Paraneoplastic Syndromes in Neuro-Ophthalmology.

BACKGROUND: Paraneoplastic syndromes that affect the visual pathways and present with neuro-ophthalmologic signs or symptoms may involve the afferent or efferent systems. Afferent syndromes may involve the optic nerve or retina and, in some cases, these may be associated with systemic neurologic disease. Efferent symptoms typically affect eye movements and may involve the neuromuscular junction or involuntary eye movements. EVIDENCE ACQUISITION: Literature review and personal clinical and research experience. RESULTS: Diagnosis of paraneoplastic syndromes relies on clinical and laboratory ev

Vascular disorders in neuro-ophthalmology.

PURPOSE OF REVIEW: The aim is to briefly discuss the currently controversial management of nonarteritic anterior ischemic optic neuropathy (NA-AION) and central retinal artery occlusion (CRAO). RECENT FINDINGS: The role of systemic corticosteroid therapy and aspirin in NA-AION and of thrombolysis in CRAO is discussed. SUMMARY: NA-AION is a major cause of seriously impaired vision. A recent large prospective study has shown that systemic corticosteroid resulted in a significantly higher probability of improvement in visual acuity (P = 0.001) and visual fields (P = 0.005), compared to an untre

[Clinical neuro-ophthalmology of vision].

To diagnose a patient with a failing visual system, it is necessary to localize the site of the lesion in the system, and identify the etiology that has produced it. Physicians do not see diseases but just their manifestations. Clinical neuro-ophthalmology provides the basic principles on how to progress from manifestations to the diseases they indicate. The Frank B. Walsh Neuro-Ophthalmology Society (the Walsh Society) that originated in 1969 in the United States has been the center of clinical neuro-ophthalmology case studies throughout the world. In Japan, the Ronald M. Burde Clinical Neuro…

Ophthalmology in proverbs and aphorisms.

After a comprehensive review of available texts and literature covering five millennia, the authors present a collection of ophthalmic proverbs and aphorisms with an examination of their applicability within current medical knowledge. A total of 74 ophthalmic proverbs and aphorisms were retrieved from 130 texts and divided into seven categories: (i) importance of vision ('The eyes, like sentinels, hold the highest place in the body'); (ii) definition of blindness and anatomical observations ('The eye is protected by the lashes, eyebrows, and orbit'); (iii) diseases of the eye such as presbyopi

Neuro-ophthalmology of mitochondrial diseases.

PURPOSE OF REVIEW: To review recent data on mitochondrial diseases with emphasis on their neuro-ophthalmic manifestations. RECENT FINDINGS: Numerous studies have associated mitochondrial diseases with neuro-ophthalmic manifestations. Although there has been an explosion of studies on the genetics of mitochondrial diseases over the past few years, pathogenesis is only partly understood and therapy remains very limited. Over the past year, new mutations in Leber's hereditary optic neuropathy have been reported, and at least three genes associated with autosomal dominant chronic progressive exte

Developmental neurogenetics and neuro-ophthalmology.

The field of developmental neurogenetics has burgeoned over the past decade. Through the combined efforts of developmental biologists, geneticists, and clinicians, genetic defects resulting in neuro-ophthalmic disorders such as holoprosencephaly, microphthalmia, dominant optic atrophy, and optic nerve colobomas have been identified and characterized at the molecular level. Experimental studies in model organisms are continuing to identify novel genes critical for ocular and central nervous system development. Mutations in some of these genes have revealed a spectrum of pathology similar to tha

Color-Doppler sonography in ophthalmology.

BACKGROUND: With the present study we considered ocular blood flow by the use of color Doppler ultrasound. We compared the most clinically important arteries of both eyes. We valued the central retinal artery, the posterior long ciliary arteries, the posterior brief ciliary arteries and the central retinal vein. METHODS: We submitted to an ecd examination of the ophthalmic arteries 50 patients, of whom 30 were healthy, 4 presented a pallor of the optic disk, 6 were affected by chronic simple glaucoma and 10 were affected by arterial hypertension. RESULTS: This study has allowed us to define

Mitochondrial disorders in neuro-ophthalmology.

The pathogenesis of mitochondrial disorders relevant to neuro-ophthalmology has been further clarified by identifying and correlating to disease manifestations the three primary pathogenic mutations of Leber's hereditary optic neuropathy, and by correlating genetic, biochemical, and histochemical abnormalities in the mitochondrial myopathies and encephalomyopathies.

Neuro-ophthalmology of multiple sclerosis.

Neuro-ophthalmic manifestations are important in multiple sclerosis. Optic neuritis is a frequent harbinger of the disease, and is characterized by subacute painful visual loss, with disproportionate loss of color and contrast sensitivity, central or cecocentral scotoma, and an afferent pupillary defect. Visual recovery is the rule, but tests have been developed to detect most cases of recovered optic neuritis. Intraocular inflammation has come to be recognized as a frequent accompaniment of multiple sclerosis. Because of the predilection of multiple sclerosis for the infratentorial white matt

Update on llama medicine. Ophthalmology.

Review of the limited literature on camelid eyes suggests they are anatomically similar to those of domestic livestock species, except they lack meibomian glands and have iridial folds (rather than corpora nigra). The microbial flora of the healthy camelid conjunctival sac also appears to be similar to those of domestic livestock and pets, except that no Mycoplasma have been isolated from camelids. Ocular diseases for which camelids are presented to veterinarians are numerous and varied. The most frequently presented conditions are ocular trauma and congenital abnormalities. Trauma to cornea,

Office Ophthalmology: What is Urgent?

The author describes eye problems in general practice, from problems appearing in neonates to those in the elderly. Refractive problems are most common. Visually threatening eye disease that requires urgent treatment or referral is important, despite its rarity. Early detection and treatment of amblyopia is important in children. In diabetic retinopathy, a potentially blinding disease, maculopathy and proliferative retinopathy require immediate referral. Chronic simple glaucoma and cataracts cause gradual loss of vision in the older adult. Sudden loss of vision can be caused by optic neuritis,

Acquired immunodeficiency syndrome and ophthalmology.

1. Ocular manifestations of AIDS are, for the most part, due to the opportunistic infections and neoplasias seen in the syndrome. 2. Ocular infections seen in AIDS patients include cytomegalovirus, Toxoplasma gondii, Candida sp, Cryptococcus sp, herpes simplex or zoster, Mycobacterium sp, and ocular syphilis. 3. Handwashing and disinfection of instruments are recommended to prevent transmission of infection in the ophthalmic practice.

Neuro-ophthalmology for the pediatrician.

The pediatrician needs to develop some skills in evaluating afferent visual functions and ocular motor abnormalities. He must know some fundamental neuro-ophthalmologic facts to prevent his patients from undergoing unnecessary diagnostic and surgical procedures. In addition, he needs to understand the fundamentals of strabismus and amblyopia, which are briefly considered in this article. First, the most common type of nystagmus in children is congenital nystagmus. These children often have a head turn or tilt. Also, it should be remembered that numerous drugs may cause nystagmus. Second, any c

[Psychosomatic correlations in pediatric ophthalmology].

Somatopsychic disturbances encompassing both ophthalmologic as well as child-psychiatric phenomena mainly occur in ADD: Central disorders of eye motoricity (strabismus) and disorders of visual perception are encountered. Dyslexia or dysorthography often occur as secondary phenomena. The psychosomatic or psychogenic disturbances affect not only the lid mechanism but also visualization. In the lid region, nictation tics and trichotillomania of the eyebrows and eyelashes occur. Psychic impairment of vision may range from a slight amblyopia, often associated with limitation of the field of vision,
