Supported employment and compensatory strategies for enhancing vocational outcome following traumatic brain injury.

This paper describes two approaches to improving the employment prospects of those suffering from head injury. It is noted that unemployment within the first 7 years post-head injury is as high as 70% and that this contributes to the long-term difficulties faced by the head-injured and their families. A 'supportive' approach to employment involving the use of job coordinators or job coaches is described and a number of 'compensatory' strategies described. These two approaches are compatible and are derived from the individual needs of the head injured person in relation to retaining or seeking

Financial factors and the implementation of medications for treating opioid use disorders.

OBJECTIVES: Despite the established effectiveness of pharmacotherapies for treating opioid use disorders, implementation of medications for addiction treatment (MAT) by specialty treatment programs is limited. This research examined relationships between organizational factors and the program-level implementation of MAT, with attention paid to specific sources of funding, organizational structure, and workforce resources. METHODS: Face-to-face structured interviews were conducted in 2008 to 2009 with administrators of 154 community-based treatment programs affiliated with the National Institu

Polio eradication is just over the horizon: the challenges of global resource mobilization.

This study draws lessons from the resource mobilization experiences of the Global Polio Eradication Initiative (GPEI). As the GPEI launched its eradication effort in 1988, it underestimated both the difficulty and the costs of the campaign. Advocacy for resource mobilization came as an afterthought in the late 1990s, when achieving eradication by the target date of 2000 began to look doubtful. The reality of funding shortfalls undercutting eradication leads to the conclusion that advocacy for resource mobilization is as central to operations as are scientific and technical factors.

Developing a multidisciplinary complex wound care service.

This article will describe the development of a dedicated multidisciplinary complex/chronic wound care service. Using a recognized framework, the article will demonstrate how the needs of patients were first assessed and then addressed. The article will conclude that although more research is required, the results of this audit suggest that patients with long-standing complex/chronic wounds benefit from the care of a dedicated multidisciplinary wound care team. For the purpose of this article the term 'complex' is defined as any wound which is not progressing as expected despite the applicatio

Department of palliative care in Bratislava and the development of the palliative care movement in Slovakia.

Slovakia is a country with no tradition of home care services and a long history of regarding death and dying as taboos and therefore institutionalising them. Increased attention to palliative care issues has resulted in some important changes, to the benefit of patients in need of palliative care. These include general availability of oral slow-release forms of strong opioids (cost completely reimbursed by the insurance companies), a developing network of home care agencies, and increased attention to the needs of palliative patients, especially among oncologists and pain specialists. The administrative and clinical rationale for the total organization approach to continuous quality improvement.

In our view TQM and CQI represent important innovations in the continuing effort to develop higher performance organizations. Never before has the need been so great to improve quality while at the same time constraining, or reducing, costs. An increasing number of health care organizations can document their experiences that as quality goes up, costs can come down. The contribution of these new approaches is in some sense the wedding of many long established methodologies--the scientific method, statistical quality control, planning, joint problem solving, participative management, and empowe

Availability of quality vaccines: policies of a non-government organization.

The availability of vaccines, or any other health service, depends upon, first, the existence of a reliable system of delivery, and the effective management of this system to reach the target population and, second, the acceptance by parents or guardians of the value of the vaccine in preventing death and disability in young children and their mothers. This system must be fully funded and resourced for the foreseeable future if the service is to be sustainable. Today the major obstacles to effective immunization of young children in developing countries is the inadequate, insecure and unpredic

[Medical Care for Homeless People - Individual Right and a Social Duty for an Inclusive Society].

Homelessness is a social phenomenon of increasing frequency in Germany and of high relevance for an inclusive society. Its expression is both caused psychosocially and moderated socioeconomically, often with negatively reinforcing feedback-loops. This overview describes health effects and strategies for improvement, using the example of the "Mainzer Modell". The comprehensive availability of adequate medical care are both an individual right and a social duty for an inclusive society.

Organizational inaction sustains destructive leadership in healthcare.

