The Thompson-McFadden Commission and Joseph Goldberger: contrasting 2 historical investigations of pellagra in cotton mill villages in South Carolina.

As pellagra reached epidemic proportions in the United States in the early 20th century, 2 teams of investigators assessed its incidence in cotton mill villages in South Carolina. The first, the Thompson-McFadden Commission, concluded that pellagra was likely infectious. The second, a Public Health Service investigation led by Joseph Goldberger, concluded that pellagra was caused by a dietary deficiency. In this paper, we recount the history of the 2 investigations and consider how the differences between the 2 studies' designs, measurements, analyses, and interpretations led to different conc…

Protocol for a Case-Control Study to Investigate the Association of Pellagra With Isoniazid Exposure During Tuberculosis Preventive Treatment Scale-Up in Malawi.

Background: Pellagra is caused by niacin (vitamin B3) deficiency and manifested by a distinctive dermatitis. Isoniazid is critical for treating tuberculosis globally and is a component of most regimens to prevent tuberculosis. Isoniazid may contribute to pellagra by disrupting intracellular niacin synthesis. In 2017, Malawian clinicians recognized a high incidence of pellagra-like rashes after scale-up of isoniazid preventive treatment (IPT) to people living with HIV (PLHIV). This increase in pellagra incidence among PLHIV coincided with a seasonal period of sustained food insecurity in the re…

Pellagra-Fine arts and literature: A short journey from Pietro Belloti and Johann Wolfgang Goethe to John Wesley Caroll and Amedeo John Engel Terzi.

Pellagra is a non-hereditary disorder of the niacin and tryptophan metabolism. Cutaneous and gastrointestinal signs and symptoms are early signs of the disease, but neuropsychiatric findings follow, if the disease is not recognized and treated soon enough. Although pellagra had not been identified as a separate entity before 1735, artists noticed the characteristic cutaneous manifestations. An example is Pietro Bellotti's masterpiece Vecchia popolana con un ragazzo (Old Peasant Woman with a Boy; ca 1680-1690) with a most realistic style. In 1786, the German writer and scientist Goethe traveled…

Pellagrous encephalopathy presenting as alcohol withdrawal delirium: a case series and literature review.

BACKGROUND: Alcohol withdrawal delirium (AWD) is associated with significant morbidity and mortality. Pellagra (niacin deficiency) can be a cause of delirium during alcohol withdrawal that may often be overlooked. OBJECTIVES: We present a three-patient case series of pellagrous encephalopathy (delirium due to pellagra) presenting as AWD. METHODS: We provide a brief review of pellagra's history, data on pellagra's epidemiology, and discuss pellagra's various manifestations, particularly as related to alcohol withdrawal. We conclude by providing a review of existing guidelines on the managemen…

Nicotinamide: a double edged sword.

Enrichment of diet with Nicotinamide in the West was introduced in the 1940s to prevent the dietary deficiency disorder Pellagra. Pellagra was caused by a particular form of poor vegetarian diet leading to Nicotinamide and Tryptophan deficiency. Arguably Pellagra would have disappeared if dietary measures suggested at the time had been implemented before Nicotinamide was even discovered. Diets may sometimes now be too high in selected pyridines and inadvertently we have exchanged one neurodegenerative disease for another. Parkinson's disease triggered in contrast to Pellagra by a particular fo…

[Niacin deficiency and cutaneous immunity].

Niacin, also known as vitamin B3, is required for the synthesis of coenzymes, nicotinamide adenine dinucleotide (NAD) and nicotinamide adenine dinucleotide phosphate (NADP). Niacin binds with G protein-coupled receptor (GPR) 109A on cutaneous Langerhans cells and causes vasodilation with flushing in head and neck area. Niacin deficiency due to excessive alcohol consumption, certain drugs or inadequate uptake in diet causes pellagra, a photosensitivity dermatitis. Recently several studies have revealed the mechanism of photosensitivity in niacin deficiency, which may pave a way for new therapeu…

[An Uncommon Diagnosis].

We report on a 61-year-old patient who suffered from severe protein-energy malnutrition due to an inadequately treated exocrine pancreatic insufficiency. In this context, a thiamine deficiency was not recognized and there were clinical manifestations of beriberi disease with decompensated biventricular heart failure. In the course of time, a manifest niacin deficiency (pellagra) with dermatitis, diarrhea and persistent delirium occurred, which was recognized and could be treated. We highlight differential diagnostic considerations about the consequences and the treatment of malnutrition, with …

Profile of Joseph Goldberger (1874-1929): a remarkable physician, epidemiologist and scientist.

Joseph Goldberger (1874-1929) was a physician and epidemiologist conducting research in the United States in the first half of the 20th century. His greatest achievements include proving that the cause of pellagra are nutritional deficiencies. Pellagra is a serious disorder manifesting through diverse skin lesions and damage to internal organs - without proper treatment leading to death. The results of Joseph Goldberger's research enabled effective treatment of this disease and contained its epidemic in the United States. The aim of the work is to present the most crucial aspects of the life a…

Lesions in the skin, intestine, and central nervous system induced by an antimetabolite of niacin.

