OBJECTIVE: Stereotactic radiosurgery has proven effective in the treatment of acoustic neuromas. Prior reports using single-stage radiosurgery consistently have shown excellent tumor control, but only up to a 50 to 73% likelihood of maintaining hearing at pretreatment levels. Staged, frame-based radiosurgery using 12-hour interfraction intervals previously has been shown by our group to achieve excellent tumor control while increasing the rate of hearing preservation at 2 years to 77%. The arrival of CyberKnife (Accuray, Inc., Sunnyvale, CA) image-guided radiosurgery now makes it more practica…

Two hundred and seventy-one Japanese patients who underwent surgical removal of neuroma from the cerebello-pontine angle using the translabyrinthine approach were retrospectively reviewed in order to investigate the nerve origin on an anatomical basis. In 269 out of the 271 cases, the origin of the neuromas was identified. Among these cases, 84.8 per cent of tumours originated from the inferior vestibular nerve (IVN), followed by the superior vestibular nerve (SVN) (8.9 per cent). Of the 5.6 per cent of tumours arising from the vestibular nerve, however, it could not be determined whether they…

The response of the vestibular system after acoustic neuroma surgery was investigated in nine patients. The otolith system was studied by means of ocular counterrolling, assessed by video oculography. Horizontal vestibulo-ocular reflex (VOR) function was tested by the sinusoidal harmonic acceleration test using electronystagmography. The results were compared with those obtained from a normal control population. The response to slow rotation tests was symmetric, but the gain was significantly reduced when compared to the normal population. Phase lag was significantly increased. No difference i…

AIM: To analyse the risk factors involved in the development of ocular complications after acoustic neuroma resection, in particular corneal complications and visual loss, and to identify measures that may reduce these.
METHODS: 62 patients who underwent surgery for acoustic neuroma had a standardised ophthalmic examination and retrospective case note review.
RESULTS: At final review (mean 37.6 months), although 38 patients reported ocular symptoms, only 22% saw 6/12 or worse. Patients with hypoaesthetic corneas had a higher incidence of corneal pathology (79%) than those with normal sensati…

Nowadays smaller acoustic tumors are being detected earlier. We started to remove these tumors with attempt of preservation of hearing in 1994. Since 1994, in an attempt to preserve hearing, the retrosigmoid approach was used. Our first results are presented and criteria of preoperative selection and postoperative evaluation are discussed. A fair chance to keep a useful hearing can be given to a patient with a small tumor, preferrably smaller than 1,5 cm and provided that the average hearing levels differ by no more than 30 dB from the other ear. In all the other cases the translabyrinthine ro

Despite recent advance in diagnosis and surgical techniques of acoustic neuroma, ideal criteria for hearing preservation attempt remain unsatisfactory. This study reports sonomotor vestibular reflex recorded pre and postoperatively in 20 patients with acoustic neuroma. Hearing preservation was attempted according to tumor size and preoperative pure tone audiometry results. The studied population was divided in 2 groups according to hearing preservation was attempted (group 1 n = 13, retrosigmoid approach) or not (group 2 n = 7, translabyrinthin approach). In accordance with postsurgical auditi…

Acoustic neuroma is a rare but important cause of sensorineural hearing loss. Patients with asymmetric hearing loss or unilateral tinnitus should be evaluated expeditiously to prevent further neurologic damage. Gadolinium-enhanced magnetic resonance imaging is the best tool for making the diagnosis of acoustic neuroma. Surgical removal using a translabyrinthine approach, is the favored treatment, although radiation therapy and expectant management may be chosen for patients at high surgical risk.

Hearing conservation in acoustic tumour surgery remains controversial. There have been few previous reports in the British literature. The s … ing conservation should be considered as a factor in the management of patients with small acoustic neuromas.

Two cases of acoustic neuroma are reported. Orofacial anesthesia was the principal symptom in each case. This presentation is unusual for this type of tumor; therefore, the relevant literature is reviewed.

The palisaded encapsulated neuroma is a solitary, small peripheral nerve lesion that occurs primarily on the facial skin of middle-aged adults. We report a series of 13 cases of palisaded encapsulated neuroma occurring on oral mucosa. The lesions presented as solitary, firm, sessile, partly or completely encapsulated, 2 to 3 mm nodules in adults with a mean age of 51 years (median 55 years). Nine of the neuromas occurred on the hard palate; the remainder were found on the soft palate, lower lip mucosa, upper lip mucosa, and maxillary anterior alveolar ridge. None of the cases was associated wi…

Thirteen patients with acoustic neuroma were studied on a 1.5 T superconductive magnetic resonance (MR) imager. Acoustic neuromas appeared as lower signal intensity than the surrounding brain stem on T1 weighted image (W.I.), and as higher signal intensity on T2 W.I.. Axial and coronal sections of T1 W.I. were very useful in observing the tumor in the auditory canal and in investigating the anatomical relations of the tumor and the surrounding structures. MR imaging is very excellent examination to make early diagnosis of the acoustic neuroma and preoperative anatomical evaluation.

In this series, 27 patients underwent surgery for recurrent Morton's neuroma through a plantar incision. All patients were women, with an age range of 35 to 65 years. Followup ranged from 12 months to three years. Twenty-one patients had recurrent neuromas in the third intermetatarsal space, and six patients had recurrence in the second intermetatarsal space.Twenty-four of 27 patients had excellent or good results, a success rate of 89%. Three patients with poor results had undergone previous surgery, including metatarsal osteotomies and adjacent nerve exploration, with resultant fat pad atrop…

The records of 10 patients, who underwent removal of small vestibular schwannomas by the middle cranial fossa approach at the University of Iowa Department of Otolaryngology and Maxillofacial Surgery between October 1974 and November 1977, are reviewed. The principal deviation from previously described operative techniques is that after identification of appropriate landmarks, the internal auditory canal is first unroofed in its medial portion near the porus rather than following the facial nerve to the fundus of the canal. There were no intraoperative complications, but one patient died after
