Identification of the skeletal progenitor cells forming osteophytes in osteoarthritis.

OBJECTIVES: Osteophytes are highly prevalent in osteoarthritis (OA) and are associated with pain and functional disability. These pathological outgrowths of cartilage and bone typically form at the junction of articular cartilage, periosteum and synovium. The aim of this study was to identify the cells forming osteophytes in OA. METHODS: Fluorescent genetic cell-labelling and tracing mouse models were induced with tamoxifen to switch on reporter expression, as appropriate, followed by surgery to induce destabilisation of the medial meniscus. Contributions of fluorescently labelled cells to os…

Surgical treatment of anterior cervical osteophytes causing dysphagia.

PURPOSE: Dysphagia due to anterior cervical osteophytes is a rare condition. However, it can become serious enough to permanently impair the quality of life up to making normal food intake impossible. If conservative treatment fails, there is the option of surgical resection of the osteophytes. The objective of this study was to assess the outcomes of resections of anterior cervical osteophytes causing spondylogenic dysphagia, taking literature into consideration. METHOD: Resection of anterior cervical osteophytes using a standard anterior approach was performed in 14 consecutive patients wit…

Surgical Management of Dysphagia Due to Anterior Cervical Osteophytes.

PURPOSE: The present study was performed to share personal experiences and to discuss the advantages and disadvantages of anterior cervical osteophytes surgical procedures. METHODS: The operative records of anterior cervical osteophytes patients who did not benefit from conventional treatments and underwent osteophytectomy were analyzed retrospectively. Transoral and transcervical anterolateral methods were applied, considering both the localization of the osteophyte and the patient's preference. RESULTS: Five patients were operated with the transcervical anterolateral method, and 3 patients…

Cervical osteophytes: a rare cause of obstructive sleep apnea.

Uncertain etiology of cervical osteophytes, in particular emerging in geriatric population, is a rare skeletal system disease. Often, the cases are asymptomatic and may lead to symptoms such as dysphagia, cough, dyspnea, and dysphonia. We present a patient who had anterior osteophytes causing symptoms of severe obstructive sleep apnea (OSA), and literature on etiology of OSA has been reviewed. A 57-year-old male patient with complaints of snoring and cessation of breathing during sleep was referred to the ear nose throat clinic. Cervical radiograph and computed tomography showed the osteophyte…

The development of dysphagia and dysphonia due to anterior cervical osteophytes.

Cervical osteophytes may cause dysphagia by compressing the esophagus and may cause dysphonia by compressing the larynx and inferior laryngeal nerve. The occurrence of dysphagia and dysphonia due to cervical osteophytes has rarely been reported in literature. In this article, a case, in which the multiple cervical osteophytes were found to be the cause of dysphagia and dysphonia, was evaluated by imaging methods and electrophysiological evaluation of swallowing and the case was discussed in the light of relevant literatures.

High signal in knee osteophytes is not associated with knee pain.

OBJECTIVE: Our understanding of the local source of pain in osteoarthritis (OA) remains unclear. We undertook this study to determine if the presence of high-signal osteophytes on magnetic resonance imaging (MRI) was associated with pain presence, location or severity. METHODS: Subjects were chosen from the Boston Osteoarthritis of the Knee Study, a natural history study of symptomatic knee OA. Assessments included knee MRI, pain assessments and information on weight and height. Osteophyte signal was defined as areas of increased signal intensity in the osteophyte on fat-suppressed T2 weighte…

The 'anvil' osteophyte-a primary cause of fixed flexion of the knee?

Three cases are reported in which clinical presentation was with arthritic symptoms and inability to straighten the knee. An unusually prominent anterior tibial osteophyte, whose shape is remarkably suggestive of a blacksmith's anvil, appeared to be a contributory factor through its apparent impingement with the femoral trochlea at the limit of extension. The osteophyte was radically removed arthroscopically in each case as an isolated gesture, and the patients were followed up with particular emphasis on fixed flexion deformity. At clinical review, after a mean follow-up period of 5.3 years, …

Retro-pharyngeal obstruction in association with osteophytes of the cervical spine.

We describe five patients with cervical spondylosis and large anterior osteophytes causing pharyngeal compression. All had dysphagia, two had obstructive sleep apnoea and another two had dyspnoea and stridor on inspiration. One, with perforation of the pharynx, required emergency tracheostomy. Only three had pain in the neck or arm. Compression of the retroglottic space was confirmed in all patients by pharyngoscopy and in all the symptoms were relieved by excision of the osteophytes. Three also underwent intervertebral fusion. One had some persistent sleep apnoea.

