Cardiac Ultrasound for the Nephrologist: Know Thy Heart to Know Thy Kidneys.

Kidney disease patients have a high prevalence of cardiovascular morbidity and mortality. It can be challenging to adequately assess their c … asic cardiac ultrasound techniques. After a short course of training in focused cardiac ultrasound, nephrologists can quickly and reliably assess ventricular size and function, detect clinically relevant pericardial effusion and volume status in their patients. Additional training in Doppler ultrasound can extend their capability to measure cardiac output, righ …

What a paediatric nephrologist should know about preeclampsia and why it matters.

Preeclampsia is a protean syndrome causing a kidney disease characterised by hypertension and proteinuria, usually considered transitory and reversible after delivery. Its prevalence ranges from 3-5 to 10% if all the related disorders are considered. This narrative review, on behalf of the Kidney and Pregnancy Study Group of the Italian Society of Nephrology, focuses on three reasons why preeclampsia should concern paediatric nephrologists and how they can play an important role in its prevention, as well as in the prevention of future kidney and cardiovascular diseases. Firstly, all diseases …

A practical guide to diagnosis and assessment of chronic kidney disease for the non-nephrologist.

Chronic kidney disease (CKD) is common. People with CKD have a wide range of comorbidities and, therefore, the majority of non-nephrologists will care for people with CKD. This paper aims to provide a brief overview of the diagnosis and management of CKD for the non-nephrologist. Identifying those with CKD and optimising treatment is essential as CKD has a direct association with adverse patient outcomes. There are modifiable factors where interventions may delay progression of CKD, including: smoking cessation, dietary advice, hypertension management, renin-angiotensin system blockade, glycae…

[Pediatric nephrology: What should an adult nephrologist know about these diseases?].

In nephrology, some diseases begin specifically during childhood; however, they must be known by adult nephrologists so that to ensure continuity and homogeneity for their management. The aim of this review is therefore to propose a brief overview of the main pediatric diseases, for which a specific knowledge is warranted, and notably pediatric idiopathic nephrotic syndrome, cystinosis, primary hyperoxaluria and hereditaries tubulopathies.

Effective use of loop diuretics in heart failure exacerbation: a nephrologist's view.

Unfortunately, patients with congestive heart failure suffer frequent admissions for the management of fluid overload. Loop diuretics are pivotal in the management of this common clinical problem. Although loop diuretics have been in clinical use since the 1960s, we still do not understand how to optimally administer these drugs. It is unknown why some decompensated heart failure patients exhibit improvements in renal function with diuresis, whereas others display renal function deterioration, limiting attainment of euvolemia. Here the physiologic interactions between the failing heart and kid

Transition to the adult nephrologist does not induce acute renal transplant rejection.

BACKGROUND: In spite of the overall increased renal graft survival, long-term allograft survival has remained least successful in adolescent … n decreases after transition to the adult unit. There is less risk reduction in immigrant patients. Nephrologists should pay special attention to these patients.

Catheter placement by nephrologists: A safe and effective method for improving access to peritoneal dialysis.

INTRODUCTION: Peritoneal dialysis (PD) is an effective treatment option for patients with kidney failure and offers them a better quality of life. Despite its advantages, PD remains underutilized, particularly in Morocco. The aim of our study was to evaluate the impact of catheter placement by nephrologists on the number of incident PD patients, compare the success rates of this technique with those of surgical placement, and assess the short- and long-term complications associated with the two approaches. METHODS: This was a single-center retrospective study including 69 PD patients, conduct…

Epidemiology of End-Stage Kidney Disease on Renal Replacement Therapy: 35 Years Analysis Study Nephrologists.

Incidence and prevalence of end-stage kidney disease (ESKD) population on renal replacement therapy (RRT) for some of the nations are well published. Oman's publication on this aspect is limited and therefore, this study was conducted. This study analyzed the data obtained from the RRT register in Oman. The main measurements and aim are to identify the incidence and prevalence of Oman's treated RRT population (1983-2018) with a major focus on the hemodialysis (HD) cohort. The year 1983 is the year when renal care was started in Oman. The total number of patients registered on Oman's RRT regist

From combined heart-kidney to kidney transplantation program: what nephrologists should know about dilated cardiomyopathy.

