Nonviral infectious pericarditis.

Infections of the pericardium can occur as incidental findings in patients with systemic disease or dominate the clinical situation, representing a major threat to life. These infections appear to arise from the contiguous spread of infected tissue or from hematogenous dissemination. The clinical manifestations can resemble the "textbook description," but more often are subtle, nonspecific, or altered by the patient's basic disease. The diagnosis should never be discarded because dyspnea or chest pain are not elicited, friction rub and pulsus paradoxus are not detected, and chest x-rays and el

Nocardia beijingensis pericarditis presenting with cardiac tamponade: A case report.

We report the first case of Nocardia beijingensis pericarditis in a 32-year-old HIV-infected patient. He presented with cardiac tamponade after failing to respond to treatment for smear-negative pulmonary and pericardial tuberculosis (TB). The pericardial fluid was examined several times before it eventually revealed filamentous branching organisms in Gram and modified acid-fast bacilli stain. The culture grew Nocardia spp. and was identified by 16s rRNA sequencing as N. beijingensis. Eight previously reported cases of Nocardia pericarditis in HIV-infected patients were caused by Nocardia aste

Early recognition of cardiac tamponade.

Nine patients with cardiac tamponade were seen in an 11-month period. Analysis of the clinical and laboratory data indicated that pulsus paradoxus was the most useful physical sign and echocardiography the most useful investigative technique. Three of nine patients died but in only one was the late recognition of tamponade a possible factor in the outcome. Pericardial fenestrations were required in four patients. Viral pericarditis accounted for the tamponade in three cases. There were two cases each of uremia and malignant disease and one case of tuberculous pericarditis. One other case follo

The Evolving Etiologic and Epidemiologic Portrait of Pericardial Disease.

Pericardial disease includes a variety of conditions, including inflammatory pericarditis, pericardial effusions, constrictive pericarditis, pericardial cysts, and primary and secondary pericardial neoplasms. The true incidence of this varied condition is not well established, and the causes vary greatly across the world. This review aims to describe the changing pattern of epidemiology of pericardial disease and to provide an overview of causative etiologies. Idiopathic pericarditis (assumed most often to be viral) remains the most common etiology for pericardial disease globally, with tuberc

[Ascites due to constrictive pericarditis].

INTRODUCTION: We report a case in which ascites revealed chronic constrictive pericarditis (CCP) of tuberculous origin. OBSERVATION: A 65 year-old man had developed ascites the past over few weeks and oedema of the lower limbs. Treatment was surgical and consisted in pericardiectomy. DISCUSSION: Chronic constrictive pericarditis has become rare in industrialised countries with the reduction in the incidence of tuberculosis. Currently, its principle aetiologies identified are previous cardiac surgery and radiotherapy. Treatment is usually surgical and gives excellent results. This clinical ca

Pulmonary and extrapulmonary tuberculosis.

Pulmonary tuberculosis: primary tuberculosis, usually asymptomatic, represents the first infection and is shown by a parenchymal mostly mid-pulmonary focus and satellite lymphadenopathy. Postprimary pulmonary tuberculosis, mostly located in the upper fields may be caused by endogenous reinfection for reactivation of a hematogenous focus formed during primary infection or from exogenous reinfection. Extrapulmonary tuberculosis: it includes numerous forms mostly from hematogenous spread. Miliary tuberculosis may involve a number of organs and apparatus besides the lung. Tuberculous meningitis pr

Extrapulmonary tuberculosis--a simple diagnosis? A retrospective study at Dar es Salaam, Tanzania.

Data from the national tuberculosis programme show that extrapulmonary tuberculosis is increasing rapidly in Tanzania, most likely caused by the human immunodeficiency virus (HIV) epidemic. Retrospective data from 271 patients admitted to Muhimbili Medical Centre (MMC), Dar es Salaam between January 1, 1987 and December 31, 1988 with the diagnosis of extrapulmonary tuberculosis reveal that in only 18% of the cases a bacteriological or histological confirmation of the diagnosis had been made. This figure is only 3% when the tuberculous lymphadenitis cases are excluded. The most common diagnosis

A life-threatening pericardial tamponade revealing disseminated tuberculosis: A challenging medical care.

Tuberculous pericarditis (TBP) is one of the most important causes of pericarditis in developing countries in which tuberculosis remains a public health problem. We report the case of a 20-year-old immunocompetent woman who presented with hypotension and tachycardia echocardiography showed a massive pericardial effusion and signs of tamponade. The patient underwent an urgent pericardiocentesis; 700 ccs of serosanguinous pericardial fluid was drained. She had a surgical biopsy of the pleuropericardial window. The pathological examination confirmed TBP. Oral anti-tuberculosis therapy was initiat

Pericardiectomy: a functional anatomical perspective for the choice of left anterolateral thoracotomy.

