Foot forces induced through Tai Chi push-hand exercises.

The low impact forces of Tai Chi push-hand exercises may be particularly suited for older people and for those with arthritis; however, the biomechanics of push-hand exercises have not previously been reported. This paper examines the ground reaction forces (GRFs) and plantar force distributions during Tai Chi push-hand exercises in a stationary stance with and without an opponent. Ten male Tai Chi practitioners participated in the study. The GRFs of each foot were measured in three perpendicular directions using two force plates (Kistler). The plantar force distribution of each foot was measu

Irreducible dorsal dislocation of the interphalangeal joint of the great toe.

A 54-year-old man had an irreducible dorsal dislocation of the interphalangeal (IP) joint of the great toe, and injury that seems not to have been previously reported in the literature. The stability at the IP joint afforded by collateral ligaments, tendons, and plantar accessory ligament along with the short level arm of the distal phalanx normally resists dislocation. A strong dorsiflexion force to this joint with the foot and metatarsophalangeal joint fixed in a weight-bearing position is necessary to produce this injury. Treatment should begin with attempted closed reduction, but if this p

Congenital Chopart amputation. A functional assessment.

Of 11 patients with congenital Chopart-level foot amputations, the average age was 6.75 years. Data collected from each patient included leg-length and calf-circumference measurements, range of motion at the ankle and subtalar joints, and a record of stump appearance. The patients were timed running a 50-m dash and measured in a standing broad-jump performance. Patients with congenital Chopart amputations functioned within normal limits in all of the authors' tests. No patient had a plantar flexion deformity. A slipper-style shoe filler or ankle foot orthosis with a foot plate and shoe filler

Bilateral Asymmetries of Plantar Pressure and Foot Balance During Walking, Running, and Turning Gait in Typically Developing Children.

Biomechanical asymmetries between children's left and right feet can affect stability and coordination, especially during dynamic movements. This study aimed to examine plantar pressure distribution, foot balance, and center of pressure (COP) trajectories in children during walking, running, and turning activities to understand how different movements influence these asymmetries. Fifteen children participated in the study, using a FootScan plantar pressure plate to capture detailed pressure and balance data. The parameters, including time-varying forces, COP, and Foot Balance Index (FBI), were

Gait, balance and plantar pressures in older people with toe deformities.

Older people with toe deformities have been identified as having an increased risk of falling. Little is known, however, about the biomechanical changes that might contribute to this increased risk. Therefore, the purpose of this study was to determine whether older people with hallux valgus and lesser toe deformities displayed different gait, balance and plantar pressure characteristics compared to individuals without toe deformities. The presence of hallux valgus and lesser toe deformities were assessed for 312 community-dwelling older men and women. Spatiotemporal gait parameters were measu

Slow velocity of the center of pressure and high heel pressures may increase the risk of Sever's disease: a case-control study.

BACKGROUND: This study determined if the body mass index, dynamic plantar-pressures, plantar surface contact-area, velocity of the centre of pressure (COP), gastrocnemius equinus, and gastrocnemius soleus equines are related to calcaneal apophysitis (Sever's disease) in athletic children. METHODS: This case-control study examined 106 boys enrolled in a soccer academy, including 53 with Sever's disease and 53 age-matched healthy controls. The dynamic average and maximum peak plantar-pressures, plantar surface contact-area, and velocity of the COP were evaluated with a digital pressure sensor p

Percutaneous Reduction of a Dislocation of the Interphalangeal Joint of the Great Toe: A Case Report.

Dorsal dislocation of the interphalangeal joint of the great toe is quite rare. Closed reduction is often attempted in the emergency setting, but this measure is seldom successful because of invagination of the sesamoid-plantar plate complex into the interphalangeal space. Generally, open reduction is indicated when closed reduction fails. In this report, percutaneous reduction of the incarcerated sesamoid was performed under local and intraarticular anesthesia at our outpatient clinic, leading to successful reduction.

Biomechanical Comparison of Hook Plate vs Headless Compression Screw Fixation of Large Fifth Metatarsal Base Avulsion Fractures.

BACKGROUND: Debate exists on the optimum fixation construct for large avulsion fractures of the fifth metatarsal base. We compared the biomechanical strength of 2 headless compression screws vs a hook plate for fixation of these fractures. METHODS: Large avulsion fractures were simulated on 10 matched pairs of fresh-frozen cadaveric specimens. Specimens were assigned to receive two 2.5-mm headless compression screws or an anatomic fifth metatarsal hook plate, then cyclically loaded through the plantar fascia and metatarsal base. Specimens underwent 100 cycles at 50%, 75%, and 100% physiologic

Effect of shaft stiffness and sole flexibility on perceived comfort and the plantar pressures generated when walking on a simulated underground coal mining surface.

