BACKGROUND: Patellar fractures represent approximately 0.5% to 1.5% of all trauma-related fractures, and patella baja, or an abnormally low-lying patella, may be a result of treatment. This complication is underappreciated, and although patella baja may result in patient discomfort, stiffness, and later degenerative changes, the association between fracture type, operative treatment, and this complication is not well described.
QUESTIONS/PURPOSES: (1) What percentage of patients treated surgically for patellar fractures developed patella baja, and which fracture patterns were more likely to d

OBJECTIVES: Altered patellar height following patella fracture repair may affect clinical outcomes, but the significance remains unclear. The purpose of this study was to evaluate the impact of immediate and final follow-up Insall-Salvati Ratio (ISR) on postoperative outcomes following patella fracture fixation.
METHODS: A retrospective review at a multi-site academic urban hospital system was conducted. All patients underwent patella open reduction and internal fixation (ORIF) for displaced patella fracture (OTA 34) from 2012 to 2024. Final intraoperative and final follow-up radiographs were…

INTRODUCTION: Patellar fractures constitute almost 1% of total skeletal injuries. Existing common operative methods include the use of different types of Tension band wiring techniques like cannulated screws and K-wires. The Patellar plating construct is a relatively newer operative method for patellar fracture fixation with promising outcomes with fewer complications.
METHOD: We conducted a pilot prospective study at our Apex Trauma center in twenty patients aged 18-65years with displaced patellar fractures, AO type A2, A3, B2, B3, C1, C2, C3. Fractures were fixed with low profile patellar p

PURPOSE: Tension band wiring remains a common treatment for patella fractures, but complication rates are high, with unsatisfactory results. The purpose of this observation study was to evaluate clinical results and complication rates of a novel patella locking plate fixation.
METHODS: Twenty patients (mean age, 59.2 ± 18 years) with displaced patella fractures were prospectively enrolled. Range of motion, knee scores (Tegner, Lysholm, Kujala), complications, and revision surgeries were assessed six weeks, six months, 12 months, and 24 months after surgery. Results were compared to the situat…

BACKGROUND: The cable pin system is an effective device for fixation of transverse patella fractures. However, whether this device provides superior results using a minimally invasive technique instead of conventional open surgery using the K wire tension band method is unclear.
QUESTIONS/PURPOSES: We asked whether a minimally invasive technique would be associated with (1) increased operative time; (2) reduced postoperative pain; (3) faster recovery of ROM; (4) higher knee scores; and (5) reduced complications.
METHODS: Forty patients with displaced transverse fractures of the patella parti…

The results in thirty-six knees that had a fracture of the patella after a total condylar arthroplasty were reviewed, and were analyzed according to the type of fracture and the alignment of the implant and the limb. Most of the fractures occurred two years or less after the initial operation. Fourteen knees were rated fair or poor and twenty-two, good or excellent. None of the thirty-six implants had been aligned in the neutral range. In sixteen knees, the alignment had a minor variation from the neutral range--that is, the knees were in minor malalignment. In general, these knees had the lea

We have studied the long-term effect of comminuted fractures of the patella with particular reference to the development of degenerative changes in the tibiofemoral joint. Degenerative changes which could convincingly be ascribed to the original injury were found in only one case, but we stress that the numbers are small, and that carefull search sould be made at operation for damage to the femoral condyles.

BACKGROUND: Sleeve fracture of the patella is an unusual fracture, almost unique to children. The majority of sleeve fractures involve the inferior patellar pole. However, sleeve fractures of the superior pole of the patella are extremely rare in adults.
CASE SUMMARY: An 18-year-old male patient fell while running in the morning. The patient had tenderness to palpation at the superior pole of the patella, with a palpable gap over the upper part of the patella in both knees. We applied two 4.5-mm suture anchors with the Krackow stitch to repair the sleeve fracture, augmented by autogenous grac

BACKGROUND: Patella fractures that require surgery are conventionally treated using Kirschner wires (K-wires) and stainless steel wires. In recent years, the nonabsorbable polyester has been reported to have excellent outcomes clinically. Therefore, the goal of our study was to evaluate the effects of Kirschner wires combined with 5-Ethibond on treating patellar fractures.
METHODS: From July 2018 to January 2022, 22 patella fracture patients were treated with Kirschner wires combined with 5-Ethibond. Radiographs of the knees were used to evaluate fracture healing and hardware complications. T…

