In the WHO classification of brain tumors they are differentiated in a separate group, and further subdivided into: germinoma, teratoma (mature, immature, malignant), embryonal carcinoma, choriocarcinoma, endodermal sinus tumor and mixed germinal tumors. Of them germinoma is the most frequently encountered (41-65 per cent of all GCT), whereas the incidence rate of mixed GCT may reach 32 per cent Mainly median structures are being involved--III ve

[Germ Cell Tumours of the Testis: Classification, Frozen Section Diagnostics, Sentinel Lymph Node Biopsy, and Special Biological Variants].

Germ cell tumours (GCTs) are the most common testicular neoplasms and are classified into type I, type II, and type III tumours depending on age of onset, molecular biology, and histomorphology. There are six histological subtypes, whose distribution varies among the three groups: seminomas, embryonal carcinomas, choriocarcinomas, yolk-sac tumours, teratomas, and spermatocytic tumours. The most common are malignant type II GCTs, typically treated with radical orchiectomy. In cases of small tumours where tumour dignity is uncertain - such as when serum markers are negative - initial enucleation

[Surgical resection of a massive residual retroperineal mass after chemotherapy in non-seminomatous germ cell tumor of the testis: a borderline case].

BACKGROUND: According to the last EAU Guidelines about testicular cancer, surgical resection of residual masses after chemotherapy in NSGCT is indicated in the case of visible residual masses and when serum levels of tumor markers are normal or normalizing. If markers are not normalized and when several chemotherapeutic regimens have failed to cure metastatic disease, resection of residual tumors (so called "desperation surgery") should be offered to these patients. METHODS: We are going to present the case of a 30-year-old patient, affected by metastatic NSGCT. According to the prognostic-ba
