Hyperfunctioning carcinoma of the parathyroid gland.

Carcinoma of the parathyroid gland is rare, constituting about 1% of most series of patients with hyperparathyroidism. In 1969 a collective review identified forty-six unequivocal cases, and since then a further seven cases have been described in the literature. A brief review of the condition is presented, and one further case of hyperfunctioning adenocarcinoma of the parathyroid gland is reported in detail. Primary treatment is discussed, together with postoperative biochemical problems, and the therapeutic manoeuvres available to deal with recurrences are presented.

The fine structure of the parathyroid gland.

The fine structure of the parathyroid of the macaque is described, and is correlated with classical parathyroid cytology as seen in the light microscope. The two parenchymal cell types, the chief cells and the oxyphil cells, have been recognized in electron micrographs. The chief cells contain within their cytoplasm mitochondria, endoplasmic reticulum, and Golgi bodies similar to those found in other endocrine tissues as well as frequent PAS-positive granules. The juxtanuclear body of the light microscopists is identified with stacks of parallel lamellar elements of the endoplasmic reticulum o

The inferior parathyroid glands preserved in site recover faster than the superior parathyroid glands preserved in site after thyroid surgery for carcinoma.

Due to the great difficulty in being preserved in site for the variable positions, the inferior parathyroid glands were advised to being routinely autotransplanted to prevent permanent hypoparathyroidism. The aim of this study was to compare the performance in the function of the superior parathyroid glands preserved in site with that of the inferior parathyroid glands preserved in site.We conducted a retrospective study including patients who underwent thyroid surgery for papillary thyroid carcinoma at our department between January 2014 and June 2018. According to the number and original pos…

Ultrasound detection of normal parathyroid glands: a preliminary study.

OBJECTIVE: No previous prospective study has investigated the detection of normal parathyroid glands (PTGs) and their features using real-time ultrasound (US). This study aimed to assess the preoperative US detection of normal PTGs in patients who underwent hemithyroidectomy. METHODS: Between August and October 2016, 44 patients underwent hemithyroidectomy using a low-collar incision, and 5 were excluded from the study. A single radiologist performed the preoperative US examination in all patients, and the surgical data for the PTGs were obtained by a single surgeon. Based on the surgical fin…

Location of abnormal parathyroid glands: lessons from 810 parathyroidectomies.

BACKGROUND: Primary hyperparathyroidism (pHPT) is commonly treated with targeted parathyroidectomy (PTX) guided by preoperative imaging and intraoperative parathormone monitoring. Despite advanced imaging techniques, failure of parathyroid localization still occurs. This study determines the anatomical distribution of single abnormal parathyroid glands, which may help direct the surgeon in PTX when preoperative localization is unsuccessful. METHODS: A retrospective review of prospectively collected data of 810 patients with pHPT who underwent initial PTX at a tertiary medical center was perfo…

Intraoperative optical coherence tomography imaging to identify parathyroid glands.

OBJECTIVE: Optical coherence tomography (OCT) is a non-invasive high-resolution imaging technique that permits characterization of microarchitectural features in real time. Previous ex vivo studies have shown that the technique is capable of distinguishing between parathyroid tissue, thyroid tissue, lymph nodes, and adipose tissue. The purpose of this study was to evaluate the practicality of OCT during open and minimally invasive parathyroid and thyroid surgery. METHODS: During parathyroid and thyroid surgery, OCT images were generated from parathyroid glands, thyroid tissue, lymph nodes, an…

Near-infrared autofluorescence for the detection of parathyroid glands.

A major challenge in endocrine surgery is the intraoperative detection of parathyroid glands during both thyroidectomies and parathyroidectomies. Current localization techniques such as ultrasound and sestamibi scan are mostly preoperative and rely on an abnormal parathyroid for its detection. In this paper, we present near-infrared (NIR) autofluorescence as a nonintrusive, real-time, automated in vivo method for the detection of the parathyroid gland. A pilot in vivo study was conducted to assess the ability of NIR fluorescence to identify parathyroid glands during thyroid and parathyroidecto…

Anatomy of thyroid and parathyroid glands and neurovascular relations.

Historically, thyroid surgery has been fraught with complications. Injury to the recurrent laryngeal nerve, superior laryngeal nerve, or the parathyroid glands may result in profound life-long consequences for the patient. To minimize the morbidity of the operation, a surgeon must have an in-depth understanding of the anatomy of the thyroid and parathyroid glands and be able to apply this information to perform a safe and effective operation. This article will review the pertinent anatomy and embryology of the thyroid and parathyroid glands and the critical structures that lie in their proximi…

Contact endoscopy for identifying the parathyroid glands during thyroidectomy.

Aim of this study was to analyse contact endoscopy as an auxiliary method for identifying parathyroid glands during thyroid surgery and to identify other variables that may interfere with this correlation. Overall, 125 patients underwent thyroid surgery between January 2004 and February 2006. The variables analysed were: the total duration of surgery; time taken to locate and identify parathyroid glands; improvement in identifying these; numbers of parathyroid glands located by the surgeon and confirmed by contact endoscopy; histopathological diagnosis; presence of thyroiditis; thyroid weight;…

Multimodality imaging of the parathyroid glands in primary hyperparathyroidism.

Primary hyperparathyroidism is a common endocrine disorder, affecting approximately 1 in 500 women and 1 in 2,000 men. Surgical removal of the hyperfunctioning parathyroid gland is the primary curative treatment. The last decade has witnessed the development of minimally invasive parathyroidectomy, which is based on the fact that the vast majority of cases are caused by single adenomas. However, the success of this technique relies on accurate preoperative localisation of the parathyroid lesions. The imaging modalities used vary at different institutions according to local expertise and availa