Pellagra in the human is characterized by the clinical "three D's," namely, dermatitis, diarrhea, and dementia. Newborn mice that received a single intraperitoneal injection of 6-aminonicotinamide (6-AN) (50 mg/kg body weight), an antagonist of niacin, consistently developed lesions in the skin, intestinal tract, and central nervous system. Anterior horn cells in the spinal cord as well as motor neurons in the brain showed the ultrastructural features of neuronal chromatolysis, while glial and ependymal cells showed postinjection (PI) edematous changes on Day 5. In the skin, correlating with c…

Until the lab takes it away from epidemiology.

BACKGROUND: It is proposed that, historically, when the conditions are right for laboratory sciences to contribute to causal knowledge, they take the place of epidemiology as leaders in a given field. METHODS: Review examples of cholera, pellagra, and Kaposi's sarcoma. RESULTS: In the cases of cholera, pellagra, and Kaposi's sarcoma, epidemiology ceased to be at the forefront of knowledge acquisition when laboratory sciences identified more proximate causes than those that epidemiologic group comparisons could reveal. CONCLUSION: These three examples deal with diseases, which, at the indivi…

Niacin metabolism and indoleamine 2,3-dioxygenase activation in malnourished patients with flaky paint dermatosis.

Flaky paint dermatosis, characterized by extensive, often bilateral areas of flaking and pigmentation, mostly in sun unexposed areas is considered a feature of kwashiorkor in both children and adults, and must be differentiated from other dermatosis, including chapped and xerotica skin, and pellagra. In this case series we provide evidence that malnourished patients with flaky paint dermatosis and infection/inflammation shown laboratory data suggestive of indoleamine 2,3-dioxygenase (IDO) activation, besides decreased urinary excretion of N1-methylnicotinamide (N1 MN), a marker of pellagra. We…

The diagnostic importance of photosensitivity dermatoses in chronic alcoholism: report of two cases.

Many of the vitamin deficiency diseases have been almost completely eliminated in developed countries. Niacin deficiency is considered one of them. However, cases of pellagra are recently reported in West Europe, USA, Australia, and Japan in connection with chronic alcoholism, gastrointestinal malabsorption, and some medications. We report two cases of pellagra, manifesting as photosensitivity dermatoses with mental deterioration in chronic alcoholic abusers in the Mediterranean basin, the island of Crete in Greece. The report highlights the fact that all physicians should be alerted to photos…

The discovery of niacin, biotin, and pantothenic acid.

The aim was to describe the discovery of niacin, biotin, and pantothenic acid. By the 1920s, it became apparent that 'water-soluble B' (vitamin B) is not a single substance. In particular, fresh yeast could prevent both beriberi and pellagra, but the 'antipolyneuritis factor' in yeast is thermolabile, while the antipellagra factor is heat stable, suggesting that there are at least two water-soluble vitamins. Various terms were proposed for these water-soluble factors, but vitamins B(1) and B(2) were most widely used to refer to the thermolabile and heat-stable factors, respectively. Although v…

Biochemical assessment of niacin deficiency among carcinoid cancer patients.

OBJECTIVE: Carcinoid cancer patients often have elevated levels of serotonin or its precursor 5-hydroxytryptophan. Normally, serotonin synthesis accounts for a small fraction of tryptophan catabolism, which should be directed along a pathway that allows partial conversion to niacin; hence, increased diversion of tryptophan toward serotonin could cause variable degrees of niacin deficiency in carcinoid patients. Therefore, the prevalence of niacin deficiency among carcinoid patients was investigated by clinical assessment of pellagra and biochemical assessment of whole blood niacin number, a ra…

The dominant diseases of modernized societies as omega-3 essential fatty acid deficiency syndrome: substrate beriberi.

About 1900, modern food selection and processing caused widespread epidemics of the B vitamin deficiency diseases of beriberi and pellagra which, for genetic reasons, often expressed as different diseases ranging from bowel and heart disease to dermatoses and psychoses. But the B vitamins merely help convert essential fatty acids (EFA) into the prostaglandin (PG) tissue regulators and it now turns out that, through hydrogenation, milling and selection of w3-poor southern foods, we have also been systematically depleting, by as much as 90%, a newly discovered trace Nordic EFA (w3) of special im…

Malnutrition and skin disease in Far East prisoners-of-war in World War II.

During the Second World War, thousands of captured British and Commonwealth troops were interned in prisoner-of-war (POW) camps in the Far East. Imprisonment was extremely harsh, and prisoners developed multiple pathologies induced by physical hardship, tropical infections and starvation. Immediately after the war, several POW doctors published their clinical experiences, including reports of skin disease caused by malnutrition. The most notable deficiency dermatoses seen in Far East POWs were ariboflavinosis (vitamin B2 or riboflavin deficiency) and pellagra (vitamin B3 or niacin deficiency).…
