Magnetic resonance imaging of the shoulder in children with brachial pLexus birth palsy.

Five patients suffering from Erb-Duchenne brachial plexus birth palsy were prospectively studied with MRI. A group of 11 healthy children was used as a control to understand the MRI anatomy of the normal growing glenohumeral joint. A hypoplastic and flattened posterior part of the glenoid fossa and a blunt posterior labrum were found in all patients. Four patients had a blunt anterior labrum and a flattened humeral head. Three patients presented with a posterior subluxation of the humeral head. These results suggest that MRI provides a non-ionising and non-invasive method of demonstrating the

Comparing McRoberts' and Rubin's maneuvers for initial management of shoulder dystocia: an objective evaluation.

OBJECTIVE: This study was undertaken to objectively compare delivery traction force, fetal neck rotation, and brachial plexus elongation after 3 different initial shoulder dystocia maneuvers: McRoberts', anterior Rubin's, and posterior Rubin's. STUDY DESIGN: We developed a laboratory birthing simulator comprised of a maternal model with a 3-dimensional bony pelvis, an instrumented fetal model, a force-sensing glove, and a computer-based data acquisition system. A single operator performed 30 simulated shoulder dystocia deliveries using standard downward traction after 1 maneuver was performed

Anaesthetic management of labour and caesarean delivery of a patient with hyperkalaemic periodic paralysis.

We describe a parturient with hyperkalaemic periodic paralysis who presented for induction of labour and subsequently, caesarean section. Epidural analgesia and anaesthesia were used successfully in a multidisciplinary plan aimed at avoiding a peripartum attack and providing safe delivery. Management of this rare condition is discussed along with a review of the available literature.

The role of medico-legal reviews in medical research.

Serial malpractice reviews offer an opportunity for the study of the effects of various practice patterns. In the specialty of obstetrics, the usefulness of this approach has been demonstrated repeatedly: (a) Much of the data indicating that bromocriptine, when given postpartum for milk suppression, has life endangering propensities have derived from medico-legal reviews. (b) Important new information about factors predisposing to or associated with neonatal brachial plexus injuries have emerged from critical analysis of malpractice claims. There is reason to anticipate that objective evaluati

Brachial plexus palsy: an old problem revisited again. II. Cases in point.

OBJECTIVES: In spite of mounting evidence to the contrary, plaintiffs' expert witnesses continue to maintain that brachial plexus impairment is almost always the result of excessive lateral traction on the head during the last phase of delivery. Case studies are presented to challenge this concept. STUDY DESIGN: Examples encountered in medicolegal consultations were analyzed with this purpose as our focus. RESULTS: Cases of brachial plexus impairment were encountered in which there was no evidence of shoulder dystocia or extreme lateral traction on the fetal head. In one in which shoulder dy

Accidental total spinal block: a complication of an epidural test dose.

A case is presented of a 36-yr-old parturient who developed a total spinal block after an epidural test dose. After placement of an epidural catheter and confirming negative aspiration for blood or CSF, 3 ml lidocaine 1.5% (45 mg), with 1:200,000 epinephrine (15 micrograms) was injected via the catheter over 30 sec. Within two minutes the patient developed hypotension and extensive sensory and motor block including respiratory paralysis and aphonia. She remained fully conscious and alert and spontaneous respiration recommenced in five minutes. A live healthy infant was delivered by emergency C

Diaphragmatic paralysis after phrenic nerve injury in newborns.

BACKGROUND: Phrenic nerve injury (PNI) from birth trauma is a recognized phenomenon, generally occurring with ipsilateral brachial plexus palsy (BPP). In severe cases, PNI results in diaphragm paresis (DP) and respiratory insufficiency. Surgical diaphragmatic plication (SDP) is a potential management strategy for patients with PNI and DP, but timing and outcomes associated with SDP have not been rigorously studied. METHODS: Records from 49 tertiary United States pediatric hospitals in the Pediatric Health Information System from 2004 to 2018 were analyzed. The study cohort included patients d

[Antepartum brain injuries].

It is generally accepted that congenital brain lesions and especially cerebral paralysis are produced nearly always during the process of delivery. The origin can be hypoxia or obstetrical trauma. This has been true until the 70s, but from that period onwards, the incidence due to the delivery process has been reduced so dramatically, that its appearance can be considered as an accident, as in the umbilical cord prolapsus or during abruptio placentae. More than 80% of the cerebral paralysis occur before delivery due to a variety of causes that are detailed in this report and only the resting 2

Anaesthesia challenges of a parturient with paramyotonia congenita and terminal filum lipoma presenting for labour and caesarean section under epidural anaesthesia - a case report.

