Prophylactic use of pentoxifylline on inflammation in elderly cardiac surgery patients.

Источник пометил публикацию как отозванную

BACKGROUND: Inflammation plays a pivotal role in the pathogenesis of organ injury after cardiopulmonary bypass (CPB). Elderly patients appear to be especially prone to develop general inflammation. Use of pentoxifylline (PTX) before surgery may be a promising approach to minimize the negative effects of CPB in these patients. METHODS: In a prospective, randomized study, patients more than 80 years old undergoing aortocoronary artery bypass grafting received either PTX (n = 15) after induction of anesthesia (initial bolus of 300 mg followed by a continuous infusion of 1.5 mg.kg(-1).h(-1) durin…

Pentoxifylline improves circulatory failure and survival in murine models of endotoxaemia.

Pentoxifylline, a methylxanthine derivative, has been widely used to improve erythrocyte deformability and capillary blood circulation in patients with claudication and cerebrovascular disorders as well as in animals with sepsis. Here, we investigate the effects of pentoxifylline on the hypotension, vascular hyporeactivity to noradrenaline, release of tumour necrosis factor-alpha (TNF-alpha) and nitric oxide (NO), and inducible NO synthase protein expression in a rat model of circulatory shock induced by bacterial endotoxin (Escherichia coli lipopolysaccharide). In addition, we have evaluated …

Successful application of pentoxifylline in the treatment of HTLV-I associated myelopathy.

Fifteen patients with human T-cell lymphotropic virus type-I (HTLV-I)-associated myelopathy (HAM) were treated in an uncontrolled preliminary trial by oral administration of pentoxifylline (PTX). Motor function, neurological evaluation, immunological markers and parameters were evaluated after four weeks. In 13 of the 15 patients, motor disability, especially spasticity, improved substantially. PTX suppressed spontaneous proliferation of peripheral blood mononuclear cells in 14 of the 15 patients at four weeks. No adverse effect was observed. We concluded that PTX may be a safe and beneficial …

Effects of pentoxifylline pretreatment on Kupffer cells in rat liver transplantation.

Previous research with pentoxifylline (PTX), a methylxanthine phosphodiesterase inhibitor, suggests that this drug may be capable of suppressing the activation of Kupffer cells and thereby help decrease liver injury after transplantation. To investigate this possibility, the current study sought to determine whether the release of O2- and tumor necrosis factor (TNF) from Kupffer cells in donor livers can be suppressed if the organs are exposed to PTX before preservation. In an in vitro experiment, rat livers were flushed with PTX (25 mg/kg body weight) in University of Washington (UW) solution…

The effect of pentoxifylline on mouse in-vitro fertilization and early embryonic development.

This study was designed to assess the effect of pentoxifylline (PTX) on fertilization and early embryonic development in the mouse. Oocytes from superovulated B6CBA female mice were inseminated in vitro with spermatozoa from B6CBA males incubated with PTX according to different protocols, i.e. (i) 3.6 and 7.2 mM PTX washed out prior to insemination, (ii) 3.6 and 7.2 mM PTX diluted six times in the insemination medium and (iii) PTX present at 3.6 and 7.2 mM in the insemination medium. After insemination and washing, fertilization was assessed by the presence of 2-cell stage embryos. These were …

Variability of the response to pentoxifylline in vitro in infertile normozoospermic and asthenozoospermic patients.

The in vitro effect on the motility of spermatozoa of the methylxanthine pentoxifylline was studied in 30 patients consulting for infertility (13 normozoospermic and 17 asthenozoospermic). After separation by centrifugation on Percoll gradient, the spermatozoa were incubated for 30 min in pentoxifylline (3.6 mM), then the pentoxifylline was removed by washing and centrifugation. The residual effect of pentoxifylline was studied by computer-assisted videomicrographic analysis of the sperm motion parameters. In both groups of patients a decrease in the percentage of motile spermatozoa after expo…

Effect of pentoxifylline on intraperitoneal adhesions after intestinal resection in rats.

Pentoxifylline, an analogue of the methylxanthine theobromine, inhibits glycosaminoglycan and collagen synthesis by dermal fibroblasts in vitro and also inhibits the proliferation of dermal fibroblasts. It may have the same effect on fibroblasts derived from postoperative adhesion bands, thus preventing postoperative adhesion formation. An animal model was developed to evaluate the effect of pentoxifylline. Twenty-four male Wistar rats were divided into four groups, and all underwent laparotomy with a 15 cm intestinal resection and reanastomosis. The intestinal serosa was scratched to induce a…

Enhancement of radiation response on human carcinoma cells in culture by pentoxifylline.

PURPOSE: Pentoxifylline, a methylxanthine, has been shown to improve tumor tissue oxygenation in hypoxic murine tumors and prevent the late radiation injury of normal tissues in mice. The present cell culture studies were carried out to determine whether pentoxifylline would enhance the cellular radiation response of several human carcinoma cells in culture under various culture conditions. METHODS AND MATERIALS: Experiments were carried out with three human carcinoma cells grown in Eagle's MEM supplemented with 10% FCS. Cell survival was assayed by the colony forming ability of single plated…

Pentoxifylline modulates activation of human neutrophils by amphotericin B in vitro.

The antifungal agent amphotericin B (AmB) alters neutrophil (polymorphonuclear leukocyte [PMN]) function, and this may be the mechanism for some of the adverse effects caused by AmB. AmB is a potent inhibitor of PMN migration, increases PMN adherence and aggregation, and primes PMN for increased oxidative activity in response to a second stimulus. AmB also stimulates mononuclear leukocytes (MNLs) to release inflammatory mediators which augment the effects of AmB on PMN function. In the present study, we observed that the methylxanthine derivative pentoxifylline decreased the effects of AmB on …

Efficacy of pentoxifylline as a modulator of alkylating agent activity in vitro and in vivo.

Pentoxifylline, a methylxanthine that is used to treat veno-occlusive disease, can increase perfusion in undervascularized tissues. Addition of high concentrations, like caffeine, causes progression through radiation or drug induced G2 phase blocks, thereby limiting time for repair of DNA breaks and crosslinks. We have examined the potential of pentoxifylline to augment the effects of antitumor alkylating agents in vitro and in vivo. In MCF-7 human breast cancer cells in vitro, pentoxifylline (2 mM) present for 24 h was only slightly cytotoxic (approximately 10% cell kill at 2 mM), but when pr…

[Treatment of vaso-occlusive crisis of sickle-cell disease with pentoxifylline].

Sickle-cell disease is usually complicated by vaso-occlusive attacks. A review of the case-histories of 26 patients permitted an analysis of 92 severe attacks. We compared the outcome of 52 attacks treated with hyperhydration associated with a modifying erythrocyte membrane agent (pentoxifylline), with 40 attacks treated with hyperhydration and other drugs. This result compares well the current knowledge of the rheologic mechanisms involved in vaso-occlusive attacks of sickle-cell disease.