A 42-year-old hemodialysis (HD) patient was investigated in our department for symptomatic heart failure (HF) despite daily home dialysis. He had a history of living donor kidney transplantation at the age of 18 that lasted 7 years. Home dialysis was then started. At the age of 40, he developed acute heart failure symptoms. Echocardiography revealed severe dilated cardiomyopathy (DCM). Coronarography and myocardial perfusion scintigraphy showed no abnormal findings. Betablockers were administrated, and RAAS inhibitor dosing was optimized. Dyspnea persisted, and patient was referred to our depa

The impact of integrating nephrologists into the postoperative cardiac intensive care unit: a cohort study.

BACKGROUND/AIMS: We evaluated the potential preventive effect of Nephrology On-Site (i.e. nephrologists integrated into the postoperative cardiac intensive care unit, ICU, team) versus Nephrology On-Demand (i.e. nephrology consultation depending on intensivist criteria) in the ICU on in-hospital outcomes. METHODS: This was a retrospective cohort study comparing outcomes during 2 consecutive time periods: from March 1, 2009 to February 28, 2010 with Nephrology On-Demand, and from March 1, 2010 to February 28, 2011 with Nephrology On-Site. Adult patients admitted to the postoperative cardiac IC…

Echocardiography and right ventricular function in NKF stage III cronic kidney disease: Ultrasound nephrologists' role.

TAPSE measurement during echocardiography is a well known measure of right heart systo-diastolic function. Low TAPSE means reduced cranio-caudal excursion of tricuspidal annulus, sign of both reduced ejection fraction and reduced distensibility of right ventricle. It is a good prognostic index for cardiac mortality risk in CHF patients, adding significant prognostic information to NYHA stadiation. Nephrologists do not always fully aware of right ventricular function in their patients affected by chronic renal failure (CRF), even if this datum is probably crucial in vascular access policy. Our …

The management of iron metabolism in recombinant human erythropoietin treated dialysis patients by Dutch nephrologists.

BACKGROUND: The regulation of iron metabolism is an important aspect of r-HuEPO treatment. METHOD: All Dutch nephrologists involved in dialysis were asked to complete a questionnaire about iron metabolism management in dialysis patients. RESULTS: The response rate was 68%, covering 83% of all Dutch dialysis units. Iron status is assessed before starting r-HuEPO by 96% of the respondents, but only 58% waits for the results. Serum ferritin is determined by 98%, MCV by 77%, transferrin saturation by 44%, the percentage hypochromic red blood cells by 6%, bone marrow iron staining by 4%, and seru…

[French nephrologists’ practices in the management of anemia and iron deficiency in chronic kidney disease].

BACKGROUND: Real-world practices for managing anemia and iron deficiency in chronic kidney disease appear heterogeneous and sometimes controversial. This study aimed to describe the prescribing preferences and habits of French nephrologists in this area. METHODS: All nephrologists seeing patients at one of the 40 centers participating in the Chronic Kidney Disease – Renal Epidemiology and Information Network (CKD-REIN) cohort were invited to participate in two waves (2015–2016 and 2019–2020) of a practice survey. The self-administered questionnaires collected information on nephrologists’ cha…

Expert Opinion on the Conceptual, Clinical, and Therapeutic Aspects of Fabry Nephropathy: A Nephrologists' Perspective.

BACKGROUND: Progressive nephropathy (Fabry nephropathy), as observed in both the early-onset and later-onset types of Fabry disease and in as many as 50% of males and 20% of females, represents a major driver of overall disease burden in this population. Nonetheless, recognizing and diagnosing Fabry nephropathy remains clinically demanding for nephrologists given its pivotal role in altering disease progression and improving patients' quality of life. Therapeutics presents additional complexity as evidence-based criteria for treatment and prognostic algorithms remain limited. SUMMARY: Against…
