Noninvasive mechanical ventilation in early acute respiratory distress syndrome.

Noninvasive ventilation (NIV) has a well-‑established role in the treatment of acute-‑on-‑chronic respiratory failure and cardiogenic pulmonary edema. Its role in acute hypoxemic respiratory failure has been increasingly investigated, but its impact on the management and outcome of the subset of patients with acute respiratory distress syndrome (ARDS) is still to be determined. ARDS could be a risk factor for NIV failure, and in these patients, delayed endotracheal intubation can lead to an increased mortality. On the other hand, in a subset of patients with ARDS, endotracheal intubation can b…

Noninvasive ventilation in life-threatening asthma: A case series.

BACKGROUND: The use of noninvasive ventilation (NIV) in severe acute asthma is controversial. A pH < 7.25, PaCO2 > 60 mmHg, and altered mental status have been described as contraindications to NIV in acute asthma. We hypothesized that NIV was safe and effective in asthma patients with a pH < 7.25 or PaCO2 > 60 mmHg. METHODS: Following institutional review board approval, the medical records of subjects who received NIV for acute asthma in the emergency department between January 2010 and July 2012 were reviewed. Subjects were included if they had a pH < 7.25 or PCO2 > 60 mmHg on either an ar…

Noninvasive ventilation for patients with hypoxemic acute respiratory failure.

Noninvasive ventilation (NIV) has an established efficacy to improve gas exchange and reduce the work of breathing in patients with hypoxemic acute respiratory failure. The clinical efficacy in terms of meaningful outcome is less clear and depends very much on patient selection and assessment of the risks of the technique. The potential risks include an insufficient reduction of the oxygen consumption of the respiratory muscles in case of shock, an excessive increase in tidal volume in case of lung injury, and a risk of delayed or emergent intubation. With a careful selection of patients and a…

Fatal pneumomediastinum associated with use of noninvasive mechanical ventilation.

We present a case of fatal pneumomediastinum in a patient with acute respiratory failure caused by acute exacerbated chronic obstructive pulmonary disease (AECOPD) and interstitial lung disease (ILD) precipitated by noninvasive mechanical ventilation (NIMV). To our knowledge, this is the first case reported in the literature. NIMV is very useful to treat acute respiratory failure due to AECOPD improving survival and avoiding endotracheal intubation. Use of NIMV in end stage ILD is not standardized and efficacy is to be proven. No data are reported to manage patient with concomitant COPD and IL

A trial comparing noninvasive ventilation strategies in preterm infants.

BACKGROUND: To reduce the risk of bronchopulmonary dysplasia in extremely-low-birth-weight infants, clinicians attempt to minimize the use of endotracheal intubation by the early introduction of less invasive forms of positive airway pressure. METHODS: We randomly assigned 1009 infants with a birth weight of less than 1000 g and a gestational age of less than 30 weeks to one of two forms of noninvasive respiratory support--nasal intermittent positive-pressure ventilation (IPPV) or nasal continuous positive airway pressure (CPAP)--at the time of the first use of noninvasive respiratory support

Noninvasive ventilation in pulmonary edema complicating acute myocardial infarction.

BACKGROUND: Because it remains unclear whether noninvasive ventilation (NIV) is an effective therapy for cardiogenic pulmonary edema secondary to acute myocardial infarction (AMI), we retrospectively evaluated our experience with NIV in the treatment of pulmonary edema secondary to AMI and other cardiac conditions. METHODS AND RESULTS: The study group included 206 patients with cardiogenic pulmonary edema, divided into an AMI group (53 patients) and a non-AMI group (153 patients). The weaning rate from NIV was similar in the AMI and non-AMI groups (90.6% vs. 90.8%, P=0.950). Heart rate, blood…

Noninvasive mechanical ventilation in patients having declined tracheal intubation.

PURPOSE: Noninvasive ventilation (NIV) is a treatment option in patients with acute respiratory failure who are good candidates for intensive care but have declined tracheal intubation. The aim of our study was to report outcomes after NIV in patients with a do-not-intubate (DNI) order. METHODS: Prospective observational cohort study in all patients who received NIV for acute respiratory failure in 54 ICUs in France and Belgium, in 2010/2011. RESULTS: Goals of care, comfort, and vital status were assessed daily. On day 90, a telephone interview with patients and relatives recorded health-rel…

Leakage estimation using Kalman filtering in noninvasive mechanical ventilation.

Noninvasive mechanical ventilation is today often used to assist patient with chronic respiratory failure. One of the main reasons evoked to explain asynchrony events, discomfort, unwillingness to be treated, etc., is the occurrence of nonintentional leaks in the ventilation circuit, which are difficult to account for because they are not measured. This paper describes a solution to the problem of variable leakage estimation based on a Kalman filter driven by airflow and the pressure signals, both of which are available in the ventilation circuit. The filter was validated by showing that based

[Does noninvasive ventilation avoid intubation of the critical patient?].

