Air embolism in spinal surgery.

We present two cases of fatal venous air embolism that occurred in previously healthy adolescents during posterior spine fusion and instrumentation for scoliosis. Previously recorded cases in the literature have been rare and only briefly discussed. Air embolism has been a complication noted in the anesthesia and neurosurgical literature, occurring primarily in total hip patients and those patients undergoing neurosurgical procedures in a sitting position. An examination of the pathophysiology of air embolism in posterior spinal procedures would indicate the use of properly placed central line

Spontaneous pneumocephalus caused by pneumococcal meningitis.

Pneumocephalus is a condition characterized by the presence of air in the cranium, and it is mainly caused by trauma or a neurosurgical procedure. In the absence of head trauma or a neurosurgical procedure, meningitis is an extremely rare cause of pneumocephalus. Here, the authors present a rare case of spontaneous pneumocephalus caused by pneumococcal meningitis, in which simple lateral radiography and computed tomography (CT) findings of the skull suggested the diagnosis. Cerebrospinal fluid analysis showed bacterial meningitis which later revealed streptococcus pneumonia. The patient was tr

Safety and efficacy of microsurgery for complex cranial pathologies in the ultra-geriatric population.

OBJECTIVE: The global increase in the "ultra-geriatric" population (aged 80 years and older) has led to higher demand for neurosurgical procedures in this vulnerable population. The objective was to evaluate the safety and efficacy of advanced microsurgical procedures on ultra-geriatric patients, in our modern era of neurosurgery, anesthesiology, and advanced medical care. METHODS: The study examined 66 complex cranial procedures, performed on 65 ultra-geriatric patients, who constituted 3.8% of the 4461 consecutive neurosurgical procedures performed by a single neurosurgeon over 14-years in

Meeting tomorrow's needs: a single-centre study in geriatric neurosurgery.

BACKGROUND: The world is experiencing a demographic shift towards a larger proportion of older people. This has important consequences for the healthcare system, including neurosurgical departments. Geriatric patients represent a range of unique challenges related to treatment, resource allocation, prioritization and ethical concerns. In order to meet the challenge of increasing demand of geriatric neurosurgical care, and prepare for planning and establishment of health service requirements, we must first understand the demographic trends, common diagnoses, and types of procedures geriatric pa

Surgery without assistance in newborns and infants with hydrocephalus and neural tube defects.

Standard neurosurgical procedures for hydrocephalus and open neural tube defects in newborns and infants under 6 months of age were performed by a single neurosurgeon on his own without the help of an assistant or scrub nurse. The objective of this study was to assess the outcome of these procedures in terms of operating time, the presence of bacterial infection, and wound healing. Between 2001 and 2004, a total of 126 procedures were performed on 82 patients under 6 months of age. We observed 1 bacterial and 2 fungal infections. Two infections had already been detected at the beginning of the

Smartwatch-based wrist tremor assessment in neurosurgical simulator training.

While surgical skill is acquired during training, objectively evaluating training's impact is challenging. However, new possibilities in measuring wrist tremor during surgical procedures have emerged due to novel wearable sensors. In this study, the effect of simulator-based neurosurgical resident training on wrist tremor was assessed using a smartwatch. A study involving nine neurosurgical residents performing simulator-based training on unruptured intracranial aneurysm clipping procedures was conducted. Residents performed four training sessions on two days (180 clippings in total). Three co

Neurosurgery and pregnancy.

BACKGROUND: Pregnant women with pathological conditions requiring a neurosurgical intervention pose a unique therapeutic challenge. Changes in normal physiology add to the complexity of patient management. We describe our experience in treating various neurosurgical diseases in parturient women. METHODS: Thirty-four pregnant and early postpartum women were treated at our center between 2003 and 2010. The general guideline used in these patients (now deserving re-evaluation based on the presented data) was to postpone surgery until the patient reached term (weeks 34-38 of gestation) unless the

The effect of institutional learning curve on paediatric ventricular shunt survival: a retrospective cohort study from a new paediatric neurosurgical centre.

