Trigeminal neuralgia and glossopharyngeal neuralgia: two orofacial pain syndromes encountered by dentists.

BACKGROUND: Dentists frequently evaluate patients for oropharyngeal pain that may or may not eventually be related to oral pathology. Two rare neurological disorders that present with severe orofacial pain are trigeminal neuralgia, or TN, and glossopharyngeal neuralgia, or GPN. Both are secondary to cranial nerve compression by arteries and veins at the point at which the nerves exit the pons and brainstem. RESULTS: The authors present the results for two series of patients treated for TN and GPN. Significant success can be seen after intracranial microvascular decompression for both disorder…

An update on botulinum toxin treatment of painful diabetic neuropathy, post-traumatic painful neuropathy/neuralgia, post-herpetic neuralgia and occipital neuralgia.

The literature in botulinum toxin treatment for painful diabetic neuropathy (PDN), post traumatic neuralgia (PTN), postherpetic neuralgia (PHN) and occipital neuralgia (ON) was reviewed up to Oct 1st, 2024. Using the efficacy criteria set forth by the Assessment and Guideline subcommittee of the American Academy of Neurology, the current levels of efficacy for these conditions could be designated as followings: PDN: B (probably effective, two class II study), PTN: A (effective, two class I studies); PHN: A (effective, two class I studies), ON: (undetermined due to lack of blinded investigation…

Trigeminal and Glossopharyngeal Neuralgia.

Trigeminal neuralgia and glossopharyngeal neuralgia are craniofacial pain syndromes characterized by recurrent brief shock-like pains in the distributions of their respective cranial nerves. In this article, the authors aim to summarize each condition's characteristics, pathophysiology, and current pharmacotherapeutic and surgical interventions available for managing and treating these conditions.

Neuromodulation for Trigeminal Neuralgia.

Clinical studies on neuromodulation intervention for trigeminal neuralgia have not yet shown promising results. This might be due to the fact that the pathophysiology of chronic trigeminal neuropathy is not yet fully understood. Chronic trigeminal neuropathy includes trigeminal autonomic neuropathy, painful trigeminal neuropathy, and persistent idiopathic facial pain. This disorder is caused by complex abnormalities in the pain processing system, which is comprised of the affective, emotional, and sensory components, rather than mere abnormal sensation. Therefore, integrative understanding of …

Familial classic trigeminal neuralgia.

INTRODUCTION: The classic form of trigeminal neuralgia is usually sporadic (no familial clustering). However, around 2% of all cases of trigeminal neuralgia may be familial. Describing this entity may be useful for diagnosing this process and may also be key to determining the underlying causes of sporadic classical trigeminal neuralgia. We report on cases in a series of 5 families with at least 2 members with classic trigeminal neuralgia, amounting to a total of 11 cases. MATERIAL AND METHODS: We recorded cases of familial classical trigeminal neuralgia between March 2014 and March 2015 by s…

Dofetilide induced trigeminal neuralgia.

Anti-arrhythmic medications are uncommonly used due to their pro-arrhythmic effect. However, just like any other class of medication, they can cause idiosyncratic reactions that may or may not be related to their mechanism of action. Those reactions can be severe enough to warrant discontinuation of a successful therapeutic intervention. In this case, we present a case of trigeminal neuralgia caused by dofetilide.

Management of pudendal neuralgia.

Pelvic pain is a frequent complaint in women during both reproductive and post-reproductive years. Vulvodynia includes different manifestations of chronic vulvar pain with no known cause. Many women do not receive a diagnosis and appropriate treatment. Pudendal neuralgia is a painful condition caused by inflammation, compression or entrapment of the pudendal nerve; it may be related to or be secondary to childbirth, pelvic surgery, intense cycling, sacroiliac skeletal abnormalities or age-related changes. Clinical characteristics include pelvic pain with sitting which increases throughout the …

Radiosurgery in trigeminal neuralgia.

Trigeminal neuralgia is a rare but debilitating pain syndrome with a distinct clinical presentation mainly affecting people advancing in years. Compression of the trigeminal nerve by an aberrant vascular loop has been proposed as the causative factor in the majority of cases leading to the development of a highly effective surgical treatment approach termed "microvascular decompression". Nevertheless the mainstay of treatment remains medical. This article gives a brief overview of current treatment options for trigeminal neuralgia with special emphasis on radiosurgery. Possible mechanisms unde…

[Treatment of trigeminal neuralgia].

Classic idiopathic trigeminal neuralgia is characterized by sharp unilateral shooting pain in the distribution of one or more branches of the trigeminal nerve. It involves a diagnosis of exclusion. Initially, therapy consists of medical therapy, preferably with carbamazepine or oxcarbazepine. For patients refractory to medical therapy, microvascular decompression of the trigeminal nerve provides the best long-term outcomes, at a relatively low complication risk. In case of surgical contraindications, there are other options: radiosurgery or a neurodestructive procedure of the trigeminal gangli…

Genitofemoral neuralgia: a review.

Genitofemoral neuralgia is a cause of neuropathic pain that is often debilitating in nature. It is characterized by chronic neuropathic groin pain that is localized along the distribution of the genitofemoral nerve. The symptoms include groin pain, paresthesias, and burning sensation spreading from the lower abdomen to the medial aspect of the thigh. It may present with scrotal pain in male, while females experience symptoms radiating to the labia majora and mons pubis. Genitofemoral neuropathy has been attributed to iatrogenic nerve injury occurring during inguinal and femoral herniorrhaphy, …

Occipital neuralgia: anatomic considerations.

Occipital neuralgia is a debilitating disorder first described in 1821 as recurrent headaches localized in the occipital region. Other symptoms that have been associated with this condition include paroxysmal burning and aching pain in the distribution of the greater, lesser, or third occipital nerves. Several etiologies have been identified in the cause of occipital neuralgia and include, but are not limited to, trauma, fibrositis, myositis, fracture of the atlas, and compression of the C-2 nerve root, C1-2 arthrosis syndrome, atlantoaxial lateral mass osteoarthritis, hypertrophic cervical pa…

Trigeminal neuralgia and radiofrequency.

Trigeminal neuralgia (TN) is a painful neurological disorder often mistaken for pain of dental origin by the patient and dentist. Dentists should be acquainted with TN to differentiate it from orofacial pain and prevent unnecessary tooth extraction. TN pain alleviating modalities are numerous, yet not uniformly effective. Radiofrequency, known for 25 years, is a minimally-invasive outpatient procedure used for TN when drugs are ineffective. The authors evaluate radiofrequency in trigeminal neuralgia patients referred from the dental office.

Update on trigeminal neuralgia.

Trigeminal neuralgia leads to paroxysms of short-lasting but very severe pain. Between attacks, patients are usually asymptomatic, but a constant dull background pain may persist in some cases. The incidence of trigeminal neuralgia is 4.3 per 100,000 persons per year, with a slightly higher incidence for women (5.9 per 100,000) compared with men (3.4 per 100,000). There is a lack of certainty regarding the etiology and pathophysiology of trigeminal neuralgia. There are a wide range of medical and surgical treatments available. The preferred medical treatment for trigeminal neuralgia consists o…
