Risk factors for postoperative infection after non-elective femur fracture fixation in pediatric patients.

BACKGROUND: Pediatric femoral shaft fractures are common and often require surgical fixation. Postoperative infections remain a serious complication, yet risk factors specific to this population are not well defined. METHODS: We analyzed 14,037 pediatric patients undergoing non-elective femur fracture fixation in the 2019 and 2022 Kids' Inpatient Database (KID). Multivariable logistic regression identified independent predictors of postoperative soft tissue, device-related, and overall infection. Secondary outcomes included length of stay (LOS) and hospital charges. RESULTS: Infections occur

A New Stem Guide and a Tuberosity Anchoring "Lasso" System for Shoulder Hemiarthroplasty in the Treatment of Complex Proximal Humerus Fractures.

Hemiarthroplasty implantation for proximal humerus fracture is challenging. In case of young demanding patients with functional rotator cuff, humeral head replacement is an option if open reduction internal fixation is not a viable solution. The criteria for success in terms of clinical outcomes and longevity are anatomic healing of the tuberosities and stem positioning (restoring correct height, version, and axis). The purpose of this article is to describe a technique including a new stem guide and a tuberosity anchor "lasso" system for the implantation of hemiarthroplasty in complex proxima

Potential errors in axial alignment using intramedullary instrumentation for total knee arthroplasty.

Accurate restoration of normal limb alignment is crucial for the long-term survivorship of total knee arthroplasty (TKA). A mathematical model was used to evaluate the maximum error in varus and valgus alignment that could occur when cutting the tibia or femur during TKA using intramedullary (IM) guides of varying length and diameter. Minor deviations in the insertion point of IM instrumentation during TKA can result in malalignment of several degrees. This error can be minimized by careful attention to the entry point of the IM instrumentation or by increasing the IM rod diameter and length u

[Surgical stabilization of juvenile lower leg fractures using the external fixator. Retrospective analysis of 10 cases].

Ten cases seen over the last eight years were reviewed by clinical and radiological re-examination one year after injury. The results encourage us to continue treating young patients with critical soft-tissue injury, compartmental syndrome, open fracture and multiple injury by this technique.

Distraction physiolysis in the rabbit.

Axial forces across the growth plate of the upper tibia during controlled distraction were studied in 24 rabbits close to skeletal maturity. Distraction rates of 0.13, 0.26, and 0.53 mm every 24 hours were applied through a dual-frame external fixator. Strain gauges were bonded to the fixator, and axial forces were estimated prior to and following distraction. The results demonstrate two distinct patterns. In one group, forces increased to maximum values of 20-32 newtons and then suddenly decreased on subsequent distraction. This force pattern indicated fracture of the growth plate with associ

Reduction and stabilization of grade IV spondylolisthesis.

Virtually any degree of spondylolisthesis can be reduced and held using variable screw placement (VSP) slotted plates and transpedicular screws. All of the soft tissue holding the displaced vertebrae must be stretched prior to pulling the vertebrae back into normal alignment. Complete control of the displaced vertebrae is gained through the transpedicular screws, and fixation is accomplished by bolting the screws to the spine plates that have been bent to achieve the proper lumbar lordosis. Ultimate stability is gained by both a bilateral lateral and interbody fusion.

The Ilizarov distractor for the correction of relapsed or neglected clubfoot.

The correction of a relapsed or neglected clubfoot by an external distractor is an alternative to a major operation which may involve triple arthrodesis and is often associated with skin problems. We report the use of the Ilizarov method to treat nine severely deformed feet, with satisfactory results in terms of function and appearance. The distractor enables treatment to be applied before maturity and avoids the shortening of the foot that results from wedge osteotomies. We discuss the indications, technique, complications and results of the method.

Anterior Cruciate Ligament Reconstruction: Clinical Outcomes and Biomechanical Considerations of Graft Selection-A Narrative Review.

Anterior cruciate ligament (ACL) reconstruction represents the current gold standard for restoring knee stability following ligament rupture, particularly in young and physically active individuals. The aim of this narrative review is to provide an updated overview of the biomechanical, biological, and clinical considerations underlying graft selection in ACL reconstruction, with particular emphasis on graft incorporation, fixation strategies, and postoperative outcomes. While surgical techniques and fixation devices have undergone substantial refinement, the optimal choice of graft remains a

Biodegradable Bone Implants as a New Hope to Reduce Device-Associated Infections-A Systematic Review.

Bone fractures often require fixation devices that frequently need to be surgically removed. These temporary implants and procedures leave the patient more prone to developing medical device-associated infections, and osteomyelitis associated with trauma is a challenging complication for orthopedists. In recent years, biodegradable materials have gained great importance as temporary medical implant devices, avoiding removal surgery. The purpose of this systematic review was to revise the literature regarding the use of biodegradable bone implants in fracture healing and its impact on the reduc

From Models to Implants: The Expanding Role of 3D Printing in Orthopedic Care.

For decades, orthopedic implants have been essential for improving mobility, functionality, soothing pain, and rebuilding complex skeletal structures. Old manufacturing methods, such as casting, forging, and machining, have resulted in long-lasting implants but could not adapt to the complex anatomical variability in patients' anatomy. Additive manufacturing (3D printing) overcomes this limitation by allowing for complex, patient-specific computer-aided design (CAD) models to be created and implants to be manufactured containing precise porous architectures that encourage osseointegration and

Conservative and surgical management of acute isolated syndesmotic injuries: ESSKA-AFAS consensus and guidelines.

PURPOSE: Correct management of syndesmotic injuries is mandatory to avoid scar tissue impingement, chronic instability, heterotopic ossification, or deformity of the ankle. The aim of the present study was to perform a systematic review of the current treatments of these injuries to identify the best non-surgical and surgical management for patients with acute isolated syndesmotic injuries. METHODS: A review of the literature was performed according to the PRISMA guidelines. A comprehensive search of PubMed, Medline, CINAHL, Cochrane, Embase, and Google Scholar databases was performed using t

Osteochondritis dissecans of the knee: a practical guide for surgeons.

UNLABELLED: Osteochondritis Dissecans (OCD) is a condition for which the aetiology remains unknown. It affects subchondral bone and secondarily its overlying cartilage and is mostly found in the knee. It can occur in adults, but is generally identified when growth remains, when it is referred to as juvenile OCD. As the condition progresses, the affected subchondral bone separates from adjacent healthy bone, and can lead to demarcation and separation of its associated articular cartilage. Any symptoms which arise relate to the stage of the disease. Early disease without separation of the lesion
