Individual and socioeconomic factors associated with childhood immunization coverage in Nigeria.

INTRODUCTION: Immunization is the world's most successful and cost-effective public health intervention as it prevents over 2 million deaths annually. However, over 2 million deaths still occur yearly from Vaccine preventable diseases, the majority of which occur in sub-Saharan Africa. Nigeria is a major contributor of global childhood deaths from VPDs. Till date, Nigeria still has wild polio virus in circulation. The objective of this study was to identify the individual and socioeconomic factors associated with immunization coverage in Nigeria through a secondary dataset analysis of Nigeria

Postmortem sperm retrieval: the effect of instituting guidelines.

PURPOSE: Postmortem sperm retrieval (PMSR) raises serious medical and ethical concerns. In this study we report the effect of intra-institutional guidelines developed for the permissibility of the procedure on the number of procurement procedures performed. MATERIALS AND METHODS: The family members of 22 men who died suddenly sought PMSR. We performed an institutional review board approved chart review of the requests for PMSR from 1994 to 2002. A set of guidelines addressing PMSR was developed by a panel of experts at our institution. Key elements included 1) evidence of intended paternity f

Biallelic truncating TTN variants in M-band encoding exons cause a fetal lethal titinopathy.

To report two novel TTN variants associated with fetal recessive titinopathy, thereby broadening the range of TTN variants that can lead to titinopathy. Clinical information on the fetus and parents was gathered, and genomic DNAs were extracted from the fetal tissue and family members' peripheral blood samples. Exome sequencing on fetal DNA was performed and following bioinformatics analysis, the suspected pathogenic variants were confirmed through Sanger sequencing. Prenatal ultrasound performed at 29 weeks of gestation revealed hydrops fetalis, decreased fetal movements, multiple joint contr

Parental loss at age birth to 21 years and daughters' breast cancer and tumor characteristics.

BACKGROUND: Adverse events in childhood are linked to cancer risk across the life course, but evidence is lacking regarding parental death during childhood and breast cancer characteristics. We investigated whether women who experienced parental loss in childhood have a higher incidence of breast cancer and are at higher risk of aggressive disease. METHODS: The Child Health and Development Studies (CHDS) consists of more than 15 000 families that enrolled during mothers' pregnancies between 1959 and 1967; family members were followed for cancer incidence and cause-specific mortality. We const

Validation of self-reported family history of myocardial infarction using nationwide health care data.

UNLABELLED: Self-reported family history of myocardial infarction (MI) is frequently used to assess cardiovascular risk. However, validation of self-reported family history has often been limited to small samples, and to death-certificates or self-reported medical history as reference. This study investigated the accuracy of self-reported family history of MI in a population-based cohort, using register data as reference. We included 25,302 participants from the Swedish CArdioPulmonary bioImage Study (SCAPIS) with sufficient register coverage. Self-reported family history of MI in parents and

Injury related infant death: the impact of race and birth weight.

OBJECTIVE: To examine the effect of race and birth weight independent of other sociodemographic factors on injury related infant death using national data. SETTING: Infants born in the United States to mothers who were white (non-Hispanic), African American, Mexican American, and Native American. METHODS: Linked infant birth and death data from the National Center for Health Statistics for 1989-91 were analyzed to calculate unadjusted and adjusted odds ratios for death due to homicide or unintentional injury within the first year of life. In addition to maternal race and birth weight, the ri

Genetic testing in the management of inherited cardiac disorders: two cases of Filamin-C arrhythmogenic left ventricular cardiomyopathy.

BACKGROUND: Arrhythmogenic left ventricular cardiomyopathy (ALVC) is a left ventricle-dominant arrhythmogenic cardiomyopathy (ACM) subtype often associated with malignant ventricular arrhythmias, left ventricular (LV) scar and sudden cardiac death. Awareness about LV involvement is now on the rise. The diagnosis relies on structural abnormalities on cardiac magnetic resonance (CMR) imaging and known ACM-causing genetic mutations. CASE SUMMARY: A 28-year-old lady (Case 1) was referred for cardiac screening after her father passed away suddenly. Her paternal uncle (Case 2) had been diagnosed wi

Multicystic dysplastic kidney: a new association of Wolcott-Rallison syndrome.

UNLABELLED: Wolcott-Rallison syndrome (WRS) is a rare autosomal recessive disorder due to mutations in the EIF2AK3 gene. It is characterized by permanent neonatal diabetes mellitus, skeletal dysplasia, liver impairment, neutropenia and renal dysfunction. Liver is the most commonly affected organ and liver failure is the commonest cause of death in this syndrome. The EIF2AK3 gene encodes a transmembrane protein PERK, which is important for the cellular response to endoplasmic reticulum (ER) stress. The absence of PERK activity reduces the ER's abilities to deal with stress, leading to cell deat

Widowers' Grief Experiences and Perceptions of Prolonged Grief Disorder: A Qualitative Study on Male Mental Health in Taiwan.