INTRODUCTION: Osteochondral fracture of the patella is a fairly common pathology, but almost always associated with a spectrum of soft tissue injuries including anterior cruciate ligament (ACL) rupture. We present a rare case of an osteochondral fracture of the patella in the absence of ligament or soft tissue injuries and with no dislocation of the patella in a pediatric patient.
PRESENTATION OF CASE: An 11-year-old male presented to the orthopedic clinic on crutches following a football injury. The patient had pain in his left knee with flexion deformity. Plain film radiography of the left

In the article is presented the improved method of knee joint extension system restoration by means of blocking wire loop, passed through the formed canals: in tibia--one cross canal and in patella--two longitudinal canals. This method has been applied in 6 patients with injuries of proper ligament of patella and in 18--with patella avulsive fractures. Long-term results have been followed with 12 patients. 11 of them achieved complete restoration of knee joint mobility irrespective of the injury nature.

INTRODUCTION: The commonly performed procedure for treating severe hallux valgus is proximal metatarsal osteotomy or first tarsometatarsal arthrodesis combined with a soft tissue procedure in which the severe intermetatarsal angle (IMA) is corrected using proximal metatarsal osteotomy or first tarsometatarsal arthrodesis; although a severe hallux valgus angle (HVA) can be corrected using the soft tissue procedure alone, the correction ability is low. Therefore, the more severe the hallux valgus is, the more difficult it is to correct.

This is a rare case report of medial femoral condyle fracture with irreducible incarcerated patella dislocation. Following the literature search, only a few cases have been reported in the literature. In this case, the patient had undergone open reduction and screw fixation of the femoral condyle, augmented with a distal femoral locking plate (LCP). Postoperatively patient was immobilised with an above knee backslab for a month. After removing the backslab, physiotherapy was commenced to improve the range of motion and strengthening the quadriceps muscle. After 18 months of follow-up, the pati

CASE: A healthy 15-year-old girl presented with osteochondral fracture at the posterior aspect of the lateral femoral condyle (LFC) associated with a right patellar dislocation after a noncontact injury. The patient remained asymptomatic 18 months after the arthroscopically assisted reduction and internal fixation of the osteochondral fracture using bioabsorbable pins and was able to eventually resume her usual activities of daily living.
CONCLUSIONS: This is the first report of an osteochondral fracture at the posterior aspect of the LFC after an acute patellar dislocation, successfully trea

Breakage of tension band wires, used to treat the patella fracture, is not uncommon several years after the fracture fixation. Broken wires may migrate to surrounding neurovascular structures, other vital organs like heart and may cause potentially fatal complications. Once the wires have been broken, it is very difficult to remove the broken pieces of metal wires. We report a 50 years old male patient with broken tension band wires at multiple sites for patella fracture. The broken wires were removed 20 years after the initial surgery without any undue complications, however patient sustained…

Patella fracture is either an infrequent or underreported complication after anterior cruciate ligament (ACL) reconstruction using a portion of the patellar ligament. Patella fracture pattern has been related to the mechanism of injury. Direct fractures resulting from impaction forces delivered to the patella are typically stellate or comminuted. Indirect fractures resulting from tensile stresses applied across the extensor mechanism typically are displaced transverse disruptions. After ACL reconstruction using a portion of the patellar ligament, Y-shaped stellate fractures can occur. Fracture…

We present a rare case of open lateral condyle Hoffa's fracture with concurrent patellar intra-articular dislocation, quadriceps rupture and posterior cruciate ligament (PCL) avulsion. The early adolescent male sustained these injuries in a road traffic accident. Diagnostic evaluation and a multidisciplinary approach guided treatment decisions. The patient underwent single-stage open reduction and internal fixation for the lateral condyle Hoffa's fracture and quadriceps tendon repair, while conservative management was chosen for the PCL avulsion. At the 1-year follow-up, the patient exhibited
