Assessment of Outcomes Among Patients With Venous Thromboembolism With and Without Chronic Kidney Disease.

IMPORTANCE: Patients with venous thromboembolism (VTE) and concomitant chronic kidney disease (CKD) have been reported to have a higher risk of thrombosis and major bleeding complications compared with patients without concomitant CKD. The use of anticoagulation therapy is challenging, as many anticoagulant medications are excreted by the kidney. Large-scale data are needed to clarify the impact of CKD for anticoagulant treatment strategies and clinical outcomes of patients with VTE. OBJECTIVE: To compare clinical characteristics, treatment patterns, and 12-month outcomes among patients with

Assessment of Health Care Exposures and Outcomes in Adult Patients With Sepsis and Septic Shock.

IMPORTANCE: Current information on the characteristics of patients who develop sepsis may help in identifying opportunities to improve outcomes. Most recent studies of sepsis epidemiology have focused on changes in incidence or have used administrative data sets that provided limited patient-level data. OBJECTIVE: To describe sepsis epidemiology in adults. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study reviewed the medical records, death certificates, and hospital discharge data of adult patients with sepsis or septic shock who were discharged from the hospital between Oc

Assessment of functional outcomes of temporalis muscle transfers for patients with longstanding facial paralysis.

BACKGROUND: Usually, clinical evaluation of facial reanimation provides accurate information about contraction of the mimetic muscles and phonation but fails to identify smile recovery and to quantify the motility of the lower third of the face during a smile. The purpose of this study was to verify that, in longstanding facial palsy, the modified temporalis muscle transfer (MTMT) can result in the ability to smile, not only voluntarily with chewing, but also spontaneously with a sudden emotional stimulus, and to confirm that a symmetric smile can be obtained. METHODS: Ten patients of the tre

Could less be more when assessing patient-rated outcome in spinal stenosis?

STUDY DESIGN: Longitudinal study of the measurement properties of a brief outcome instrument. OBJECTIVE: In patients undergoing surgery for lumbar spinal stenosis, we compared the responsiveness of the Core Outcome Measures Index (COMI) with that of the condition-specific Swiss Spinal Stenosis Measure (SSM), an instrument developed to assess patients with neurogenic claudication. SUMMARY OF BACKGROUND DATA: The COMI is a validated multidimensional questionnaire for assessing the key outcomes of importance to patients with back problems. Being brief, it is associated with minimal respondent b

Assessing pharmacological interventions for bone metastases: the need for more patient-centered outcomes.

Bone metastases are associated with a broad spectrum of clinical sequelae. Pain, reduced mobility, skeletal complications and treatment-related events reduce quality of life. Numerous randomized controlled trials have evaluated pharmacological interventions to treat bone metastases. The primary outcomes used have evolved over the past 25 years; from improvement in pain to time to first skeletal-related event (SRE). In the current definition, a SRE consists of pathological fracture, spinal cord compression or the need for radiotherapy or surgery to the bone. Currently used outcomes can detect s

Multicenter independent assessment of outcomes in chronic myeloid leukemia patients treated with imatinib.

BACKGROUND: Imatinib slows development of chronic myeloid leukemia (CML). However, available information on morbidity and mortality is largely based on sponsored trials, whereas independent long-term field studies are lacking. PATIENTS AND METHODS: Consecutive CML patients who started imatinib treatment before 2005 and who were in complete cytogenetic remission (CCyR) after 2 years (± 3 months) were eligible for enrollment in the independent multicenter Imatinib Long-Term (Side) Effects (ILTE) study. Incidence of the first serious and nonserious adverse events and loss of CCyR were estimated

Assessment of severity, causes, and outcomes of hospitalized trauma patients at a major trauma center.

BACKGROUND AND AIM: Trauma is a major cause of hospitalization in both civilians and military personnel (MP). It constitutes one of the most common causes of work layoffs, lowered efficiency, and handicaps in the civilian as well as the military population. Assessment of injury parameters and severity is useful. It allows for grading, grouping, classification, and assessment of outcomes of trauma patients for comparison with world standards. This study aimed to assess the Injury Severity Scores (ISS), recognize the source, and types of injuries leading to hospitalization of civilian and milita

Use of neuron-specific enolase for assessing the severity and outcome in patients with neurological disorders.

