Choriocarcinoma Presenting as a Pleural Effusion.

Choriocarcinoma is a germ cell tumor characterized by widespread metastases and poorly differentiated cells. Non-gestational choriocarcinoma, or primary choriocarcinoma is a trophoblastic disease which is associated with a poor patient prognosis and is markedly angioinvasive. Primary non-gestational mediastinal choriocarcinoma is a very rare disease and represents an aggressive malignancy, primarily seen in young males. Those with primary mediastinal choriocarcinoma have symptoms that are non-specific such as cough, dyspnea, hemoptysis, and chest pain. Here we present the case of a 47-year-old

A man with pleural effusion and ascites.

A male lifelong nonsmoker aged 58 years with no prior asbestos exposure complained of gradual worsening breathlessness over 3 months. This was associated with abdominal and leg swelling and a 2-kg weight loss. He had no fever, night sweats, hemoptysis, joint pain, rash, abdominal pain, chest pain, or orthopnea. The patient had no recent travel or contact with pulmonary TB. He had stage I left-side testicular seminoma treated with left-sided radical orchidectomy 10 years previous and recently received a diagnosis of Child's B alcoholic liver cirrhosis. His hepatitis B and C screen result was no

Etiologies of bilateral pleural effusions.

BACKGROUND: To evaluate the safety, etiology and outcomes of patients undergoing bilateral thoracentesis. METHODS: This is a prospective cohort study of 100 consecutive patients who underwent bilateral thoracenteses in an academic medical center from July 2009 through November 2010. Pleural fluid characteristics and etiologies of the effusions were assessed. Mean differences in levels of fluid characteristics between right and left lungs were tested. Associations between fluid characteristics and occurrence of bilateral malignant effusions were evaluated. The rate of pneumothorax and other co

Update on uncommon pleural effusions.

Although infections, malignancies and heart failure are responsible for the majority of pleural effusions, there are many other causes and several uncommon but distinctive types of pleural fluid. For this update we have chosen several uncommon forms of pleural effusions or disorders in which there have been recent advances in our understanding over the past several years. Chylothorax, pseudochylothorax and urinothorax are associated with characteristic clinical contexts and pleural fluid parameters but are likely underdiagnosed. Yellow nail syndrome is a rare disorder that can be associated wi

Recurrent pleural effusions in a neonate.

Difficulty breathing is a common presenting complaint for infants in the emergency department (ED). Periodic breathing, respiratory or systemic infection, and congenital heart disease are common diagnoses in this age group. We report the case of a male neonate presenting to the ED on multiple occasions with respiratory distress and recurrent pleural effusions of unique origin. Unlike adult pleural effusions, pediatric effusions are most commonly exudative and of infectious origin. In neonates, acquired chylothorax secondary to surgery or trauma is the most common cause of symptomatic pleural e

Significance of pleural effusion in neuroblastoma.

BACKGROUND: Pleural effusion is uncommon at diagnosis of neuroblastoma in children. Because the presence of malignant cells in pleural fluid may significantly change the management and outcome of patients with neuroblastoma, we retrospectively analyzed a cohort of neuroblastoma patients who presented with pleural effusion at the time of diagnosis to determine the incidence, presentation, stage, treatment, and outcome of these patients. METHODS: We reviewed the presenting features of 295 patients with the diagnosis of neuroblastoma who received treatment at St. Jude Children's Research Hospita

Pleural effusions in hematologic malignancies.

Nearly all hematologic malignancies can occasionally present with or develop pleural effusions during the clinical course of disease. Among the most common disorders are Hodgkin and non-Hodgkin lymphomas, with a frequency of 20 to 30%, especially if mediastinal involvement is present. Acute and chronic leukemias, myelodysplastic syndromes, are rarely accompanied by pleural involvement. Furthermore, 10 to 30% of patients receiving bone marrow transplantation develop pleural effusions. In cases of hematologic pleural effusions, drug toxicity, underlying infectious, secondary malignant or rarely

Pathogenesis of the eosinophilic pleural effusions.