INTRODUCTION: Ventricular shunt procedures and their outcomes are commonly considered a benchmark for the performance of a neurosurgical service. The influence of an institutional learning curve on the outcomes of neurosurgical procedures has not been established; however, it is important when considering the establishment of neurosurgical centres or the introduction of new operating room teams. A benchmark high-volume procedure such as paediatric ventricular shunt insertion represents an ideal model to study this phenomenon. METHODS: We conducted a retrospective review of a prospectively mai

Gram-negative bacillary meningitis in an immunocompetent adult.

Escherichia coli is a rare cause of community-acquired meningitis comprising about 1% of adult cases. However, it is a common pathogen in neonatal meningitis and in nosocomial setting (especially after penetrating craniocerebral injury or subsequent to neurosurgical procedures). We report a middle-aged woman, who was admitted with features of acute meningitis and subsequent investigations revealed E. coli growth in cerebrospinal fluid culture. The case is distinctive as no additional predisposing risk factors associated with gram-negative bacillary meningitis (traumatic brain injury, neurosurg

Usefulness of Leksell GammaPlan for preoperative planning of brain tumor resection: delineation of the cranial nerves and fusion of the neuroimaging data, including diffusion tensor imaging.

Leksell GammaPlan (LGP) software was initially designed for Gamma Knife radiosurgery, but it can be successfully applied to planning of the open neurosurgical procedures as well. We present our initial experience of delineating the cranial nerves in the vicinity of skull base tumors, combined visualization of the implanted subdural electrodes and cortical anatomy to facilitate brain mapping, and fusion of structural magnetic resonance imaging and diffusion tensor imaging performed with the use of LGP before removal of intracranial neoplasms. Such preoperative information facilitated choosing t

[Neurosurgical treatment of Parkinson disease and other movement disorders].

The surgical treatment of movement disorders has attracted widespread attention in the past few years. Better understanding of pathophysiological mechanisms, and also the refinement of neurosurgical, neuroradiological and neurophysiological methods and techniques have contributed to the redefinition of surgical treatment. With appropriate selection criteria, symptomatic and functional benefit can be achieved in the majority of patients. Functional neurosurgical procedures nowadays are performed most frequently in patients with Parkinson's disease or cervical dystonia. Functional neurosurgery t

New-Onset POAF After Craniotomy: Impact on Neurological Outcome, ICU Utilization, and Mortality.

Background/Objectives: Atrial fibrillation is the most common sustained arrhythmia and frequently occurs in the perioperative setting. However, its clinical relevance in neurosurgical patients remains poorly defined despite increased vulnerability related to brain injury, inflammation, and perioperative stress. This study aimed to determine whether newly detected postoperative atrial fibrillation (POAF) identifies a higher-risk profile and is associated with postoperative complications, resource utilization, and short-term mortality compared with patients remaining in sinus rhythm (SR). Method

Ten-year review of neonatal neurosurgical outcomes and cost analysis.

BACKGROUND: Neurosurgical interventions in neonates may contribute to increased neonatal intensive care unit (NICU) stay. The impact of neurosurgical interventions on length of stay (LOS) and cost is not well-documented in the literature. In addition to LOS, other factors may impact overall resource utilization. Our objective was to perform a cost analysis of neonates who underwent neurosurgical procedures. METHODS: A retrospective chart review was performed on NICU patients who underwent ventriculoperitoneal and/or subgaleal shunt placement between January 1, 2010, and April 30, 2021. Postop

Neurosurgical treatment of Tourette's syndrome: a critical review.

Some patients with Tourette's syndrome (TS) remain disabled despite conventional treatment. Recently, neurosurgical procedures have been reported to be potentially effective interventions for such intractable cases. Clinicians are now being asked to make recommendations to patients about these candidate operations. This review explores the reported experience with neurosurgical treatment of TS to assess critically the evidence regarding risks and benefits. Toward that end, the rationale for the various procedures and the relevant neuroanatomy are outlined and recommendations for patient select