BACKGROUND: Paramyotonia congenita is a rare autosomal dominant myopathy which presents with periodic weakness due to cold and exercise. It is caused by mutations of the SCN4 gene which encodes the sodium channel in skeletal muscles. CASE PRESENTATION: We report a full term obstetric patient with both paramyotonia congenita and terminal filum lipoma who presents for induction of labour followed by an emergency caesarean section performed under epidural anesthesia. Her recovery is subsequently complicated by a 3-day history of postpartum paraparesis attributed to hypokalemic periodic paralysis

[Laryngopharyngeal morbidity following general anaesthesia. Anaesthesiological and laryngological aspects].

Laryngeal and pharyngeal complaints are among the subjective problems most frequently reported by patients after general anaesthesia involving endotracheal intubation, others being pain, nausea and vomiting. Hoarseness, sore throat, and vocal cord injuries restrict patients' social lives, and in some cases also their working lives. The most frequent types of laryngeal injury are swollen mucosa and haematoma of the vocal cords. Vocal cord paralysis occurs much less frequently. Knowledge of the pathophysiological aspects and other relevant factors associated with laryngopharyngeal morbidity are

Guillain-Barré Syndrome after H1N1 Shot in Pregnancy: Maternal and Fetal Care in the Third Trimester-Case Report.

Guillain-Barré syndrome is a rare neurological disease of progressive installation, usually following a previous acute infectious state, has a rare incidence, especially in pregnancy, and can induce major complications and high mortality risk. Its occurrence, after immunization to influenza during the last trimester pregnancy, has not been reported before. We presented a case of a 36-year-old pregnant woman that was immunized to H1N1 in the last trimester; 10 days later she developed shoulder and lumbar spine's pain, limbs weakness and facial paralysis with unfavorable clinical evolution and w

Pathogenesis and management of deformities of the elbow, wrist, and hand in late neonatal brachial plexus palsy.

The incidence of neonatal brachial plexus palsy (NBPP) has remained relatively stable despite awareness of the problem and improved obstetric techniques. Deformities of the forearm and hand can result either from untreated NBPP or following early microsurgical nerve reconstruction. These deformities include limb length discrepancy, flexion contracture of the elbow, supination or pronation contractures of the forearm, ulnar deviation of the wrist, and varying types of finger paralysis. The treatment options for these deformities consist of soft tissue releases, corrective osteotomies, tendon tr

Shoulder function following primary axillary nerve reconstruction in obstetrical brachial plexus patients.

BACKGROUND: In obstetrical brachial plexus palsy, suprascapular nerve reinnervation is a priority. For the most favorable outcomes in shoulder function, it is the authors' policy to also reconstruct the axillary nerve with intraplexus donors to the posterior cord (early cases) or directly with intraplexus or extraplexus motor donors (late cases). METHODS: Between 1979 and 2003, 80 consecutive patients (82 brachial plexuses) underwent plexus exploration and nerve reconstruction for obstetrical palsy. Axillary nerve reconstruction was performed in 60 plexuses, and evaluation of the results was

The natural history of obstetrical brachial plexus palsy.

Obstetrical brachial plexus palsy remains an unfortunate consequence of difficult childbirth. Sixty-six such patients were reviewed. Included were 28 patients (42 percent) with upper plexus involvement and 38 (58 percent) with total plexopathy. The natural history of spontaneous recovery in all of these patients has been determined using an appropriate grading mechanism. Sixty-one patients (92 percent) recovered spontaneously and five patients (8 percent) required primary brachial plexus exploration and reconstruction (median age 12 months), demonstrating that most patients do well. Additional

The towel test: a useful technique for the clinical and electromyographic evaluation of obstetric brachial plexus palsy.

The towel test consists of covering an infant's face with a towel and seeing if he/she can remove it with either arm. In this study it has been used to aid the clinical and electromyographic assessment of 21 infants with obstetric brachial plexus palsy. At 2 to 3 months, none of the 21 infants succeeded in removing the towel, either with their normal or affected arm. At 6 months, all the infants succeeded in removing the towel with their normal arm, but 11 could not with their affected arm, and the same was observed at a further assessment at 9 months. The towel test is a reliable technique fo