In recent years we have witnessed an increased use of noninvasive ventilation (NIV) in our intensive care units (ICUs). Its use is intended mainly in cases of acute respiratory failure (ARF), in order to improve arterial oxygenation, increase alveolar ventilation, decrease work of breathing and avoid intubation. For this reason, we designed a study that has aimed to quantify the number of patients who have benefited from NIV during their admission, to know the type of NIV used in these patients and to determine whether the use of NIV has avoided tracheal intubation. We performed a retrospectiv…

Palliative noninvasive ventilation in patients with acute respiratory failure.

Over the last two decades, the increasing use of noninvasive ventilation (NIV) has diminished the need for endotracheal ventilation, thus decreasing the rate of ventilation-induced complications. Thus, NIV has decreased both intubation rates and mortality rates in specific subsets of patients with acute respiratory failure (e.g., patients with hypercapnia, cardiogenic pulmonary edema, immune deficiencies, or post-transplantation acute respiratory failure). NIV is also increasingly used as a palliative strategy when endotracheal ventilation is deemed inappropriate. In this context, palliative N…

Physiological changes during low- and high-intensity noninvasive ventilation.

In a physiological randomised cross-over study performed in stable hypercapnic chronic obstructive disease patients, we assessed the short-term effects of two settings of noninvasive ventilation. One setting was aimed at maximally reducing arterial carbon dioxide tension (P(a,CO(2))) (high-intensity (Hi) noninvasive positive pressure ventilation (NPPV)): mean ± SD 27.6 ± 2.1 cmH(2)O of inspiratory positive airway pressure, 4 ± 0 cmH(2)O of expiratory positive airway pressure and respiratory rate of 22 breaths · min(-1). The other was performed according to the usual parameters used in earlier …

Noninvasive positive pressure ventilation: Increasing use in acute care.

In the past 2 decades, noninvasive positive pressure ventilation (NIPPV) has been increasingly used in acute respiratory failure to avoid the risks associated with intubation. It is now considered standard first-line therapy in several situations. In this review, we summarize how NIPPV has evolved, the current level of evidence that supports its use in various clinical situations, its potential contraindications, and its limitations in acute respiratory failure.

Noninvasive ventilation in postoperative care of lung transplant recipients.

Noninvasive positive pressure ventilation (NIPPV), which provides consolidated treatment of both acute and chronic respiratory failure, is increasingly being used in the postoperative care of lung transplant patients. Graft- and patient-related respiratory insufficiency requiring mechanical ventilation are common features in the postoperative period; they may persist for hours to days. Prolonged intubation, particularly in these immunocompromised patients, has been considered one of the main predisposing factors for developing nosocomial pneumonia. It has been associated with increased length

Factors predicting successful noninvasive ventilation in acute lung injury.

PURPOSE: Noninvasive ventilation (NIV) has been successfully used to treat various forms of acute respiratory failure. It remains unclear whether NIV has potential as an effective therapeutic method in patients with acute lung injury (ALI). The aims of this study were to determine factors predicting the need for endotracheal intubation in ALI patients treated with NIV, and to promote the selection of patients suitable for NIV. METHODS: We conducted a retrospective study of all patients admitted to the intensive care unit (ICU) of the Nippon Medical School Hospital from 2000 to 2006 with a dia…

Noninvasive ventilation in patients with hypoxemic acute respiratory failure.

PURPOSE OF REVIEW: To discuss the recent literature concerning the use of noninvasive ventilation for hypoxemic acute respiratory failure. RECENT FINDINGS: The benefits of noninvasive ventilation for patients with hypoxemic acute respiratory failure are unclear. In immunocompromised patients and following thoracic surgery, there is a strong rationale for using noninvasive ventilation to treat acute respiratory failure. Prophylactic continuous positive airway pressure after abdominal or thoracic surgery and prophylactic noninvasive ventilation in patients at risk of extubation failure have pro…

[Noninvasive mechanical ventilation in patients with stable severe COPD].

Noninvasive positive pressure ventilation in patients with stable chronic obstructive pulmonary disease. The role of non-invasive positive pressure ventilation (NIPPV) is well documented in patients with restrictive thoracic diseases like kyphoscoliosis, tuberculosis sequelae or neuromuscular disease. There is also a good evidence for the use of NIPPV in acute respiratory failure in patients with an exacerbation of COPD. The application of NIPPV in patients with chronic respiratory failure is growing, but there is less evidence than in restrictive disorders. NIPPV can unload the respiratory mu