The origin(s) of the periarticular osteophytes of osteoarthritic knee joints.

Oestophites obtained from the margins of 25 osteoarthritic knee joints undergoing replacement have been investigated using a battery of tinctorial stains. The microarchitecture of the osteophyte appears to be identical to that of the normal articular cartilage. Other findings suggest that oesteophytes are derived from hyperplasia of the surface chondrocytes at the 'marginal transitional zone'.

Arthroscopic treatment of sports-related anterior osteophytes in the ankle.

Osteophytes on the anterior aspect of the tibia and anterior talus are common in athletes such as dancers, runners, and high jumpers who impact with quick and forceful dorsiflexion to this area. This pathology is often confirmed easily on lateral x-ray films. Excision, debridement, and/or abrasion arthroplasty can be performed arthroscopically to remove osteophytic bone. The joint space is easily approached and managed using small joint or even regular-sized arthroscopic instruments. Care is taken to reshape the anterior tibia and/or talus to its original contour, thus avoiding impingement of …

Traction osteophytes of the lumbar spine: radiographic-pathologic correlation.

Previous reports have emphasized two types of osteophytes on the anterior aspects of the lumbar vertebral bodies: the common claw osteophyte and the less common but more significant traction osteophyte, which is indicative of spinal instability. To delineate the importance of the traction osteophyte, a radiographic-pathologic study was conducted. The results indicate that claw osteophytes are more frequent than traction osteophytes, that both may coexist in a single vertebral body, and that, in most cases, these osteophytes appear to represent different stages of the same pathologic process.

Tonsillar Fullness Mimicking Malignancy Secondary to a Cervical Osteophyte: A Case Report.

Tonsillar asymmetry and firmness on examination often raise concern for underlying malignancy, particularly squamous cell carcinoma or lymphoma. However, rare benign etiologies may mimic these findings and lead to diagnostic uncertainty. We present the case of an 83-year-old female who presented with globus sensation and was found to have left-sided tonsillar fullness with firmness on physical exam, raising concern for malignancy. Flexible laryngoscopy was unremarkable. Computed tomography of the neck demonstrated a prominent anterior cervical osteophyte at the C1-C2 level, causing mass effect…

Cervical osteophyte complex causing compressive myelopathy leading to a diagnosis of acromegaly.

A young male patient presented with acute onset spastic quadriparesis. Clinically, he exhibited features of acromegaly alongside spastic quadriparesis. Neuroimaging revealed an osteophyte complex causing cord compression and canal stenosis. Biochemical and radiological assessments confirmed a growth hormone-secreting pituitary macroadenoma. He underwent anterior cervical discectomy with vertebral fusion of the cervical cord to relieve his neurological symptoms. His acromegaly was subsequently managed with surgery, followed by radiotherapy and medical therapy. While acromegaly may rarely presen…

Psoas Muscle Impingement by a Lumbar Disc-Osteophyte Complex: A Case Report.

Psoas tendon impingement is not a frequently encountered condition, but impingement at the muscular level is not reported in the literature. The term refers to the mechanical impingement of the psoas muscle with secondary myositis. We report a case of psoas muscle impingement by a lumbar disc-osteophyte complex. This study reports on a 61-year-old female who presented to our facility complaining of severe low back pain with increased intensity in the past two weeks. Radiographic imaging and magnetic resonance imaging revealed psoas muscle impingement by lumbar disc-osteophyte. No signs of nerv…

Diet influences knee osteoarthritis osteophyte formation via gut microbiota and serum metabolites.

Osteophyte formation, a key indicator of osteoarthritis (OA) severity, remains poorly understood in its relation to gut microbiota and metabolites in knee osteoarthritis (KOA). We conducted 16S rDNA sequencing and untargeted metabolomics on fecal and serum samples from 20 healthy volunteers, 80 KOA patients in Guangdong, and 100 in Inner Mongolia, respectively. Through bioinformatics analysis, we identified 3 genera and 5 serum metabolites associated with KOA osteophyte formation. Blautia abundance negatively correlated with meat, cheese, and bean consumption. The 5 serum metabolites negativel…