The death of a spouse is a major life stressor for men, yet cultural and gender norms shape how grief is experienced and expressed. This qualitative study explored widowers' grief experiences and perceptions of prolonged grief disorder (PGD) in Taiwan, focusing on implications for male mental health. Eighteen widowers were recruited from five outpatient clinics and interviewed using a semi-structured guide, and data were analyzed using a phenomenological approach based on Giorgi's method. Four themes emerged: grief subordinated to paternal and familial responsibilities, with men prioritizing c

Balanced reciprocal translocation at amniocentesis: cytogenetic detection and implications for genetic counseling.

Balanced translocation is a common structural chromosomal rearrangement in humans. Carriers can be phenotypically normal but have an increased risk of pregnancy loss, fetal death, and the transmission of chromosomal abnormalities to their offspring. Existing prenatal screening technologies and diagnostic procedures fail to detect balanced translocation, so genetic counseling for carriers remains a challenge. Here, we report the characteristics of chromosomal reciprocal translocation in 3807 amniocentesis cases. Of the 16 detected cases of fetal reciprocal translocation, 8 cases (50%) showed po

Sacrifice, the Bush Way: From Self to Others.

The Walker Bush dynasty has marked the last American century, promoting "corporate democracy" as a means to expand its wealth. As 43rd President of the United States, George Walker Bush's biography illustrates how the members of our powerful elite sacrifice the inner self of their own children for the sake of political success. In his case, the childrearing violence and emotional neglect he experienced created the psychological basis for his later re-enactments as commander-in-chief in the wake of 9/11. From that standpoint, his intergenerational legacy of trauma bears strong affinities with t

Disorders of Sleep and Ventilatory Control in Prader-Willi Syndrome.

Prader-Willi syndrome (PWS) is an imprinted genetic disorder conferred by loss of paternal gene expression from chromosome 15q11.2-q13. Individuals with PWS have impairments in ventilatory control and are predisposed toward sleep disordered breathing due to a combination of characteristic craniofacial features, obesity, hypotonia, and hypothalamic dysfunction. Children with PWS progress from failure to thrive during infancy to hyperphagia and morbid obesity during later childhood and onward. Similarly, the phenotype of sleep disordered breathing in PWS patients also evolves over time from pred

First Brazilian birth after Preimplantation Genetic Testing for Severe Methylmalonic Acidemia: preventing sudden infant death.

Methylmalonic acidemia (MMA) is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the MMUT gene, often associated with severe neonatal manifestations and sudden unexpected death in infancy (SUDI). Preimplantation Genetic Testing for Monogenic Diseases (PGT-M) offers a valuable reproductive option for at-risk couples seeking to prevent transmission of such conditions. This represents the first Brazilian case of a healthy birth following combined PGT-M and PGT-A for MMA known. After the neonatal loss of their first child due to mut0-type MMA, a couple underwent genet

A novel missense mutation in tropomyosin 1 gene associated with hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a common genetic heart disorder that can lead to heart failure or sudden death. Family-based identification of rare sarcomeric variants can support molecular diagnosis and cascade screening in inherited HCM. This study aimed to identify and evaluate a novel TPM1 variant found in a Vietnamese family with HCM. The proband, a 3-year-old boy diagnosed with HCM, and eight relatives from three generations underwent clinical and genetic evaluation. A candidate variant initially identified by targeted next-generation sequencing was validated by PCR and Sanger seque

Diagnosis of a Neonate With Long QT Syndrome and Severe Complications Delayed due to an Unrecognized Familial History.

Long QT syndrome (LQTS) is a hereditary arrhythmic disorder associated with sudden cardiac death. We report a neonatal case of congenital LQTS that went undiagnosed in utero, leading to severe postnatal complications. A male preterm infant was delivered by emergency cesarean section due to fetal hydrops. Shortly after birth, he developed respiratory failure and metabolic acidosis, followed by ventricular tachycardia and torsades de pointes requiring prolonged resuscitation. During cardiopulmonary resuscitation, the father collapsed, and it was subsequently established that he had been receivin

The Role of TRPM4 Gene Mutations in Causing Familial Progressive Cardiac Conduction Disease: A Further Contribution.

Progressive cardiac conduction disease (PCCD) is a relatively common condition in young and elderly populations, related to rare mutations in several genes, including SCN5A, SCN1B, LMNA and GJA5, TRPM4. Familial cases have also been reported. We describe a family with a large number of individuals necessitating pacemaker implantation, likely due to varying degrees of PCCD. The proband is a 47-year-old-patient, whose younger brother died at 25 years of unexplained sudden cardiac death. Three paternal uncles needed a pacemaker (PM) implantation between 40 and 65 years for unspecified causes. At

Postnatal anxiety prevalence, predictors and effects on development: A narrative review.

The increasing prevalence of postnatal anxiety highlights the need for summarizing the recent research on this condition to inform screening and intervention efforts. This narrative review of the literature was derived from a search on PubMed and PsycINFO for papers published since 2010. The demographic risk factors for postnatal anxiety include being a young mother, having more education and being employed. Childbirth risk factors include being primiparous in one sample and multiparous in another, caesarean delivery, fear of the birth and of death during delivery, lack of control during labor
