Platelet transfusions: utilization and associated costs in a tertiary care hospital.

We implemented a prospective study to evaluate platelet transfusion utilization, resource use, and costs in a tertiary care hospital over a 6-month period. All hospitalized patients receiving platelet transfusions between July and December 1996 were followed prospectively to determine platelet use and costs. Clinical and financial data were collected, evaluated, and compared to identify trends in resource utilization based on admitting service and platelet-refractory status. One thousand nine hundred forty-four platelet units were transfused to 245 hospitalized patients (50.6% male, mean age 4…

Alloimmunization to platelets in heavily transfused patients with sickle cell disease.

Bone marrow transplantation (BMT) is now an option for some patients with sickle cell disease (SCD). Many SCD patients are multiply transfus … nts are alloimmunized to HLA and/or platelet-specific antigens. These patients may be refractory to platelet transfusion, a condition that would increase their risk during BMT. Leukodepletion in the transfusion support of SCD patients should be considered to prevent platelet alloimmunization.

Hemolysis following platelet transfusions from ABO-incompatible donors.

Two hemolytic transfusion reactions related to isoagglutinins present in group O platelet concentrates are reported. The first, a severe reaction in a group A 10-kg patient, resulted from transfusion of 200 ml of plasma containing a hemolytic anti-A, titer 16, and an IgG anti-A, titer of 32,000. In the second case, a group B adult received between 50 and 70 ml of O plasma with a titer (16,000) of anti-B. This was followed by hemolysis of approximately 40 percent of the patient's red cell volume.

Platelet Additive Solutions as an Alternative Storage Medium of Apheresis Platelets to Reduce ABO Antibody Titer for ABO-Incompatibility Platelet Transfusion.

Introduction Platelet additive solutions (PASs) are nutrient media commonly used to replace and reduce the need for storage plasma. They are an alternative medium to maintain high-quality platelets lasting longer on the shelf for about seven days. Platelets with high titer of ABO antibody can pose a hemolytic transfusion reaction (HTR) risk if units are given across the ABO barrier. The risk of complication is greater when group O platelet is released to non-group O patients. The PAS has been known as a safe medium, where the titer of ABO antibodies is expected to be diluted. In this study, we

ABO-mismatched platelet and plasma transfusion practices and the potential for transfusion-related alpha-gal syndrome: The Biomedical Excellence for Safer Transfusion Collaborative Study.

BACKGROUND: Alpha-gal syndrome (AGS) is caused by IgE antibodies against the alpha-gal oligosaccharide, which is structurally similar to the Group B antigen. Recent case reports of severe allergic transfusion reactions (ATRs) in Group O patients receiving Group B plasma and platelets raise the possibility of a new adverse event, herein called transfusion-related AGS (TRAGS). The primary goal of this study was to assess the frequency of Groups B and AB plasma and platelet transfusions to Group O patients. STUDY DESIGN AND METHODS: In this multi-site retrospective study, participating sites sub…

Platelet Transfusions: Current Practices and Emerging Alternatives in the United States.

Platelet transfusions are a cornerstone of hemorrhage management in patients with thrombocytopenia or platelet dysfunction, yet their indications and dosing are largely based on expert opinion and low-quality evidence. This review offers a timely and comprehensive analysis of platelet transfusion practices in the United States (U.S.), uniquely integrating clinical evidence, such as the pivotal PLADO trial, with emerging technological advancements. Using a holistic approach, this manuscript addresses not only conventional practices (such as dosing standards and storage methods), but also cuttin…

[Evidence-based and practical: scrutiny of the new guidelines on platelet transfusion].

The transfusion of platelet concentrates is an established treatment for thrombocytopenia or platelet dysfunction. The new international guideline from the Association for the Advancement of Blood and Biotherapies in collaboration with the International Collaboration for Transfusion Medicine Guidelines is based on 21 randomized and 13 observational studies comparing restrictive and liberal transfusion strategies. The evidence shows that restrictive strategies do not significantly increase 30-day mortality or the risk of severe bleeding (WHO grade 3-4), but are associated with a lower rate of t

Platelet to erythrocyte ratio and mortality in massively transfused trauma patients.

BACKGROUND: The optimal transfusion ratio of platelets (PLT), plasma and red blood cells (RBC) in trauma patients with massive haemorrhage is still a subject of discussion. The objective of this study is to assess the effect of platelet transfusion on mortality in trauma patients who received massive transfusion. METHODS: Data were collected from four Dutch level-1 trauma centres. All trauma patients aged ≥ 16 years who received ≥ 6 RBC / 6 h from the time of injury were included. Patients were divided based on PLT:RBC ratio (no platelets, low (<1:5) and high (≥1:5)). Primary outcome measure …

Audit of appropriate use of platelet transfusions: validation of adjudication criteria.

