Peyronie's disease is an uncommon condition involving middle-aged men and probably resulting from penile trauma. The injury causes an inflammation in the tunica and ultimately scarring and penile curvature develop. It may also occur due to nonspecific inflammation. The use of medications such as beta-blockers can sometimes result in Peyronie's disease. In Peyronie's disease, the normal elastic tissue of the tunica is replaced by scar tissue. MRI can delineate finer details of penile anatomy and pathology due to inherent higher soft-tissue contrast and spatial resolution. It can characterize inflammation and identify abscesses, localize penile fractures, guide surgical planning in penile fibrosis and Peyronie's disease, and depict components of the penile prosthesis and its complications. MRI is a great investigative tool for penile neoplasms, including locally infiltrative neoplasms where clinical examination is limited, and local staging is crucial for surgical planning.

OBJECTIVE: To present our initial experience with extended plication repair for men with severe and/or biplanar penile curvature.
MATERIALS AND METHODS: A review of men who underwent plication repair for complex penile deformity (biplanar curvature or curvature ≥ 60°) was performed. Peyronie's disease appears to be a condition of middle-aged men. In many of them, the disease is accompanied by erectile dysfunction, which, until recently, was attributed to penile deformity. Later, it turned out that the erectile dysfunction has the same causes as in men without Peyronie's disease with vascular and psychological components. In all cases, the pathology of the disease is characterized by the formation of a fibrous plaque, which will lead to penile curvature at different degrees and directions during erection and pain.