PURPOSE OF REVIEW: Eosinophilic pleural effusions (EPE) are defined as those that contain at least 10% eosinophils. EPEs account for 5 to 16% of exudative pleural effusions. However, their pathogenesis is poorly understood. The purpose of this review is to discuss the mechanisms that lead to eosinophilic pleural inflammation. RECENT FINDINGS: Eosinophilic pleural effusions are caused by the presence of air or blood or both in the pleural space, infectious or other inflammatory diseases, malignancy, pulmonary emboli, asbestos exposure, and drug reactions. Differences in the clinical features s

Pleural effusions in acute pancreatitis.

BACKGROUND: Acute pancreatitis ranks third among the inflammatory diseases causing acute abdomen; it accounts for about 2% of all cases of acute surgical abdomen. The clinical course of the disease is characterised by a number of local and systemic complications, the most frequent being respiratory disorders. METHODS: In the present study we review our experience with the management of acute pancreatitis in 75 patients complicated by pleural effusions in some of them. The patients were allocated into two groups depending on the severity of the disease: group I included patients with mild panc

Pleural effusions in patients with AIDS.

OBJECTIVE: To describe the range of pathology causing pleural effusions in HIV infected patients with acute respiratory episodes and to attempt to identify whether any associated radiological abnormalities enabled aetiological discrimination. METHODS: Prospective study of chest radiographs of 58 consecutive HIV infected patients with pleural effusion and their microbiological, cytological, and histopathological diagnoses. RESULTS: A specific diagnosis was made in all cases. Diagnoses were Kaposi's sarcoma, 19 patients; para-pneumonic effusion, 16 patients; tuberculosis, eight patients; Pneum

Evaluation of pleural and peritoneal effusions.

Certain diseases cause an increase in the amount of fluid present in the pleural and/or peritoneal cavity (an effusion). Uroperitoneum subsequent to kidney, ureter, bladder, or urethra rupture also can cause an increased amount of fluid in the abdomen. Evaluation of fluid samples often is helpful in identifying the mechanism causing the effusion and, occasionally, results in a specific diagnosis. The TP, TNCC, and general cytologic examination can be performed easily, quickly, and inexpensively in-house. The TP and TNCC are used to classify effusions as transudates, modified transudates, or ex

Thoracoscopy in the diagnosis of pleural effusions.

Between January 1981 and December 1986, thoracoscopies were carried out on 152 patients in order to evaluate the diagnostic accuracy of the technique in cases of pleural effusion. All patients had previously undergone a thoracentesis together with an unguided pleural biopsy but had remained undiagnosed. Of them, 74 cases were confirmed as malignant; the others were benign. In 71 of the 74 cases of malignant pleural effusion, an accurate diagnosis had been obtained from the thoracoscopy and guided pleural biopsy, a diagnostic accuracy of 94.6%. Among 78 cases, diagnosed by the thoracoscopy as b

Pleural effusions in the atypical pneumonias.

Patients with atypical pneumonias, whether caused by bacterial, fungi, or viruses are associated with pleural effusions. The effusions generally are small and ipsilateral to the parenchymal infiltrate. Usually, the pleural fluid is a serous exudate with a predominance of mononuclear cells. The pleural fluid glucose and, presumably, the pleural fluid pH are not low. The etiologic organism has been isolated from pleural fluid but usually is not necessary to establish the diagnosis. Pleural biopsy is not helpful in diagnosis with the exception of acute coccidioidal pneumonia and effusion. Pleural

Fibronectin in exudative pleural effusions.

Fibronectin is a glycoprotein found in body fluids, loose connective tissue matrix and in basement membranes. Fibronectin in pleural effusion was found to be immunologically indistinguishable from the plasma form, as shown by double-diffusion analysis. Fibronectin isolated from pleural fluid by affinity chromatography on gelatin-Sepharose had a polypeptide pattern similar to that of plasma fibronectin in SDS-polyacrylamide gel electrophoresis. In 28 patients with infectious or non-specific pleural effusion fibronectin concentrations in pleural fluid were 335 +/- 104 micrograms/ml (mean +/- SD)