BACKGROUND AND OBJECTIVES: Platelet (PLT) transfusions must be used appropriately, as they are in chronic short supply, costly and risky to … c transfusions for bleeding patients had the lowest (73%, n = 99). The lowest levels of appropriate platelet transfusions were observed in the operating rooms (60%) and when ordered by the general surgery service (55%). CONCLUSIONS: One in five platelet transfusions may be unnecessary, suggesting that interventions to improve PLT transfusion practice are warranted.

Platelet and not erythrocyte microparticles are procoagulant in transfused thalassaemia major patients.

The level of circulating platelet-, erythrocyte-, leucocyte- and endothelial-derived microparticles detected by high-sensitivity flow cytometry was investigated in 37 β-thalassaemia major patients receiving a regular transfusion regimen. The phospholipid procoagulant potential of the circulating microparticles and the microparticle-dependent tissue factor activity were evaluated. A high level of circulating erythrocyte- and platelet-microparticles was found. In contrast, the number of endothelial microparticles was within the normal range. Platelet microparticles were significantly higher in s

Platelet transfusions: impact on hemostasis, thrombosis, inflammation and clinical outcomes.

Platelet transfusion is one of the most crucial therapeutic approaches in Medicine. However, severe and fatal adverse reactions may develop. In addition to their important function in hemostasis, platelets' role in inflammation has become more evident. Recently, platelets are also recognized as the main source of circulating soluble CD40 ligand (sCD40L, (CD154)), which plays significant roles in hemostasis, platelet activation, clot stability, interactions with other cells, and upregulation of different mediators. In this review, we will briefly highlight the importance of platelet transfusion…

Transfusion of ABO-incompatible platelets causing severe haemolytic reaction.

Not infrequently clinical demand dictates that patients receive transfusions of ABO-incompatible platelets when there is a short supply of group-specific platelets. This carries a risk of causing a haemolytic reaction, as illustrated in the clinical case we report. In discussing this potential complication, we suggest a strategy for avoiding it.

Alloimmunization to public HLA antigens in multi-transfused platelet recipients.

Multitransfused recipients of random donor platelet concentrates frequently form broadly-reactive human leukocyte antigen (HLA) antibodies which restrict the pool of suitable donors to those who are HLA matched with the recipient. In an effort to expand the donor pool for such patients, a retrospective analysis of the antibodies formed by alloimmunized recipients was performed taking sensitization to public HLA antigens into account. A total of 144 sera from 18 subjects was analyzed. Peak reactive sera from these patients ranged from 53 to 100 percent when tested against lymphocyte panels. Whe

Platelet Transfusion Is Associated With Increased Thrombosis and In-Hospital Mortality Among Patients Hospitalized With Platelet Consumptive Disorders.

BACKGROUND: Platelet transfusion is commonly used to reduce bleeding risk in platelet consumptive disorders. However, platelets may also promote thromboinflammatory pathways, and contemporary data evaluating the association between platelet transfusion, thrombosis, and mortality in this population are limited. METHODS: We performed a retrospective cohort study using the National Inpatient Sample from 2018 to 2021. Non-elective hospitalizations with immune thrombocytopenic purpura, thrombotic thrombocytopenic purpura, heparin-induced thrombocytopenia, or other primary thrombocytopenias were in…

Platelet HLA gene bank digital matching technology for platelet transfusion refractory patients with malignant tumors: a case report.

The platelet human leucocyte antigen (HLA) gene bank contains the genetic information of HLA loci in a large number of blood donors. Currently, the most effective treatment for platelet transfusion refractoriness (PTR) is to evaluate the probability of antigenic mismatch by HLA genotyping of patients and to select HLA-matched donors in the gene bank through an information system. This case report describes the treatment of PTR in patients with malignant tumors using the platelet HLA gene bank digital matching technique. The analysis of individual cases will help guide transfusion strategies fo…

Evaluation of Human Leucocyte Antigen Mediated Platelet Transfusion Refractoriness and Platelet Crossmatching Assay in Patients with Hematologic Disorders.

OBJECTIVES: Platelet refractoriness complicates the platelet transfusion, which is essential for managing thrombocytopenia in patients with hematological disorders. It is associated with adverse clinical outcomes and increased health care costs. We conducted a prospective study to determine the effectiveness of cross-matched compatible platelets in a group of patients refractory to platelets from random donors and to evaluate human leukocyte antigen (HLA)-mediated refractoriness. METHODS: This prospective study was conducted on 40 patients with different hematological disorders requiring plat…