Neuron-specific enolase (NSE) is a glycolytic enzyme present almost exclusively in neurons and neuroendocrine cells. NSE levels in cerebrospinal fluid (CSF) are assumed to be useful to estimate neuronal injury and clinical outcome of patients with serious clinical manifestations such as those observed in stroke, head injury, anoxic encephalopathy, encephalitis, brain metastasis, and status epilepticus. We compared levels of NSE in serum (sNSE) and in CSF (cNSE) among four groups: patients with meningitis (N=11), patients with encephalic injuries associated with impairment of consciousness (ENC

The outcome of renal ultrasound in the assessment of 556 consecutive patients with benign prostatic hyperplasia.

PURPOSE: We determined the additional value of renal ultrasonography in the assessment of patients with benign prostatic hyperplasia. MATERIALS AND METHODS: Renal ultrasound was performed in 556 consecutive patients with benign prostatic hyperplasia and the results were correlated with other clinical parameters. RESULTS: Of the patients 14 (2.5%) had dilatation of the renal pelvis, 65 (11.7%) had renal cysts and 1 (0.18%) had renal cell carcinoma. The serum creatinine level appeared to be correlated with dilatation of the renal pelvis. To predict dilatation additional information can be obta

Multidimensional Assessment of Patient-Reported-Outcomes in a Multicenter Cohort of Inborn Errors of Immunity.

Patient-reported outcomes are critical to multidisciplinary, patient-centred approaches in diseases requiring lifelong management. Among inborn errors of immunity (IEIs), reports on this subject are typically limited to specific diagnostic subgroups or focus narrowly on the route of immunoglobulin replacement therapy (IgRT), offering a restricted perspective. We aimed to evaluate the health-related quality of life (HRQoL) and IgRT-related treatment satisfaction (TS) of a heterogeneous cohort of IEI patients and identify factors influencing these outcomes to guide improving the health and well-

Multimodal assessments of therapist characteristics are largely unrelated to patient outcomes: A preregistered analysis.

While it is known that therapists vary in effectiveness, it is unclear what therapist-level characteristics predict this variation. We conducted a large-scale, preregistered study (n = 97 therapists from the United States and Canada, n = 6,152 patients) examining a multimodal set of 38 therapist-level predictors that have been empirically or theoretically linked with patient outcomes. We examined associations with pre-post change and rate of change in psychological distress, and likelihood of attending >1 treatment session. We largely did not find associations between therapist-level character

ASSESSMENT BY ORAL HEALTH PROFESSIONALS OF PATIENT-REPORTED OUTCOMES IN OBSTRUCTIVE SLEEP APNEA.

Oral health professionals, including general dentists and orthodontists, may participate in the provision of care to adults and children with obstructive sleep apnea (OSA) when referred by a sleep medicine specialist. The use of oral appliances (OA) has been presented as a possible management option for mild obstructive sleep apnea among adults. In children, although oral interventions are not supported as a first-line stand-alone management option for OSA, short-term studies have suggested an improvement in sleep signs and symptoms in milder cases. The monitoring of management outcomes from O

Symptoms Burden as a Clinical Outcomes Assessment in Heart Failure Patients With Atrial Fibrillation.

BACKGROUND: Safe and effective pharmacologic therapy for atrial fibrillation (AF) in heart failure (HF) is an unmet need. In AF clinical trials, the standard primary endpoint of time to first symptomatic AF event (TTFSE) has several disadvantages, which could theoretically be overcome by measurement of AF-specific symptoms burden during an entire follow-up period. OBJECTIVES: The authors sought to develop and validate a method of measuring symptom burden of AF in a HF population. METHODS: The authors constructed a patient-reported outcome instrument (AFSQ [Atrial Fibrillation Symptoms Questi