Destructive leadership is increasingly recognized as a distinct and harmful form of influence, yet the response of organizations to such leadership remains theoretically underdeveloped. Objective: This article develops a theory of organizational inaction in the face of destructive leadership and explains why organizations differ in their capacity to recognize and confront such conduct. Using a theory-driven, integrative conceptual-development approach, this Hypothesis and Theory article synthesizes research on destructive leadership, organizational silence, psychological safety, institutional

Psychosocial and Organizational Determinants of Perceived Occupational Exposure and Job Satisfaction Among Intensive Care Unit Nurses.

Introduction: Intensive care units are characterized by a high level of clinical and psychosocial demands. Previous research indicates that the work environment plays a significant role in shaping nurses' occupational well-being; however, analyses that simultaneously consider organizational factors, perceived occupational exposure, and job satisfaction are still limited, particularly in the context of Central and Eastern European countries. Objectives: The aim of the study was to assess the relationships between psychosocial and organizational factors in the work environment and the level of p

Organizational Context and Facilitation Interactions on Delirium Risk in Long-Term Care: A Cross-Sectional Study.

OBJECTIVES: Organizational context (eg, leadership) and facilitation (eg, coaching behaviors) are thought to interact and influence staff best practices in long-term care (LTC), including the management of delirium. Our objective was to assess if organizational context and facilitation-individually, and their interactions-were associated with delirium in LTC. DESIGN: Retrospective cross-sectional analysis of secondary data. SETTING AND PARTICIPANTS: We included 8755 residents from 281 care units in 86 LTC facilities in 3 Canadian provinces. METHODS: Delirium (present/absent) was assessed us

Challenges to Caring for Injured Children With Special Health Care Needs in Emergency Settings.

BACKGROUND AND OBJECTIVES: Injured children and youth with special health care needs (CYSHCN) can present competing priorities for prehospital and hospital care. Clinicians must balance the potential impact of chronic conditions-such as attention-deficit/hyperactivity disorder, autism, diabetes, or depression-with concerns about acute injury treatment for CYSHCN. The objective of this study is to identify the organizational and macro-level challenges and opportunities that clinicians face when caring for injured CYSHCN in acute care settings. METHODS: We conducted qualitative interviews (N =

The mediating role of organizational justice in the relationship between cost consciousness and resource efficiency among human service professionals.

BACKGROUND: Organizational psychology research suggests that employees' perceptions of workplace practices play an important role in shaping organizational outcomes. In resource-constrained Human Service Organizations (HSOs), organizational justice may represent a key psychological mechanism linking cost-conscious practices to resource-efficient behaviors. OBJECTIVE: This study examined whether organizational justice explains the association between cost consciousness and resource efficiency among professionals working in Human Service Organizations (HSOs). METHODS: A cross-sectional survey

The relationship between role stress, psychological empowerment and organizational silence of psychiatric nurses: a variable-centered and individual-centered analysis.

BACKGROUND: In psychiatric nursing, organizational silence behavior seriously affects patient safety and nursing quality. Role stress is an important predictor of organizational silence, and psychological empowerment can alleviate role stress and reduce silence behavior. However, the mediating relationship among these three variables and group heterogeneity remain unclear. OBJECTIVE: To explore the mediating role of psychological empowerment between role stress and organizational silence, and to identify different latent classes of the "role stress-psychological empowerment" dyad using latent

Strategies for a risky business: How drug dealers manage customers, suppliers and competitors in Italy, Slovenia and Germany.

BACKGROUND: A growing body of literature aims to improve understanding of the operations of drug trafficking markets through conducting interviews with dealers and traffickers. Insight into how these individuals conduct business can provide evidence to inform the efforts by policy makers, law enforcement and practitioners to disrupt illicit markets. This paper aims to make a contribution to this evidence base by extending the number of European countries in which interviews have been conducted with incarcerated drug dealers and traffickers. METHODS: It draws on interviews with 135 men convict

Perceived organizational support for safety and employee safety voice: the mediating role of coworker support for safety.

In the present study, we modeled 2 sources of safety support (perceived organizational support for safety and perceived coworker support for safety) as predictors of employee safety voice, that is, speaking out in an attempt to change unsafe working conditions. Drawing on social exchange and social impact theories, we hypothesized and tested a mediated model predicting employee safety voice using a cross-sectional survey of urban bus drivers (n = 213) in the United Kingdom. Hierarchical regression analysis showed that perceived coworker support for safety fully mediated the relationship betwee