Although pericardiectomy remains an established method for pericardial resection, the choice of surgical approach is not definitive. Within South Africa, surgical referral for tuberculosis-induced chronic constrictive pericarditis has not declined. Anecdotal reports have indicated good operative results that appear to show an association with choice of surgical technique used. This study aimed to provide a functional anatomical perspective for performance and recovery of the heart during pericardiectomy based on anatomical dissection and surgical notes. En bloc specimens were harvested from 16

Chronic constrictive pericarditis in children - etiology, clinical picture and treatment. A report of 20 cases.

Twenty children with constrictive pericarditis are reported on. Tuberculous etiology was established in five, could not be excluded in 7, while the etiology could not be identified in 8 cases. The main symptoms were venous congestion involving the pulmonary and systemic circulation (hepatomegaly, pulmonary congestion), pericardial click at the apex, abnormalities of repolarization in the ECG. X-ray evidence of pericardial calcification and a "quiet heart" were occasionally found. Hypoproteinaemia, hypalbuminaemia and ascites, signs of an associated protein losing enteropathy may occur in const

[Corticosteroids as adjuvants in the treatment of tuberculosis].

The anti-inflammatory effect of corticosteroids has been known for a long time, as have their use in treating diseases with an inflammatory component. Corticosteroids have also been used as an adjunct in treating tuberculosis. We have reviewed the literature to evaluate the rationale for our present treatment strategy. We find no reason for the use of Isoniazide in routine prophylaxis of Mantoux positive or former tuberculosis patients who are treated with corticosteroids. Corticosteroids could be added when treating severe pulmonary tuberculosis. When treating tuberculous pericarditis and sev

Pneumopericardium with massive pericardial effusion in the setting of tuberculosis constrictive pericarditis: a rare case report.

Constrictive pericarditis is an uncommon complication of acute pericarditis, mainly caused by non-idiopathic sources. Pneumopericardium is the presence of air in the pericardial sac resulting from various procedures and circumstances, including trauma, iatrogenic, non-iatrogenic and natural causes. Here, we report a 16-year-old girl who came to the cardiology outpatient complaining of weakness, abdominal distention and shortness of breath while lying down and exertion. An echocardiography evaluation revealed a thickened precordium and massive pericardial effusion. Pericardiocentesis was perfor

[A case of Vietnamese immigrant with tuberculous mediastinopericarditis].

A case of vietnamese immigrant with tuberculous mediastinopericarditis was reported and the literature was reviewed. A 39 year-old man was admitted to our hospital with chief complaints of dyspnea and weakness. Blood chemistry suggested the existence of congestive liver dysfunction. Chest X-ray film revealed marked cardiomegaly and a abnormal mass in the anteriorly superior area which was confirmed as anterior mediastinum on CT scan. Echocardiogram disclosed a large volume of pericardial effusion and thickened pericardium. Tuberculin test was positive. Firstly, this mediastinal mass was believ

Major vessel venous thrombosis in patients of posttubercular chronic constrictive pericarditis undergoing pericardectomy: A rare scenario.

We are reporting two cases of neck and arm major venous thrombosis in patients of posttubercular chronic constrictive pericarditis posted for pericardectomy. There was unanticipated difficulty in placement of Internal Jugular vein catheter and subsequent ultrasound revealed thrombosis in the major veins. It was not diagnosed in the preoperative period. This report raises this major complication and highlights the use of ultrasound in such scenarios.

Aetiology of chronic constrictive pericarditis.

In a consecutive series of 32 cases of chronic constrictive pericarditis treated by pericardiectomy during the past 25 years, four were attributable to rheumatoid disease, two to trauma, one to sarcoidosis, and four, at a maximum, to tuberculosis. In the remaining 21 cases of undetermined aetiology there was no evidence of tuberculosis. It appears, therefore, that tuberculosis was not a common cause of chronic constrictive pericarditis during the period under review, which included the 1950s and early 1960s when tuberculosis was widespread.

Infective pericarditis in Nigerian children.

53 children with infective pericarditis were seen at the University College Hospital, Ibadan, between 1967 and 1976. Their ages ranged from 10 days to 15 years but 53% of them were aged 5 years and below. Cough, fever, and breathlessness were the most common symptoms; cardiac decompensation was evident in over 30% of them, 23% had muffled heart sounds, but a pericardial friction rub was audible in only one. The main pathogens identified were Mycobacterium tuberculosis (11 cases), Staphylococcus aureus (11 cases), Escherichia coli (4 cases), Pneumococcus and Pseudomonas (3 cases each). Most of