The structural features of work boots worn by underground coal miners affect comfort, foot motion and, in turn, loading of the plantar surface of miners' feet. Although shaft stiffness and sole flexibility appear to be boot design features that could influence perceived comfort and plantar pressures, no study has systematically altered these boot design features to truly understand how they affect these parameters. This study aimed to systematically investigate the effect of changes to shaft stiffness and sole flexibility on perceived comfort and plantar pressures when 20 males walked on a sim

Arthrodesis or arthroplasty for end stage hallux rigidus: A comparison of 3D kinematic and plantar pressure gait metrics and PROMs.

BACKGROUND: In end stage hallux rigidus, treatment is mainly surgical in the form of joint arthrodesis or arthroplasty. Although promising results have been reported for both surgical techniques a detailed comparison of gait biomechanics has not been conducted. RESEARCH QUESTION: How do post-operative 3D kinematics, plantar pressures and patient reported outcome measures (PROMs) differ following arthrodesis or arthroplasty for end-stage hallux rigidus? METHODS: Eleven arthrodesis and eleven arthroplasties were performed. Pre- and post-operative (follow-up time, 7.5 ± 1.0 months) data were co

Presurgical Perspective and Postsurgical Evaluation of Morton's Neuroma and Other Nerve Lesions.

Magnetic resonance imaging (MRI) is a robust method used for both preoperative and postoperative evaluation of Morton's neuroma and other neural lesions. MRI is used to confirm the diagnosis and for precise localization, estimation of outcome, and differential diagnoses. The differential diagnoses include mechanically induced plantar plate ruptures with associated Morton's neuroma-like tumors in the intermetatarsal/interdigital spaces; mechanical fibrosis cushion formations and pseudo bursae in the plantar foot adipose tissue; rheumatologic affections, such as rheumatoid nodules, gouty nodules

Disorders of the Lesser Metatarsophalangeal Joints.

Pain in the region of the lesser metatarsophalangeal joints (often termed metatarsalgia) is a common complaint. It can be due to a variety of causes, and accurate diagnosis is essential for effective treatment. Understanding the anatomy and functions of the extrinsic and intrinsic musculature and the plantar plate, ligaments, and fat pad is important in evaluating metatarsophalangeal joint disorders. Claw toe is a hyperextension deformity of the metatarsophalangeal joint in combination with a hammer toe. Pathologic changes involving an isolated metatarsophalangeal joint may be due to monarticu

Interphalangeal dislocation of the fourth toe with avulsion-fracture in a child: report of a case.

Phalangeal dislocations of toes are extremely rare in childhood and usually can be treated by closed reduction. We present a proximal interphalangeal dislocation of the fourth toe with an irreducible avulsion fracture of the middle phalanx requiring open reduction. To our knowledge concomitant avulsion fractures in this condition have not been reported thus far in pediatric patients. The pathological mechanism of this injury is discussed, and the significance of the plantar plate for joint stability is emphasized.

Ultrasound of ankle and foot: overuse and sports injuries.

Sports and overuse injuries of the ankle and foot are commonly encountered in clinical practice. Ultrasound (US) has been established as an excellent diagnostic modality for foot and ankle injuries, providing a rapid noninvasive, economical, and readily available tool that is well tolerated by the patient with acute or chronic pain. The opportunity for dynamic examination is another advantage of US in evaluating ankle and foot pathology, where maneuvers such as muscle contraction and stressing of the joint may be particularly helpful. In many cases, US can be used as a first-line and only imag

Keller's arthroplasty with proximal metatarsal opening wedge osteotomy in treating severe Hallux valgus deformity--surgical techniques and case report.

The paper presents an operative technique proposed for treating severe Hallux Valgus deformity with arthrosis of the first metatarsal joint in elderly people. The procedure consists of the Keller's arthroplasty in combination with basal osteotomy of the first metatarsal bone, whereby the head of the metatarsal is displaced laterally and plantarward. The osteotomy is fixed with a reconstructive plate. The results show a significant improvement in the hallux valgus angle, the first metatarsal angle, as well as in the plantar pressure distribution.
