Epithelium and bacteria in periapical lesions.

OBJECTIVE: The purpose of this study was to evaluate 50 human periapical lesions for bacteria and epithelium in a case study in dental practice. STUDY DESIGN: Specimens were obtained from the extraction of 50 untreated teeth that had lesions attached to their apices. The specimens were histologically evaluated using serial sections. RESULTS: Bacteria were found in all teeth, colonizing necrotic tissue in the main canal, dentinal tubules, or apical ramifications, and in the body of the periapical lesion in 18 abscesses or cysts. Twenty-one lesions were epithelialized; 14 abscesses, 20 granulo

On the causes of persistent apical periodontitis: a review.

Apical periodontitis is a chronic inflammatory disorder of periradicular tissues caused by aetiological agents of endodontic origin. Persist … ual infection can persist. Further, there are extraradicular factors -- located within the inflamed periapical tissue -- that can interfere with post-treatment healing of apical periodontitis. The causes of apical periodontitis persisting after root canal treatment have not been well characterized. During the 1990s, a series of investigations have shown that there …

Successful Healing of Periapical Pathology with Partial Pulpotomy in a Mature Permanent Molar: A Case Report.

BACKGROUND Vital pulp therapy approaches are preferred over non-surgical root canal treatment, giving the advantage of preserving the vitali … al pulpotomy should be considered as an alternative treatment to promote the return dental pulp and periapical tissue to a healthy condition. Considering a similar approach in older patients would be interesting to gain a more comprehensive understanding of its potential as a treatment method.

Co-production of vascular endothelial cadherin and inducible nitric oxide synthase by endothelial cells in periapical granuloma.

AIM: To clarify the mechanisms of inflammatory cell migration in human periapical granulomas by examining vascular endothelial (VE) cadherin and inducible nitric oxide synthase (iNOS)-producing cells. METHODOLOGY: Periapical tissues were obtained from patients during endodontic surgery and were divided into two portions. After fixing the tissues with acetone or 4% paraformaldehyde in phosphate-buffered saline, 5-microm-thick paraffin or cryostat sections were prepared, respectively. The paraffin sections of the inflamed tissues were evaluated histologically with haematoxylin-eosin stains. Cry…

Radicular Cysts and Periapical Granulomas: Data Documentation for 696 Cases and Findings on Fibrosis, and Porphyromonas gingivalis and Fusobacterium nucleatum in These Lesions.

OBJECTIVE: The objective of our study was to re-evaluate periapical lesions, including radicular cysts (RCs) and periapical granulomas (PGs) for locations, histopathological features, and degree of fibrosis in relation to the inflammatory response. In addition, we examined the presence of Porphyromonas gingivalis (Pg) and Fusobacterium nucleatum (Fn) since both are widely recognized pathogens in periodontal infections. METHODS: We re-evaluated samples of RCs and PGs (n = 728) and collected data for analyses by IBM's SPSS Statistics. Among these samples, we stained 93 samples to determine the

Factors affecting the periapical healing process of endodontically treated teeth.

Tissue repair is an essential process that reestablishes tissue integrity and regular function. Nevertheless, different therapeutic factors … f pulp or infectious processes and simultaneous neutralization of unpredictable provocations to the periapical tissue. Along with these factors, one must understand the local and general clinical conditions (systemic health of the patient) that affect the outcome of root canal treatment prediction.

Effect of EGCG-based paste as intracanal dressing, in MMPs 2 and 9 expression in dog's periapical lesions.

High expression of MMP-2 and MMP-9 in periapical lesions plays an important role in the degradation of the extracellular matrix. This study … g medication with EGCG and calcium hydroxide-based pastes provided similar repair of the apical and periapical tissues and neoformation of periodontal ligament fibers, cementum, and alveolar bone (p>0.05). The experimental groups treated in two sessions with both medications presented expression of MMP-2 and MMP-9 similar to that in healthy teeth (p>0.05), and sign …

Influence of the enlargement of the apical foramen in periapical repair of contaminated teeth of dog.

Root canals of dogs' premolar teeth were exposed to contamination and to the development of periapical inflammation over a period of 45 days. Root canals were overinstrumented 2 mm beyond the apical foramen with file No. 60 and were underfilled by 2 to 3 mm short of the roentgenographic apex. Ninety days after endodontic treatment, healing and ingrowth of connective tissue into the root canal occurred in 67.8% of the cases. These results suggest that the diameter of the apical foramen and the intensity of the canal contamination are determining factors for periapical tissue repair in cases of …

Accelerated healing of endodontically treated teeth with a periapical lesion as a result of orthodontic extrusion: evaluation and rationale.

Resolution of a periapical periodontal lesion as determined by radiographic examination may take a year or longer. Orthodontic movement is known to have a positive effect on the bone topography and morphology. The literature presents no information about the effect of orthodontic movement on the rate of periapical endodontic healing following a root canal treatment. The aim of this report was to present the use of orthodontic root extrusion in cases with a periapical lesion, where other options are ruled out. Three case reports involving endodontically treated teeth with a periapical lesion a

Periapical surgery.

A retrospective study of 572 teeth treated with periapical surgery was undertaken in order to evaluate the influence of preoperative, operative and postoperative factors in the healing process as well as the operation method used. It was also the aim to assess whether the histophatological diagnosis of biopsies taken at the operation could be correlated to healing. Statistical analyses by the AID method and conventional cross-classification methods were performed. Factors of importance were found to be the extent of the destrucion, the operation method used, the bone surrounding the destructio

MicroRNAs: new insights into the pathogenesis of endodontic periapical disease.

INTRODUCTION: Apical periodontitis is an inflammatory disease of the periradicular tissues caused by the host's immune response to infection of the root canal system. MicroRNAs (miRNAs) have been shown to play an important role in the regulation of inflammation and the immune response; however, their role in the pathogenesis of endodontic periapical disease has not been explored. The purpose of this study was to examine the differential expression of miRNAs in diseased periapical tissues as compared with healthy controls. METHODS: We first compared miRNA profiles in diseased periapical tissue…

[Periapical odontogenic processes of inflammatory origin. Enzyme activity].

A histoenzymological study was carried out on 40 tissues specimens removed at biopsy and for surgical operations of the following lesions: 5 normal oral mucosa, 5 periapical granulomas, and 30 periapical inflammatory cysts. The purpose of this study was to study some possibly significant variations in levels o activities of oxidative enzymes, and hydrolaxes enzymes. In inflammatory cysts, enzymatic activities were similar to normal epithelium. There was high levels of acid phosphatase LDH and G6PDH activity in the central cells of apical granulomas and in the exfoliating epithelial cells of pe

Development of periapical lesions.

The purpose of the present study was to follow the development of periapical lesions both radiographically and histologically in infected teeth with open and sealed root canals. The mandibular premolars from five adult monkeys were used in the experiment. Sealed infected teeth developed radiographic signs of periapical pathosis significantly earlier than unsealed teeth. Although, histological signs of pathology could be seen periapically at earlier observation periods, sealed teeth consistently developed these changes earlier than unsealed teeth. Furthermore, the histological periapical pathol
