OBJECTIVE: To determine the cytomorphologic spectrum of nipple discharge in florid gynecomastia.
STUDY DESIGN: During a 22-year period (July 1979-June 2001), nipple discharge from nine males with breast lesions were examined. Smears from four of these cases with histologically documented gynecomastia were reviewed along with the tissue sections.
RESULTS: In the three patients with florid gynecomastia the smears were cellular, with numerous benign ductal cells and papillary fragments along with foam cells and inflammatory cells. Epithelial atypia was mild in two cases and moderate in one. One

The main objective of this study is to illustrate the role of ductography in the management of nipple discharge. Thirty-six patients had ductography for the investigation of blood-stained or serous nipple discharge. The ductogram findings were reviewed. Ductography showed an intraductal lesion in 13 patients, duct ectasia in 16 and normal ducts in 7. Fourteen patients had surgery. In a retrospective study the authors analyzed 100 cases of women with serous, serosanguineous, and sanguineous nipple discharges, without palpable and radiologically visible tumors, that were submitted to resection of main galactophorous ducts and biopsy of underlying breast parenchyma. The sensibility of the cytologic examination of nipple discharges for carcinomas was 16.6%. The histopathology showed incidence of 61% of mammary dysplasia, 33% of intraductal papillomas single or multiple and 6% of carcinomas. The early diagnosis of carcinoma gave good results after radical surgical treatment.

Each uni- or bilateral secretion of the nipple--spontaneous or easy to trigger--has to be examined thoroughly, independent of the colour or the amount of the secretion. Colours of the secretions do not refer to a specific breast disease. Main causes are functional disease with and without endogenous increase of prolactin secretion and benign and malignant breast diseases. Mastopathia, papilloma, fibroadenoma, nonpuerperal mastitis, and carcinoma of the breast are the most common pathological findings associated with secretions of the mammary gland.

Gynecomastia and bloody nipple discharge are very rare in childhood. In this report, a case of infantile gynecomastia together with bloody nipple discharge is presented. A hemorrhagic discharge was expressible from the mammary gland. Endocrinologic findings were within the normal limits. Because of persisting bleeding, a subcutaneous mastectomy was performed. The specimen was reported as gynecomastia. This boy is the first reported prepubertal gynecomastia with bloody nipple discharge in the medical literature.

A case of severe abruptio placentae evolved during the course of prepartum electronic fetal heart rate (FHR) monitoring which was associated with nipple stimulation. The fetal monitor tracing and clinical course are discussed. The unpredictability of abruptio placentae is underscored by the normal appearance of the FHR tracing immediately preceding the event. The association of nipple stimulation with abruptio placentae in this case raises concerns about the potential dangers of this surveillance technique.

We report a case of an intussuscepted bladder nipple following construction of an ileal ureter. The ileal ureter has become a valuable option in reconstructing the urinary system. In a review of 249 office patients who complained of nipple discharge, breast nodularity and duct ectasia was the cause in three quarters. The clinical diagnosis was made by observing that more than one duct was involved and by the color of the discharge. Mamilloscopy is useful in identifying the mostly inflammatory lesions and dermatoses of the mamilla and areola and to follow up and supervise the course of treatment of these diseases. Two improvements in the methodology for obtaining and preparing nipple aspirates from nonlactating women are reported. The first is the development and use of a new breast pump with a controllable vacuum and cups of various sizes. The second is the use of centrifugal cytology to prepare the dispersions. Twenty-one of 24 breasts of patients in the age range 30 to 49 years produced cellular dispersions which contained foam cells; of them, 13 contained ductal cells. A comparison of glutaraldehyde and ethanol fixation indicated that the cells appeared substantially the same.

In 40 breasts with primary carcinoma, the nipple and areola were cut horizontally in order to investigate the frequency of intraductal and invasive cancer. In 20 of these breasts the nipple and/or areola were found to be involved at a depth of 1 cm (50%; 95% confidence limits: 33.8-66.2%). The records of all patients in whom a mastectomy was performed for carcinoma of the breast at this clinic during a recent two year period were reviewed, and the gross specimens were re-examined to study the incidence of cancerous involvement of the nipple and areola and related local clinical findings.
